Primary transection of the superficial peroneal nerve resulting from a distal fibula fracture.

Although a superficial peroneal nerve injury following an ankle fracture occurs frequently, primary transection of the nerve represents a rare injury. This report documents a case of primary lesion of the superficial peroneal nerve sustained following a Weber B bimalleolar ankle fracture. The nerve injury was diagnosed at the initial examination when the patient was found to have hypoesthesia in the area of her third to fifth toes. Subsequently an open reduction and internal fixation of the ankle fracture was done along with primary suture of the superficial peroneal nerve. At the patient's la

A new technique for complex fibula fracture fixation in the elderly: a clinical and biomechanical evaluation.

OBJECTIVE: To determine whether intramedullary fixation could augment plate fixation strength in comminuted and osteopenic fibula fractures. STUDY DESIGN: Retrospective clinical study and biomechanical laboratory study. METHODS: Twenty comminuted or osteopenic fibula fractures in twenty patients age fifty years or older were stabilized using plate fixation augmented with intramedullary Kirschner wires. Nineteen patients were available for follow-up which averaged 15.4 months (range, 6-43 months). In conjunction with this clinical series, a biomechanical evaluation was performed comparing fix…

The short oblique fracture of the distal fibula without medial injury: an assessment of displacement.

Eighteen patients with ankle injuries presenting as short oblique fractures of the distal fibula with no clinical or radiographic evidence of injury to the medial ankle were studied for fracture displacement. Plain radiographs and computed tomography were used for analysis. All fractures were clinically diagnosed as supination-external rotation stage 2 (SE-II) injuries under the Lauge-Hansen scheme. All exhibited slight displacement on plain radiographs and were treated nonoperatively. Computerized tomography using axial cuts across the fracture site and ankle mortise revealed normal positioni

[Treatment of the open tibial fracture with the fixateur externe with special reference to osteosynthesis of the fibula].

In the treatment of open fractures of the lower limb and of fractures combined with severe damage of soft tissues, external fixation is favoured. Additional soft tissue damages in the compartments of the lower limb are avoided by using an external fixation in form of a clamp. In defect fractures, shattered fractures and in all fractures in the distal third the stability of clamp fixation is enhanced by plating of the fibula. Furthermore by osteosynthesis the reconstruction of the correct length of the lower limb can be performed. 36 fractures healed definitively in an average of 17.1 weeks by

Expert Tibia Nail: Is a Single Nail Enough to Fix Distal Tibia and Fibula Fractures?

Background  Management of extra-articular distal tibia fractures associated with supra-syndesmotic fibula fractures remains controversial, particularly regarding the need for fibular fixation. With the advent of the Expert Tibial Nail System (ETNS), which provides multidirectional distal locking, adequate stability may be achieved without fibular fixation, potentially reducing surgical morbidity. Objective  To compare clinical, radiological, intra-operative parameters, and complication rates between isolated ETNS fixation and conventional tibial intramedullary (IM) nailing with fibular plating…

Comparison of tibiofibular syndesmosis stability following treatment of proximal, middle, and distal third fibula fractures.

PURPOSE: While treatment modalities for Maisonneuve fractures involving the proximal third of the fibula are established, no studies to date have reported outcomes associated with syndesmotic-only fixation of middle third fibular shaft fractures. The purpose of this study was to evaluate outcomes associated with syndesmotic-only fixation in the treatment of Maisonneuve fractures involving the middle third of the fibula. METHODS: A retrospective review was conducted on 257 cases of syndesmotic ankle instability with associated fibular fractures at a level 1 trauma center between 2013 and 2023.

Treatment of the tibial pilon fractures using the antero-medial fibula approach: Ten case series.

Pilon fractures are always results of the high-energy trauma. They are often accompanied with serious soft tissue injury, and tension blisters happened in most cases. For comminuted fractures and poor soft tissue, how to select the incision is challenging. This study aimed to explore the outcomes of the treatment of ten 43-B/C pilon fractures using an anteromedial fibula approach.Ten closed pilon fractures combined with fibula fractures were treated in our hospital from January 2015 to July 2016. Six cases were AO/OTA type 43-B and 4 cases were 43-C, including 9 males and 1 female with a mean …

Evaluation and Significance of Mortise Instability in Supination External Rotation Fibula Fractures: A Review Article.

UNLABELLED: Evaluation and management of ankle fractures has progressed in parallel to an evolving understanding of ankle stability. While stability of the mortise had historically been attributed to the lateral malleolus, Lauge-Hansen's contributions followed by multiple other investigations increased the emphasis on the significance of medial-sided injury in destabilizing the mortise. As the importance of the deltoid ligament has been elucidated, the means of assessing ligamentous incompetence and the prognostic significance of an unstable mortise continue to be defined. LEVEL OF EVIDENCE:

Hindfoot Valgus following Interlocking Nail Treatment for Tibial Diaphysis Fractures: Can the Fibula Be Neglected?

Purpose. We evaluated whether intramedullary nail fixation for tibial diaphysis fractures with concomitant fibula fractures (except at the distal one-third level) managed conservatively with an associated fibula fracture resulted in ankle deformity and assessed the impact of the ankle deformity on lower extremity function. Methods. Sixty middle one-third tibial shaft fractures with associated fibular fractures, except the distal one-third level, were included in this study. All tibial shaft fractures were anatomically reduced and fixed with interlocking intramedullary nails. Fibular fractures …

Free vascularized fibula grafts for salvage of failed oncologic long bone reconstruction and pathologic fractures.

Vascularized bone grafts, particularly the free fibula transfer, have incited revolutionary changes in the field of skeletal reconstruction. In no place has this been more evident than in oncologic reconstruction. The free vascularized fibula graft has been used to good effect for primary long-bone reconstruction, long-bone allograft complication salvage, and pathologic fracture salvage of the long bones. Although many of these procedures often entail significant complications, limb salvage has been made possible in a majority of patients using transfers of free vascularized fibula grafts. The

Simultaneous obliteration and treatment of infected frontal sinus fractures: novel use of the fibula flap.

We present two cases of infected frontal sinus fractures requiring simultaneous obliteration of the frontal sinus and reconstruction of the horizontal buttress. In both cases, a free fibula flap was used for obliteration and bony reconstruction in a single stage. We believe that this is a novel technique for treatment of infected frontal sinus fractures obviating the need for secondary staged procedures.

Chronic, painful ankle instability in skeletally immature athletes. Ununited osteochondral fractures of the distal fibula.

We report 10 cases of chronic, painful ankle instability in skeletally immature athletes associated with ununited osteochondral fragments of the distal fibula. All the patients experienced multiple inversion injuries with recurrent pain and instability that failed to improve after a supervised exercise and rehabilitation program. Osteochondral fragments of the distal fibula were visible on all standard radiographs of the affected ankles. Treatment consisted of operative excision of the bony fragment and an anatomic nonaugmented repair of the lateral ligament complex and capsule. All 10 patient

Fibula nail fixation in ankle fractures with significant soft tissue compromise: a retrospective cohort study.

PURPOSE: Intramedullary fixation of lateral malleolar fractures has increased in popularity recently with the introduction of the fibula nail. It has been proposed as an alternative fixation method in fractures to minimise soft tissue injury. The aim of this study was to evaluate the clinical and patient-reported outcomes of those who had an ankle fracture with concurrent significant soft tissue damage, treated with a fibula nail. METHODS: Details of patients who were managed at our institution using a fibula nail were obtained from the trauma database. The Acumed Fibula Rod System (FRS) was

Reinforced fixation of distal fibula fractures in elderly patients; A meta-analysis of biomechanical studies.

BACKGROUND: There is an increasing incidence of fragility fractures of the ankle in the elderly population. The open reduction and internal fixation of these fractures is challenging, due to reduced bone stock quality as a result of osteoporosis. Biomechanical studies have shown contradicting results using reinforced constructions in the fixation of fibular fractures. We therefore performed a meta-analysis of biomechanical studies on reinforced fixation of distal fibular fractures. METHODS: A literature search was conducted utilizing three online databases considering biomechanical testing of

Biomechanical evaluation of mini-fragment hardware for supination external rotation fractures of the distal fibula.

BACKGROUND: Supination external rotation distal fibula fractures are common, requiring fixation when associated with talar displacement. Subcutaneous distal fibula hardware may become painful, necessitating operative removal. We hypothesize that mini-fragment and small-fragment constructs will demonstrate similar biomechanical stability. METHODS: A biomechanical comparison was performed in synthetic osteoporotic sawbones. The first arm compared two 2.4-mm lag screws with one 3.5-mm lag screw for fixation of a simulated supination external rotation distal fibula fracture. The second arm compar…

[Septic leg fractures. Value of cancellous bone grafting without skin closure, aligned on the fibula].

The authors have treated 63 septic fractures of the tibia, 29 of whom were seen less than four months after the injury; the remainder were more long-standing. In both series the surgeon faced three problems--curing of the septic drainage, skin cover and bone union. In 46 cases bone excision was considered necessary: in 18 diaphyseal resection was performed. Immobilisation of the fracture was obtained by plaster cast in 14 cases, an external fixator in 48 cases, and medullary nailing in one. Reconstruction was needed in 11 cases after closed grafting, 9 of them being tibio-fibular, and in 46 af

Stabilisation of the fractured fibula plays an important role in the treatment of pilon fractures: a retrospective comparison of fibular fixation methods.

Ninety-eight pilon fractures associated with ipsilateral distal fibular fracture were included in this study. The pilon fractures were treated by open reduction and plating. The 98 fractures were divided into three groups based on the treatment method of fractured fibula. Group A was composed of 50 fibular fractures treated by open reduction and plate fixation. Group B was composed of 23 fibular fractures treated by open reduction and pin fixation. Group C was composed of 25 fibular fractures treated conservatively by closed reduction. The radiographs were reviewed for adequacy of fracture red

The treatment of unstable fractures of the tibia and fibula with flexible medullary wires. A review of two hundred and thirty-five fractures.

A retrospective study was done of the treatment of closed and open unstable fractures of the tibia and fibula with flexible intramedullary wires in 223 patients with 235 fractures. The surgical procedure is relatively atraumatic to the tibia, can be learned easily by the surgeon, and does not require complicated or expensive instrumentation. The flexible intramedullary wires permit stabilization of the fractures in a functional position. External support by a long cast in the initial phase of treatment is necessary, but full weight-bearing is usually allowed during the third to fourth week. Th

Management, reconstructive strategies and outcomes following open tibia/fibula and ankle fractures: An age-based comparison.

INTRODUCTION: Open lower limb fractures require multidisciplinary orthoplastic management. With an ageing population, older patients present with distinct injury patterns, greater comorbidity and reduced physiological reserve. In this study, we compared presentation, management and outcomes between younger and older patients. METHODS: A 4-year retrospective complete case cohort study (2018-2022) was conducted at a tertiary major trauma centre. Adult patients with open tibia, fibula or ankle fractures were included. Patients were stratified into younger adult (<65 years) and older adult (≥65 y
