Bone void filler materials for augmentation in comminuted fractures: a comprehensive review.

Comminuted fractures, characterized by multiple bone fragments, present significant challenges in orthopedic surgery. Effective treatment often requires augmentation techniques to enhance fixation stability and promote bone regeneration. This review explores the application of bone void filler materials, including autografts, allografts, polymethyl methacrylate (PMMA), and synthetic bone substitutes such as calcium phosphate ceramics and bioactive glass, in managing comminuted fractures. Autografts are the gold standard due to their superior osteogenic potential but are limited by donor site m

Comminuted long bone fractures in children. Could combined fixation improve the results?

Comminuted diaphyseal fractures in the pediatric age group represent a major orthopedic problem. It is associated with a high incidence of complications and poor outcomes because of the instability and difficulty in treatment. The aim of this study was to evaluate the efficacy of combined external skeletal fixation and flexible intramedullary nails in reconstruction of comminuted diaphyseal fracture in skeletally immature patients. Combined external fixator and elastic stable intramedullary nails were used in the management of 27 pediatric patients (15 males and 12 females) with unstable commi

Biomechanical Evaluation of Four Methods for Internal Fixation of Comminuted Subtrochanteric Fractures.

Subtrochanteric fractures are common and result in significant morbidity and mortality. Various kinds of implants have been used to fix it. The aim of this study was to compare the biomechanical performance of PFN, DHS, DCS, and the PFLP in the treatment of subtrochanteric comminuted fractures.A total of 32 antiseptic human femurs from 16 donors were randomly allocated to 4 groups for fixation with PFN, DHS, DCS, and PFLP. A 2-cm cylindrical bone fragment was removed 1 cm below the lesser trochanter to simulate OTA/AO 32-C3.2 post instrumentation fracture. All specimens in single-leg stance si

Treatment of comminuted distal radius fractures by resurfacing prosthesis in elderly patients.

In elderly patients, distal radius fractures are often associated with osteoporotic bone. Under these conditions, anatomic resurfacing implants may provide satisfactory results in terms of range of motion, pain and function. Between July 2009 and January 2012, eight elderly patients were treated with the SOPHIA™ implant at our hand surgery department. Inclusion criteria were isolated comminuted distal radius AO type C2 fractures in patients greater than 70 years of age. All patients were reviewed in February 2013 by an independent surgeon. Clinical, functional and radiographic assessments were

Tension band wiring for fixation of comminuted fractures of the distal radius.

In the treatment of unstable comminuted and intra-articular fractures of the lower end of the radius, we have found that is is essential to reconstitute the normal anatomical relationship of the radius to the carpus and to the ulna. This is possible by performing an open reduction and by maintaining the fractured fragments by Kirschner wires and adding a tension band. Patients treated thus are reported. There have been no complications and the patients regained full function of the wrist without unsightly deformity.

External Fixation of Clavicle Fractures with Comminution or in Highly Active Patients.

Conservative treatment of clavicle fractures has the disadvantage of substantial pain and discomfort, whereas internal fixation of these fractures has a high rate of non-union. External fixation is an alternative method permitting early mobilization without disrupting the surrounding soft tissue envelope. These are extremely beneficial in young, athletic and highly functional patients as well as in patients with comminuted fractures. This was a retrospective study of 17 consecutive patients with clavicle fractures. Thirteen of them were young, athletic patients while the remaining four were no

Modified cerclage wiring in comminuted transolecranon fracture-dislocations of the elbow.

Transolecranon fracture-dislocations are a result of high-energy trauma, caused due to axial loading of the flexed forearm, with associated anterior dislocation of the ulna with respect to the distal humerus. The usual management of these comminuted and unstable fractures is by using locking compression plates via the dorsal approach. However, plating in cases of poor soft tissue coverage and open wounds can be precarious. In this study, we aimed to evaluate outcomes of cerclage wiring in the management of comminuted trans-olecranon fracture-dislocations in such scenario. A total of seven pati

Isolated comminuted trapezium fracture: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Trapezium, as a carpal bone positioned at the distal row, is often associated with other wrist injuries such as distal radius fracture. Isolated trapezium fracture, especially in a comminuted form, rarely occurs. There are only six reports of isolated comminuted trapezium fractures in the literature to the best of our knowledge. CASE PRESENTATION: We present a case with an isolated comminuted trapezium fracture presenting pain in his thenar eminence and thumb motion limitation. He was treated by Open Reduction and Internal Fixation (ORIF) with the K wire pin. CLI

Comminuted fractures of the proximal humerus - principles of the diagnosis, treatment and rehabilitation.

Comminuted fractures of the proximal humerus impair shoulder function, resulting in more or less severe disability. They rank among the most frequent fractures in adults, with incidence increasing with age and the degree of bone loss (osteoporosis). Among all currently used methods of stabilization of proximal humeral fractures, the best outcomes are afforded by angularly-stable plate fixation and interlocking or reconstructive intramedullary nailing. Both methods produce comparable results enabling bone union and restoration of limb functionality. Nevertheless, in elderly patients with advanc

Anterior subtalar dislocation with comminuted fracture of the anterior calcaneal process.

Anterior subtalar dislocation is a very rare injury. We report a case of an 81-year-old woman who had her right foot injured during a motor vehicle accident. Radiographs showed anterior subtalar dislocation with comminuted fracture of the anterior calcaneal process. The dislocation was closely reduced and protected by a short leg cast. One year postinjury, the patient had only mild pain when walking on uneven ground. There was mild tenderness over the lateral heel. Subtalar motion was mildly painful. There was no pain with ankle motion.
