Clinical relevance of the relaxant effects of roflumilast on human bronchus: potentiation by a long-acting beta-2-agonist.

Roflumilast is an oral, add-on option for treating patients with severe COPD and frequent exacerbations despite optimal therapy with inhaled drugs. The present study focused on whether this phosphodiesterase 4 inhibitor and its active metabolite roflumilast N-oxide affect the tone of human bronchial rings. We also investigated the interactions between roflumilast, roflumilast N-oxide and the long-acting β2 -agonist formoterol with regard to the relaxation of isolated human bronchial rings at basal tone or pre-contracted with histamine. Our results demonstrated for the first time that at a clin

Formoterol, a clinically approved drug, inhibits ferroptosis by suppressing lipid peroxidation and attenuates APAP-induced acute liver injury.

Ferroptosis is an iron-dependent form of regulated cell death characterized by lethal lipid peroxidation and implicated in various human diseases. Despite intensive research, clinically applicable ferroptosis inhibitors remain unavailable. In this study, we identify formoterol, a β2-adrenergic agonist widely used to treat asthma and COPD, as a potent and selective ferroptosis inhibitor through scaffold-based screening of FDA-approved drugs. Formoterol confers robust protection against ferroptotic cell death induced by diverse triggers across multiple human and rodent cell lines, functioning in

Serum metabolic profiles reveal the effect of formoterol on cachexia in tumor-bearing mice.

Cancer cachexia is a complex metabolic syndrome that cannot be fully reversed by conventional nutritional support, and leads to the progressive wasting of body tissues, particularly the loss of lean muscle mass. Formoterol, a highly selective β2-adrenoceptor therapeutic drug, gives potential anabolic responses in the context of skeletal muscles and was widely confirmed to possess anti-cachexia effects. However, the possible metabolic pathways and the metabolite changes that initiate and maintain these anabolic responses remain poorly understood. In the present study, a (1)H NMR-metabonomics mo

Development and feasibility of provider implementation tools for anti-inflammatory reliever therapy in community asthma practices.

INTRODUCTION: Anti-Inflammatory Reliever (AIR) therapy-the use of inhaled corticosteroids (ICS)/formoterol or ICS with short-acting beta-agonists as reliever therapy is guideline-recommended for mild asthma. However, its uptake remains low in community settings. To improve AIR adoption, we developed an AIR Asthma Plan (AIR-AP) and implementation strategies to support its adoption, which we then pilot-tested to assess feasibility and acceptability. METHODS: Using an exploratory sequential design, we collaborated with stakeholders and pilot site providers to: (1) assess provider needs, (2) co-d

Dupilumab efficacy and safety in adults with uncontrolled persistent asthma despite use of medium-to-high-dose inhaled corticosteroids plus a long-acting β2 agonist: a randomised double-blind placebo-controlled pivotal phase 2b dose-ranging trial.

BACKGROUND: Dupilumab, a fully human anti-interleukin-4 receptor α monoclonal antibody, inhibits interleukin-4 and interleukin-13 signalling, key drivers of type-2-mediated inflammation. Adults with uncontrolled persistent asthma who are receiving medium-to-high-dose inhaled corticosteroids plus a long-acting β2 agonist require additional treatment options as add-on therapy. We aimed to assess the efficacy and safety of dupilumab as add-on therapy in patients with uncontrolled persistent asthma on medium-to-high-dose inhaled corticosteroids plus a long-acting β2 agonist, irrespective of baseli

The β2-adrenoceptor agonist formoterol attenuates hallmarks of adrenal Cushing's syndrome.

BACKGROUND AND PURPOSE: Signal transduction therapies aim to correct dysregulated cellular signalling pathways implicated in disease. Adrenal Cushing's syndrome, a disorder of excess cortisol production, is frequently driven by somatic mutations in protein kinase A catalytic subunits (PKAc), including PKAcL205R and PKAcW196R. EXPERIMENTAL APPROACH: This study used a combination of in vitro adrenal cell models and an in vivo mouse model harbouring the PKAc W196R mutation to assess the effects of formoterol treatment on mitochondrial function, steroidogenic gene expression and cortisol producti

Pro-inflammatory action of formoterol in human bronchial epithelia.

Despite the wide usage of β2-adrenoceptor agonists in asthma treatment, they do have side effects such as aggravating inflammation. We previously reported that isoprenaline induced Cl- secretion and IL-6 release via cAMP-dependent pathways in human bronchial epithelia, but the mechanisms underlying the inflammation-aggravation effects of β2-adrenoceptor agonists remain pooly understood. In this study, we investigated formoterol, a more specific β2-adrenoceptor agonist, -mediated signaling pathways involved in the production of IL-6 and IL-8 in 16HBE14o- human bronchial epithelia. The effects o

Machine learning-based prediction of aerodynamic performance in arformoterol-lactose dry powder inhaler formulations using surface roughness features.

Dry powder inhalers (DPIs) are widely used for pulmonary drug delivery, and their aerodynamic performance is highly dependent on particle surface morphology. This paper presents a machine learning-based framework to quantitatively predict DPI aerodynamic behavior using surface roughness parameters from scanning electron microscopy (SEM) images. Ten DPI formulations were prepared with various storage conditions and compositions. Thirteen surface roughness features were extracted using ImageJ and SurfCharJ and used as inputs for predictive models, including multilayer perceptrons (MLPs), support

Formulation and evaluation of carrier-based dry powders containing budesonide and arformoterol for inhalation therapy.

Asthma and Chronic Obstructive Pulmonary Disease (COPD) are major global health concerns, with inhalation therapy being a primary treatment method. Dry powder inhalers (DPIs) often face challenges related to particle aggregation, which can diminish drug delivery efficiency. This study investigates particle aggregation and aims to optimize the cohesion-adhesion balance to improve inhalation efficiency. Advanced techniques like atomic force microscopy and Raman imaging were used to analyze particle interactions, focusing on lactose ratios, particle morphology, and drug-drug interactions. The the

Altered control of breathing in a rat model of allergic lower airway inflammation.

Obstructive sleep apnea (OSA) is highly prevalent in patients with asthma. Asthma, dose-dependently to its duration, promotes incident OSA, suggesting that asthma plays a role in OSA pathogenesis. We hypothesized that asthma-related inflammation alters breathing control mechanisms, specifically the carotid chemoreflex. Accordingly, we measured hypoxic ventilatory responses (HVR) in awake, unrestrained, ovalbumin (OVA)-sensitized Brown Norway rats and compared them with responses in sham-sensitized (SALINE) controls. To differentiate the role of allergic inflammation from bronchoconstriction, w

Airway pathology in severe asthma is related to airflow obstruction but not symptom control.

BACKGROUND: Patients with asthma present structural and inflammatory alterations that are believed to play a role in disease severity. However, airway remodeling and inflammation have not been extensively investigated in relation to both symptom control and airflow obstruction in severe asthmatics. We aimed to investigate several inflammatory and structural pathological features in bronchial biopsies of severe asthmatics that could be related to symptom control and airflow obstruction after standardized treatment. METHODS: Fifty severe asthmatics received prednisone 40 mg/d for 2 weeks and ma
