Assessing a decade of public health preparedness: progress on the precipice?

September 11 and the subsequent anthrax attacks marked the beginning of significant investment by the federal government to develop a national public health emergency response capability. Recognizing the importance of the public health sector's contribution to the burgeoning homeland security enterprise, this investment was intended to convey a "dual benefit" by strengthening the overall public health infrastructure while building preparedness capabilities. In many instances, federal funds were used successfully for preparedness activities. For example, electronic health information networks,

Financing the transition: a roadmap for sustaining disease control in low- and middle-income countries after the 2025 contraction in development assistance for health.

In 2025, development assistance for health fell by about 21% in a single year, driven largely by a 67% reduction in United States financing, disrupting HIV, tuberculosis, malaria, immunization and maternal health programs across low- and middle-income countries. Modelling indicates millions of avoidable deaths by 2030 if the resulting gap is not filled, yet most recipient governments have not reprioritized budgets or raised domestic revenue in response. This brief argues that the contraction is structural rather than cyclical and sets out a sequenced transition-financing roadmap. It proposes p

Climate-related health threats in Tanzania: a multilevel analysis of key policies, strategies, and community response.

Tanzania faces increasing climate-sensitive health threats, including vector-borne diseases, waterborne infections, and malnutrition. Effective preparedness at both national and community levels is critical for health system resilience and livelihood security. This study applied a multilevel mixed-methods approach to assess Tanzania's policy structures and community-level experiences related to climate-health preparedness. Between January 2024 and May 2025, we conducted a concurrent mixed-methods study. At national level, we reviewed climate-health policy documents, mapped 30 key stakeholders,

From pollution to finance: Linking marine pollution pathways and blue finance for coastal resilience in South Asia.

South Asia's marine and coastal environments face escalating pressures from industrialization, urbanisation, and climate variability, yet policy responses remain fragmented and underfunded. This review synthesises sources, pathways, and impacts of marine pollution and advances a framework that integrates scientific assessment with blue finance mechanisms tailored to the region's socio-ecological realities. It considers both legacy contaminants (heavy metals, hydrocarbons, nutrients) and emerging threats (microplastics, pharmaceuticals, nanomaterials), linking them to governance and financial r

Mobile phone health risk policy in Germany: the role of the federal government and the Federal Office for Radiation Protection.

In order to establish a regulatory framework for a given technology important to society, the government must make decisions in the face of existing unknowingness. In the last decade, health risks originating from electromagnetic fields of mobile telecommunication transmitting stations and devices have become a regulation policy issue in Germany. This article investigates the role of the government and the Federal Office for Radiation Protection in regard to policy-making by analysing publications and Federal Parliament reports, hearings and debates. The government and Federal Parliament perce

Implementing effective cataract surgical coverage: a comparative qualitative study in Kenya and Nepal.

INTRODUCTION: The WHO has set a 2030 target to raise effective cataract surgical coverage (eCSC) by 30 percentage points in every country, requiring gains in surgical access and quality. Despite this mandate, evidence on how low- and middle-income health systems are implementing eCSC remains limited. METHODS: We conducted a qualitative comparative case study in Kenya and Nepal. 20 interviews were held with senior stakeholders from government, non-governmental organisations, academic institutions and clinical networks. Transcripts were thematically analysed using the consolidated framework for

Healthcare financing systems for increasing the use of tobacco dependence treatment.

BACKGROUND: Tobacco smoking is the leading preventable cause of death worldwide, which makes it essential to stimulate smoking cessation. The financial cost of smoking cessation treatment can act as a barrier to those seeking support. We hypothesised that provision of financial assistance for people trying to quit smoking, or reimbursement of their care providers, could lead to an increased rate of successful quit attempts. This is an update of the original 2005 review. OBJECTIVES: The primary objective of this review was to assess the impact of reducing the costs for tobacco smokers or healt

G.A.T.S. and universities: implications for research.

The likely impact of applying the General Agreement on Trade in Services (GATS) to higher education are examined. GATS aims to "open up" services to competition: no preference can be shown to national or government providers. The consequences for teaching are likely to be that private companies, with degree-awarding powers, would be eligible for the same subsidies as public providers. Appealing to the inadequate recently introduced "benchmark" statements as proof of quality, they would provide a "bare bones" service at lower cost. Public subsidies would go: education being reduced to that mini

The economic costs and its predictors for childhood autism spectrum disorders in Ireland: How is the burden distributed?

Autism spectrum disorders are associated with a substantial economic burden; although little is known about the relationship between state and family out-of-pocket expenditure. The objective of this study is to estimate the societal cost of childhood autism spectrum disorders and explain the variation in costs between state and family out-of-pocket expenditure. A bottom-up prevalence based cost-of-illness methodology was implemented using data from a combination of multiple convenience samples in Ireland of 195 parents of 222 children aged between 2 and 18 years of age with a clinically diagno

Engaging the private sector to increase access to HIV services and increase sustainability of the HIV response.

BACKGROUND: Public health facilities often manage high volumes of HIV clients and face challenges with long wait times, poor client satisfaction, and low retention in care. The private sector is a major provider of health services in many countries but is a relatively untapped resource for reducing congestion in high-volume public HIV facilities and increasing access to HIV services, particularly for clients who already use the private sector as their first point of care. Private banks, corporations, and clients themselves can also be a resource for generating additional financing for HIV and

Medicare, Medicaid, and Maternal and Child Health Services Block Grant programs; civil money penalties and assessments for false or improper claims--Department of Health and Human Services. Notice of proposed rulemaking.

These proposed regulations are intended to strengthen the Department's ability to protect the health care financing programs against persons and organizations who defraud and abuse those programs. The regulations would specify procedures for implementing the authority provided to the Department by section 2105 of the Omnibus Budget Reconciliation Act of 1981 (Pub. L. 97-35), as amended by section 137(b)(26) of the Tax Equity and Fiscal Responsibility Act of 1982 (Pub. Towards distinctions-based Indigenous health workforce reform in Canada: A comparative policy analysis.

BACKGROUND: Indigenous health workforce reform in Canada remains shaped by colonial jurisdictional fragmentation, Indigenous-specific racism, and underinvestment. Although cultural safety initiatives and recruitment supports have expanded, reforms remain uneven and poorly coordinated. OBJECTIVE: To compare recent Indigenous health workforce policy instruments in Canada and identify persistent gaps and policy directions for distinctions-based, culturally rooted reform. METHODS: We conducted a structured narrative policy review of publicly available legislation, strategies, and professional/re

Strengthening mental health care in Somalia through artificial intelligence: opportunities, challenges, and a roadmap for implementation.

Mental health needs in Somalia are substantial, yet access to timely, high-quality care remains constrained by workforce shortages, limited infrastructure and financing, fragmented services, weak health information systems, and persistent sociocultural barriers. Artificial intelligence (AI) may complement mental health system strengthening in this fragile and resource-constrained setting. This Perspective examines potential applications of AI across Somalia's mental health system and considers the associated implementation, ethical, governance, and equity challenges. Potential applications inc

Can Universal Health Coverage Be Achieved Through an Unregulated Private Health System? Somalia's Policy Paradox.

Universal Health Coverage (UHC) aims to ensure equitable access to quality healthcare without financial hardship. Somalia presents a critical test case for UHC implementation in fragile and conflict-affected settings due to decades of state collapse, weak governance, and heavy reliance on an unregulated private health sector. This commentary examines Somalia's policy paradox: the coexistence of formal commitment to UHC alongside a predominantly market-driven, donor-dependent, and weakly regulated healthcare system. Drawing on evidence from Somalia and comparable low-income settings, the articl

Effect and illustrative costs of downscheduling on government subsidised dispensings of triptans in Australia: a 10-year analysis.

Objective Acute migraine episodes affect >18% of Australians. Triptans are most effective when taken early in migraine attacks, making timely access critical. Before February 2021, triptans were prescription-only, but a downscheduling policy change enabled over-the-counter (OTC) purchase with pharmacist advice. This study examined the effect on subsidised Pharmaceutical Benefits Scheme (PBS) dispensing rates and illustrative patient costs. Methods Aggregated PBS dispensing data were used to estimate monthly triptan dispensing rates per 1000 people using population data from the Australian Bure

Inclusion of abortion-related care in national health benefit packages: results from a WHO global survey.

INTRODUCTION: Service inclusion in a country's health benefit package (HBP) is an important milestone towards universal health coverage. This study aimed to explore HBP inclusion of abortion interventions globally. METHODS: Secondary analysis of the WHO HBP survey, in which officially nominated survey focal points were asked which interventions were included within the HBP of their country or area's largest government health financing scheme. Abortion inclusion was compared by region, income, legal status of abortion and HBP design process variables. Abortion inclusion was compared with other

Radical actions to address UK organ shortage, enacting Iran's paid donation programme: A discussion paper.

Globally there is a shortage of organs available for transplant resulting in thousands of lives lost as a result. Recently in the United Kingdom 457 people died as a result of organ shortage in just 1 year. 1 NHS Blood and Transplant suggest national debates to test public attitudes to radical actions to increase organ donation should be considered in addressing organ shortage. The selling of organs for transplant in the United Kingdom is prohibited under the Human Tissue Act 2004. This discussion paper considers five ethical objections raised in the United Kingdom to paid donation and discuss
