A systematic review on the clinical implementation of 4D particle therapy.

Intrafractional motion in the thoracic and abdominal regions remains a major challenge in particle therapy representing one of the key anatomical changes during radiotherapy. Interfractional changes further contribute to cumulative uncertainties and may require adaptive strategies over the course of treatment. The high dose conformality and steep dose fall-off of particle beams, combined with motion-induced range variations, make dose delivery highly sensitive to temporal and anatomical changes. This systematic review provides an overview of clinically implemented 4D treatment approaches addre

Validation of patient-specific deep learning markerless lung tumor tracking aided by 4DCBCT.

Objective. Tracking tumors with multi-leaf collimators and x-ray imaging can be a cost-effective motion management method to reduce internal target volume margins for lung cancer patients, sparing normal tissues while ensuring target coverage. To realize that, accurate tumor localization on x-ray images is essential. We aimed to develop a systematic method for automatically generating tumor segmentation ground truth (GT) on cone-beam computed tomography (CBCT) projections and use it to help refine and validate our patient-specific AI-based tumor localization model.Approach. To obtain the tumor

[Comparison of Target Phase Positioning with Respiratory Motion between Four-dimensional CT and Four-dimensional Cone Beam CT: A Phantom Study].

PURPOSE: Cone beam computed tomography (CBCT) is the most commonly used technique for target localization in radiation therapy. Four-dimensional CBCT (4D CBCT) is valuable for localizing tumors in the lung and liver regions, where the localization accuracy is affected by respiratory motions. However, in image-guided radiation therapy for organs subject to respiratory motion, position verification is often performed using 3D cone beam CT or 2D X-ray images. While it is possible to collimate tumors at specific respiratory phases during breath-holding and to determine the tumor's motion range by

Longer Length of Delayed-Contrast Filling of Clot on 4-Dimensional Computed Tomographic Angiography Predicts Cardiogenic Embolism.

Background and Purpose- We hypothesized the length of delayed-contrast filling sign (DCFS) of intraarterial clot, indicating contrast medium penetration into the thrombus, was associated with stroke etiology. Methods- We retrospectively included patients with large vessel occlusion in anterior circulation who underwent computed tomographic perfusion within 24 hours poststroke onset. We defined DCFS as contrast medium diffusion through the thrombi after the arterial peak phase on 4-dimensional computed tomographic angiography derived from computed tomographic perfusion. We measured the length o

Diagnosis of giant cell arteritis by head-contrast three-dimensional computed tomography angiography: two case reports.

INTRODUCTION: Temporal artery biopsy is essential for the diagnosis of giant cell arteritis. It has been shown that 18F-fluorodeoxyglucose positron emission tomography-computed tomography, magnetic resonance angiography, and ultrasonography are useful for the diagnosis of giant cell arteritis. However, there are only a few reports on the usefulness of three-dimensional computed tomography angiography in the diagnosis of giant cell arteritis. We describe two cases in which giant cell arteritis was difficult to diagnose using positron emission tomography-computed tomography and magnetic resonanc

4D display of CT LV endocardial and epicardial models morphed from PET Rb-82 perfusion studies accurately quantifies segmental myocardial thickening.

BACKGROUND: MPI-derived LV wall thickening assessments for diagnostic purposes has been part of clinical guidelines for two decades. It relies on visual evaluation of tomographic slices or regional quantification displayed in 2D polar maps. 4D displays have not entered clinical usage nor have they been validated on their potential to provide equivalent information. The purpose of this work was to validate a 4D realistic display recently designed to quantitatively represent the thickening information from gated MPI into CT-morphed endocardial and epicardial moving surfaces. METHODS: Forty pati

Diffusion Model-Based Displacement Field Generation for 4D-CT Chest Image Generation.

Acquisition of data on respiratory motion and anatomical deformation at the individual level is essential for improving the accuracy of surgical procedures and radiotherapy. Although time-series imaging using three-dimensional (3D) computed tomography (CT) and deep learning-based image interpolation have been explored, the acquisition of multiple imaging volumes remains a significant burden on patients because of breath-holding requirements and additional radiation exposure. In this study, we propose a framework for four-dimensional (4D) CT image generation, with images being generated at diff

Use it or lose it: The relationship between two image-based biomarkers in better understanding osteoarthritis progression in the wrist.

Bone tissue is influenced by its mechanical environment and adapts in response to its mechanical load. This is supported by studies analyzing bone adaptation in the knee and hip. Changes to the bone have also been found to precede cartilage degeneration in diseases such as osteoarthritis (OA). Our objective was to demonstrate the relationship between joint contact and bone density in the wrists of healthy adults. Static CT scans with a calibration phantom were taken to obtain measures of bone mineral density (vBMD) in 3 normalized depths; 0 - 2.5, 2.5 - 5, and 5 - 7.5 mm. Participants underwen

Parathyroid Computed Tomography: Pearls, Pitfalls, and Our Approach.

Parathyroid imaging is predominantly used for preoperative localization of parathyroid lesions in patients with the biochemical diagnosis of primary hyperparathyroidism. Although imaging algorithms vary, in the era of minimally invasive parathyroidectomy for single parathyroid adenomas, multiphase parathyroid computed tomography (CT) (4-dimensional CT) has emerged as a favored modality for presurgical mapping of parathyroid lesions. Implementation and correct interpretation of these studies can be challenging, although confidence and accuracy improve with experience and volume. This article re

Parathyroid 4D CT: What the Surgeon Wants to Know.

Parathyroid four-dimensional (4D) CT is an increasingly used and powerful tool for preoperative localization of abnormal parathyroid tissue in the setting of primary hyperparathyroidism. Accurate and precise localization of a single adenoma facilitates minimally invasive parathyroidectomy, and localization of multiglandular disease aids bilateral neck exploration. However, many radiologists find the interpretation of these examinations to be an intimidating challenge. The authors review parathyroid 4D CT findings of typical and atypical parathyroid lesions and provide illustrative examples. Re

Model-based motion simulation framework for deep learning-assisted motion correction in dynamic wrist CT.

Objective.Four-dimensional CT (4DCT) enables visualization of dynamic wrist joint pathology but may be compromised by motion artifacts. Deep learning (DL)-assisted motion correction has the potential to enhance wrist 4DCT quality, but model development is hindered by scarce training data. To address this, we developed a model-based motion simulation framework that generated realistic 4DCT data with paired static ground truth images to demonstrate wrist motion correction using a physics-informed DL model trained with simulated data.Approach.Wrist motion was simulated by synchronizing rotational

Patient-Specific Computational Fluid Dynamics Modeling of Airway Collapse in Infants With Robin Sequence.

Robin Sequence (RS) is a congenital condition in which patients experience dynamic, periodic obstruction or collapse of the upper airway due to an underdeveloped jaw and a posteriorly displaced tongue. Current clinical techniques for evaluating airway obstruction do not provide quantifiable data and fail to account for the dynamic nature of obstruction or collapse. There is no standardized criterion to characterize obstruction or collapse severity. This study presents the first method that extracts airway motion from 4-dimensional computed tomography and performs a patient-specific, moving-mes

Cardiac motion and its dosimetric impact during radioablation for refractory ventricular tachycardia.

INTRODUCTION: Cardiac radioablation (CR) is a noninvasive treatment option for patients with refractory ventricular tachycardia (VT) during which high doses of radiation, typically 25 Gy, are delivered to myocardial scar. In this study, we investigate motion from cardiac cycle and evaluate the dosimetric impact in a cohort of patients treated with CR. METHODS: This retrospective study included eight patients treated at our institution who had respiratory-correlated and ECG-gated 4DCT scans acquired within 2 weeks of CR. Deformable image registration was applied between maximum systole (SYS) a

The use of dynamic CT imaging for tracking mandibular movements in a phantom.

Purpose. The objective of this study was to analyse the possibilities of using 4D CT scanning for the tracking of patients' mandibles.Methods. A clinical 256-slice Revolution CT was used in obtaining 4D CT scans without table movement, with a novel mandibular phantom, mounted on a programmable six degrees-of-freedom Stewart Platform in motion. The phantom was used to simulate mandibular motions which are combinations of rotations with translations (depression, elevation, protrusion, retrusion and laterotrusion). The phantom was scanned five times during identical motion patterns with a dynamic

Individualized breathing trace quality assurance for lung radiotherapy patients undergoing 4DCT simulation.

4DCT simulation is a popular solution for radiotherapy simulation of lung cancer patients as it allows the clinician to gain an appreciation for target motion during the patient breathing cycle. Resultant binning of images and production of the 4DCT dataset relies heavily on the recorded breathing trace; but quality assurance is not routinely performed on these and there lacks any substantial recommendations thereof. An application was created for Windows in C# that was able to analyze the VXP breathing trace files from Varian RPM/RGSC and quantify various metrics associated with the patient b

Lung motion estimation from 4D CT using structure-tensor-guided finite-element digital volume correlation.

Objective.Accurate lung motion estimation from 4D computed tomography (CT) is essential for image-guided radiotherapy and thoracic motion analysis, but remains challenging in vessel-rich regions where weak contrast, fine structures, and heterogeneous mechanics limit conventional registration. This work aims to develop an anatomy-informed finite-element digital volume correlation (FE-DVC) framework for accurate and mechanically plausible lung motion estimation.Approach.We propose a structure-tensor-guided heterogeneous anisotropic FE-DVC method. Simulated four-dimensional CT for markerless tumor tracking using a deep learning network with multi-task learning.

INTRODUCTION: Our markerless tumor tracking algorithm requires 4DCT data to train models. 4DCT cannot be used for markerless tracking for respiratory-gated treatment due to inaccuracies and a high radiation dose. We developed a deep neural network (DNN) to generate 4DCT from 3DCT data. METHODS: We used 2420 thoracic 4DCT datasets from 436 patients to train a DNN, designed to export 9 deformation vector fields (each field representing one-ninth of the respiratory cycle) from each CT dataset based on a 3D convolutional autoencoder with shortcut connections using deformable image registration. The Utility of 4D-CT Imaging in Primary Hyperparathyroidism Management in a Low-Volume Center.

Background: Ultrasonography (US) and the 99mTc-sestamibi parathyroid scan (SPS) may have suboptimal accuracy when detecting the localization of enlarged parathyroid gland(s) (PTG). Therefore, the more accurate four-dimensional computed tomography scan (4D-CT) has been employed for PTG imaging. Currently, there is a paucity of data evaluating the utility of 4D-CT in low caseload settings. Aim and Objectives: To evaluate the impact of PTG imaging, using 4D-CT in conjunction with its intraoperatively displayed results, on the outcomes of surgical PTX.
