We present a unique case of S. marcescens folliculitis of the trunk in a 46-year-old woman with a history of facial acne vulgaris during her teen years. Her eruption occurred at the time of elective ambulatory surgery when she was treated with pre and post-operative antibiotics. BACKGROUND: The scarring follicular disorders pose challenging therapeutic dilemmas.
OBJECTIVE: Hair removal lasers have recently been shown to be efficacious in the management of these disorders.
METHODS: We present a young, Fitzpatrick skin type VI African-American patient with recalcitrant folliculitis decalvans, whom we treated with the neodymium:yttrium aluminum garnet (Nd:YAG) laser.

A 27-year-old male patient presented with scaly erythema and crusts on the scalp. Since birth, he suffered from dry skin and inflammation of the eyelids. Scarring alopecia was noticed in some regions of his scalp. Folliculitis spinulosa decalvans was diagnosed. Therapy with isotretinoin and topical corticosteroids was without effect. A 24-year-old female patient is reported, in whom severe renal insufficiency following renal transplantation for Goodpasture's syndrome was associated with a persistent generalized perforating folliculitis of the trunk and extremities. In addition to the pathologic renal parameters a hypervitaminosis A was found. On the basis of this observation as well as similar recent reports in the literature it is suggested that a subset of an unusually extensive perforating folliculitis may occur with renal insufficiency and, albeit rare, represents a clinical marker for this associated condition.

Malassezia folliculitis (MalF) mimics acne vulgaris and bacterial folliculitis in clinical presentations. The role of Gram staining in rapid diagnosis of MalF has not been well studied. In our study, 32 patients were included to investigate the utility of Gram staining for MalF diagnosis. We report a case of folliculitis decalvans (FD) successfully treated with intravenous human immunoglobulin (HIG). Many conventional treatments with topical agents and oral antibiotics had failed to achieve disease remission, treatment with HIG at a dose of 2 g/kg for the first month, reduced to 1 g/kg for second to fourth months was therefore started, which resulted in rapid improvement and ultimately complete resolution of FD.
