[Corrective saccades in patients with multiple sclerosis].

Basing on the data analysis of natural and erroneous fast eye movements, the quantitative estimation of static and dynamic features of corrective saccades in patients with multiple sclerosis has been carried out. The differences in latency, maximal velocity and other parameters of corrective movements are used in the discussion of brain strategies on the development of normal and corrective eye movements and neuronal pathways for their realization.

Tracking in 3-D space under natural viewing condition.

To track a small visual target in 3-D space, the two eyes have to move in different directions and/or at different velocities. This tracking might be accomplished by a disjunctive pursuit system, which uses separate motion processing of each individual eye but no disparity signal (hypothesis 1), or by the conjugate pursuit and the vergence system (hypothesis 2). To test the validity of the two hypotheses we recorded eye movements in five healthy human subjects with the scleral search-coil method. A small dim laser stimulus was presented on an earth horizontal platform. A position-ramp stimulus

Contralateral conjugate eye deviation during deep brain stimulation of the subthalamic nucleus.

OBJECT: Deep brain stimulation of the subthalamic nucleus (STN) in patients with Parkinson disease is often very effective for treatment of debilitating motor symptoms. Nevertheless, the small size of the STN and its proximity to axonal projections results in multiple side effects during high-frequency stimulation. Contralateral eye deviation is produced in a small percentage of patients, but the precise mechanism of this side effect is at present poorly understood. METHODS: Contralateral eye deviation was produced by high-frequency stimulation of 22 contact sites in nine patients undergoing

[Case of ocular neuromyotonia with transient and gaze-induced diplopia].

A 71-year-old man had recurrent spells of transient diplopia for about four years. Vertical diplopia were induced by downward gaze, mostly to the lower-left, and lasted for about ten seconds. During the spells, limitation in elevation of the left eye was observed. Other neurological examinations were normal except for a minor paresis of the left trochlear nerve revealed only by detailed neuroophthalmological examination. The patient was treated with carbamazepine 200 mg daily, which abolished the spells. He was diagnosed to have idiopathic ocular neuromyotonia of the left third or fourth nerve

Oculomotor biofeedback therapy.

Biofeedback therapy refers to the process of gaining voluntary control over some bodily function by the immediate use of information regarding its physiological state. In this paper we review the use of oculomotor biofeedback therapy in three common ocular disorders: nystagmus, strabismus, and amblyopia. Experimental and clinical test results have been encouraging. We believe oculomotor biofeedback therapy should be attempted, either alone or in conjunction with orthoptic and/or surgical procedures, in these and other ocular disorders manifesting an abnormal oculomotor component.

Binocular vision in strabismic humans with alternating fixation.

Grating acuity, interocular suppression and binocular interaction were evaluated at different positions in the visual field of strabismic humans with alternating fixation. In all cases, the suppression of the deviated eye was most intense in a region corresponding to the fovea of the fixating eye, but reduced or absent in the periphery. In addition, suppression was found in part of the peripheral visual field of the fixating eye. For most subjects, the monocular properties of both eyes were normal. However, for one subject with a strong preference for one of the eyes, visual acuity and luminan

Effects of luminance and stimulus distance on accommodation and visual resolution.

Measurements of accommodation and visual resolution were obtained at a number of luminance levels and stimulus distances. With reductions in luminance the eye approached a "fixed-focus" condition of accommodation for intermediate distances, resulting in successively larger errors in accommodation for both near and far stimuli. The visual resolution values were initially affected by both the luminance and distance of the stimulus. Subsequent measurements of resolution, following the correction of accommodative errors, were found to be independent of the stimulus distance. The findings are discu

Resection of both medial rectus muscles in organic convergence insufficiency.

Six patients with a prolonged history of severe muscular asthenopia caused by persistent convergence insufficiency underwent resection of both medial rectus muscles. A temporary consecutive esotrapia occurred in each instance and was controlled with prisms. Five weeks after surgery the consecutive esotropia had disappeared in all but one patient who still required prismatic correction for distance fixation. All patients experienced relief from their symptoms. Whereas orthoptic training is useful in most cases of convergence insufficiency a resection of both medial rectus muscles should be cons

An investigation into the mechanisms causing antipodean strabismus.

BACKGROUND: Antipodean strabismus is a rare clinical entity in which a patient manifests an esotropia when fixating with one eye and an exotropia when fixating with the other eye. It has been described in the settings of marked uncorrected anisometropia, dissociated strabismus or combinations of paresis, and mechanical restriction of eye movement. METHODS: A retrospective review of four patients with antipodean strabismus. RESULTS: All patients demonstrated a unique and uncommon pattern in which there was esotropia with one eye fixating and exotropia with the other eye fixating. None of the

Exploration of the functional consequences of fixational eye movements in the absence of a fovea.

A recent theory posits that ocular drifts of fixational eye movements serve to reformat the visual input of natural images, so that the power of the input image is equalized across a range of spatial frequencies. This "spectral whitening" effect is postulated to improve the processing of high-spatial-frequency information and requires normal fixational eye movements. Given that people with macular disease exhibit abnormal fixational eye movements, do they also exhibit spectral whitening? To answer this question, we computed the power spectral density of movies of natural images translated in s

Quantitative comparison of open reduction and internal fixation versus the Gillies method in the treatment of orbitozygomatic complex fractures.

BACKGROUND: Precise repair of orbitozygomatic complex fractures is essential for proper re-establishment of facial symmetry, ocular globe position, and infraorbital nerve function. Controversy regarding the optimal treatment method remains. METHODS: To compare uniform study groups, only patients without previous craniofacial injuries or operations who had sustained moderate-energy orbitozygomatic complex fractures, based on preoperative computed tomography scans, and who were treated using the Gillies repair or open reduction and internal fixation were selected. Quantifiable end-points, inclu

OculoMotor Assessment Tool Test Procedure and Normative Data.

SIGNIFICANCE: This study establishes normative data and a testing procedure for the oculomotor assessment tool. The oculomotor assessment tool standardizes visual targets for the Vestibular/OculoMotor Screening assessment and provides additional metrics that may aid in the differentiation between those with normal and those with abnormal oculomotor function potentially caused by a concussion. PURPOSE: This study aimed to assess the oculomotor endurance of healthy participants with no self-reported history of concussions using the oculomotor assessment tool. METHODS: Healthy participants (n =

Effect of 3-Dimensional Central Stimuli on Near Point of Convergence.

AIM: Since an interaction between binocular disparity and the vergence system takes place in order to reduce retinal disparity, it is likely that vergence control, as measured with the near point of convergence (NPC), will be better if the target used for measurement has 3-dimensional (3-D) features in its most central parts. The aim of the present study was therefore to investigate if the NPC would give a better result using a fixation target with centrally placed 3-D features when compared with a two-dimensional (2-D) target. METHODS: Twenty-three asymptomatic subjects (2 men, 21 women; ave

Refractive astigmatism acts predominantly as a source of high spatial frequency image distortion: the associated lineal distortions can be overcome by using a low pass spatial filter!

PURPOSE: Surprisingly, an important characteristic of astigmatism has been overlooked by ophthalmic and clinical scientists. Apparently, refractive astigmatism is due largely to a form of high spatial frequency image distortion. METHODS: Characteristic astigmatic image distortion can be minimized or eliminated by using a low-pass spatial filter (here, a ground glass plate was employed for this purpose). The ground glass is placed a short distance in front of a visual acuity chart, or it may be used with other tests, such as vernier acuity. RESULTS: This test has been performed by us on large

Binocular fixation misalignment measured by border enhancement: a simplified technique.

In a previous study, a new method of monitoring binocular fixation performance was presented. The method is based upon the effect of fixation eccentricity on border enhancement: that is, the width of the enhancement band increases consistently with increasing distance of the border stimulation from the foveal center. Unlike the conventional methods of measuring fixation disparity, the procedure is essentially independent of interocular nonius alignment. However, there are some difficulties inherent in the interpretation of measurements of binocularly presented border stimulation, for it is not

Scanning laser ophthalmoscope fundus perimetry after surgery for choroidal neovascularization.

PURPOSE: Submacular surgery is under investigation for the treatment of subfoveal choroidal neovascularization secondary to age-related macular degeneration, ocular histoplasmosis syndrome, and other causes. The aims of this study were to determine whether the macular area from which choroidal neovascularization was removed surgically remained functional and whether there was any qualitative difference between eyes with different disease conditions or between eyes of younger and older patients. METHODS: Our study included 19 patients (19 eyes) with choroidal neovascularization, seven cases ca

Humoral and cellular immune response in ocular histoplasmosis.

We have compared several humoral and cellular immune responses in ocular and in acute histoplasmosis syndromes. T-lymphocyte subset analysis revealed elevated ratios of T-helper/T-suppressor lymphocytes in ten of 22 patients with ocular histoplasmosis, whereas ratios were depressed in 18 of 22 with acute histoplasmosis syndromes. Lymphocyte transformation responses to mitogens and to histoplasma antigens were similar in both acute and ocular histoplasmosis. However, histoplasma antibody levels as measured by immunodiffusion, complement fixation, and radioimmunoassay were elevated in only three
