Social problem solving in children with fetal alcohol spectrum disorders.

BACKGROUND: Children with fetal alcohol spectrum disorders (FASD) show impairments in social functioning. However, the factors underlying these impairments are poorly understood. Recent evidence has shown that social problem solving is a critical component of effective social functioning. OBJECTIVES: The present study sought to examine social information processing as one potential factor contributing to social skills and behavior impairments observed in children with FASD. METHODS: Forty-three children, 20 with FASD (mean age 12.6 years) and 23 typically developing controls (TDC; mean age 1…

The role of oxidative stress in fetal alcohol spectrum disorders.

The ingestion of alcohol/ethanol during pregnancy can result in abnormal fetal development in both humans and a variety of experimental anim … ure on the levels of oxidative stress in the central nervous system (CNS) of experimental models of fetal alcohol spectrum disorders (FASD). We will also review the evidence for the use of antioxidants as potential therapeutic strategies for the treatment of some of the neuropathological deficits characteristic of both rodent models of FASD and children afflicted with these disor …

Fetal alcohol spectrum disorders--a case-control study from India.

BACKGROUND: Maternal alcohol abuse during pregnancy can lead to fetal neurotoxicity and fetal alcohol spectrum disorder (FASD). AIMS: To compare the clinical features and neurobehavioral profiles of children exposed to alcohol during pregnancy with controls. METHODOLOGY: Children exposed to alcohol in utero (n = 26) and 27-years age- and sex-matched controls were compared on FAS facial features, minor physical anomalies (MPAs), anthropometric measures, behavioral problems and intellectual functioning. RESULTS: MPAs were more common in cases (p = 0.001). Among FAS facial features, only philt

Biobehavioral markers of adverse effect in fetal alcohol spectrum disorders.

Identification of children with fetal alcohol spectrum disorders (FASD) is difficult because information regarding prenatal exposure is often lacking, a large proportion of affected children do not exhibit facial anomalies, and no distinctive behavioral phenotype has been identified. Castellanos and Tannock have advocated going beyond descriptive symptom-based approaches to diagnosis to identify biomarkers derived from cognitive neuroscience. Classical eyeblink conditioning and magnitude comparison are particularly promising biobehavioral markers of FASD-eyeblink conditioning because a deficit…

Fetal alcohol spectrum disorders: Extending the range of structural defects.

Although the structural phenotype of fetal alcohol syndrome (FAS) is established, prenatal exposure to alcohol may produce a broader spectrum of defects, fetal alcohol spectrum disorder (FASD). Documenting the full spectrum of defects associated with FASD is critical to determining the true incidence of this disorder. We examined 831 children from the Collaborative Initiative on Fetal Alcohol Spectrum Disorders using a structured protocol for diagnosis of FAS using the cardinal facial and growth features, and assessment of additional structural defects thought to occur more often in children w…

Neuropsychological characteristics of Italian children with fetal alcohol spectrum disorders.

BACKGROUND: Children with fetal alcohol spectrum disorders (FASD) display many problems ranging from deficits in intelligence to behavioral difficulties. Thus, many studies have aimed at defining the neuropsychological characteristics of children with FASD. The current article describes the neuropsychological characteristics of Italian children with severe diagnosis within FASD and compares them with controls. It was expected that intellectual functioning, language comprehension, academic skills, and inattention/hyperactivity would discriminate children with FASD from randomly selected peers w…

Characterization of white matter microstructure in fetal alcohol spectrum disorders.

BACKGROUND: Exposure to alcohol during gestation is associated with CNS alterations, cognitive deficits, and behavior problems. This study i … . CONCLUSIONS: These data provide an account of cerebral white matter microstructural integrity in fetal alcohol spectrum disorders and support extant literature showing that white matter is a target of alcohol teratogenesis. The white matter anomalies characterized in this study may relate to the neurobehavioral sequelae associated with gestational alcohol exposure, especially …

Fetal alcohol spectrum disorders: a practical clinical approach to diagnosis.

In utero exposure to alcohol can have numerous adverse effects on a developing fetus. These effects represent a spectrum of structural anomalies and neurocognitive and behavioral disabilities that have recently been termed fetal alcohol spectrum disorders (FASD). Children at the most severe end of this spectrum and displaying the complete phenotype of characteristic facial anomalies, growth retardation and developmental abnormalities of the central nervous system are defined as having fetal alcohol syndrome (FAS). While FAS is the most readily clinically recognized form of FASD, other categori…

Sobering thoughts: town Hall meetings on fetal alcohol spectrum disorders.

Prenatal exposure to alcohol is one of the leading causes of preventable birth defects and developmental disabilities. During the past 30 years, fetal alcohol spectrum disorders (FASD), including fetal alcohol syndrome, have gradually begun to attract attention. However, awareness and understanding of the disorders remain low, and people who are affected are seriously underserved. The FASD Center for Excellence held a series of town hall meetings in 2002 and 2003 to gauge the issues surrounding FASD nationwide. On the basis of its findings, the center proposed a series of recommendations to be…

Alcohol-induced behavioural problems in fetal alcohol spectrum disorders versus confounding behavioural problems.

Prenatal alcohol exposure is strongly associated with disruptive behaviour in childhood and antisocial behaviour later in life. There are numerous confounding risk factors in the lives of alcohol-abusing mothers that may contribute to the behaviour problems seen in their children, rather than direct brain injury by alcohol. In fact, many of these additional environmental and genetic risk factors for childhood behaviour problems co-occur with prenatal alcohol exposure and affect the same child, creating a confluence of risk. As a result, one cannot with any certainty attribute behaviour problem

Fetal alcohol spectrum disorders and fetal alcohol syndrome: the state of the art and new diagnostic tools.

Ethanol consumption during pregnancy is a widespread problem which is increasing in the generation of young women. Gestational alcohol consumption causes fetal exposure to this teratogen and is associated with the onset of fetal alcohol spectrum disorders (FASD) including fetal alcohol syndrome (FAS). FASD and FAS can lead to several physical, cognitive and behavioral disabilities, whose early diagnosis is of primary importance to perform primary prevention with total abstinence from alcohol during pregnancy and secondary prevention in newborns and children for a proper follow up to reduce ris…

Estimating the Prevalence of Fetal Alcohol Spectrum Disorder in Australia.

INTRODUCTION: Fetal alcohol spectrum disorder (FASD) is caused by prenatal alcohol exposure (PAE) and characterised by severe neurodevelopmental impairment. Australian studies have reported PAE prevalence of between 14% and 78% of births. Estimating national FASD prevalence in the general population using gold-standard active case ascertainment is costly and time-consuming, and alternative approaches are required. METHODS: Using a published equation for the risk of FASD following PAE (estimated from an international meta-analysis) and a pooled estimate of PAE prevalence in Australia (from a m

Lived experiences of adolescents with fetal alcohol spectrum disorder.

AIM: To explore the lived experiences of adolescents with fetal alcohol spectrum disorder (FASD) to understand: the ways in which their challenges influence daily functioning; and the personal assets and external resources that facilitate participation. METHOD: Semi-structured interviews were conducted with four adolescents (one identified as non-binary, one male, and two female) with FASD aged 13 to 15 years. A photo elicitation approach facilitated participation, whereby adolescents first took photographs of their daily activities and environments. Photographs were subsequently used to gene

Utility of Genetic Testing in Fetal Alcohol Spectrum Disorder.

OBJECTIVE: To study the utility of genetic evaluation and testing in patients with suspected fetal alcohol spectrum disorder (FASD). STUDY DESIGN: We performed a retrospective chart review of all patients (n = 36) referred for evaluation for suspected FASD to the genetics clinic at Boston Children's Hospital between January 2006 and January 2013. Records of all patients were reviewed to obtain the medical history, family history, examination findings, and investigations, including genetic testing. RESULTS: Of the 36 patients, definite prenatal exposure was documented in 69%. Eight patients d
