Frontal sinus outflow anatomy.

Most of the studies of frontal sinus anatomy were completed 50 to 70 years ago. The information they provide is not necessarily relevant or helpful to the modern rhinologic surgeon who approaches the frontal sinus transnasally and endoscopically. We performed anatomical dissections of the outflow tract of 82 frontal sinuses in 41 cadaver heads to illustrate the various drainage patterns from the frontal sinus to the nose and to correlate these drainage sites with the distance and angle from the pyriform aperture. We found that the frontal sinus drained anterior to the uncinate process in 24 sp…

Chronic frontal sinus disease.

A variety of procedures are now available to manage chronic frontal sinus diseases. The choice of procedures depends upon both the extent of the disease and the experience of the surgeon.

Acute frontal sinus barotrauma.

A 25-year-old man presented to the emergency department with an acute onset of frontal sinus pain during descent on a commercial airliner. There was no history of recent upper respiratory infection, sinus infection, or chronic allergic rhinitis. Sinus radiographs demonstrated a left frontal sinus submucosal hematoma. Symptoms improved within 24 hours with systemic and topical decongestants/vasoconstrictors and a nonsteroidal antiinflammatory agent. He was asymptomatic at 1 week postinjury.

Solitary frontal sinus fracture.

Both the anterior and/or posterior wall of the frontal sinus as well as the nasofrontal duct may be injured by blunt force, commonly secondary to automobile accidents. In this group of 13 solitary frontal sinus fractures, the anterior wall was involved in all cases, and the posterior wall in 2. Antibiotic therapy was given to prevent anticipated infection. Comminuted or depressed fractures of the anterior wall were treated by elevation and/or wiring. Surgical treatment was not employed when there was a linear fracture of the anterior wall in good alignment. Posterior wall fragments were remove…

Teaching frontal sinus anatomy using the frontal sinus masterclass 3- D conceptualization model.

OBJECTIVE: Frontal sinus anatomy is complex and often is a difficult subject to both teach and learn. The traditional surgical dogma of "see one, do one, teach one" is impractical and dangerous in the frontal sinus. Based on the building block three-dimensional conceptualization module, the Frontal Sinus Masterclass (FSMC) was created to teach this anatomy. METHODS: Study was performed at two academic centers among second- to fifth-year otolaryngology residents. A pretest assessed knowledge prior to the course. Computed tomography scans of the sinuses were evaluated in triplanar view, and par…

[Endonasal frontal sinus surgery. Part 1: Frontal sinus drainage, types I and II].

Endonasal frontal sinus surgery aims to create adequate access to the frontal sinus, remove specific pathologic processes, and establish a permanent and sufficient drainage and ventilation. We present a concept of frontal sinus surgery that depends on the individual anatomy of the frontal and ethmoid sinuses and includes a stepwise intensity of the surgical procedure. Endonasal frontal sinus drainage according to Draf ranges from uncinectomy to the maximal procedure, type III drainage; these procedures are outlined in detail. A comprehensive overview of technical aspects to optimize the operat…

[Current aspects of frontal sinus surgery. IV: On therapy of frontal sinus osteoma].

Osteomas are the most common benign tumors of frontal sinus. We evaluated 15 patients with osteomas of the frontal sinus who were managed between 1979 and 1992. The average duration of follow-up was 33 months. In 4 cases osteomas were removed completely via the endonasal route using a microscope and endoscope. We recommend performing a frontal sinus drainage-type 3 primarily. Indications are osteomas of the posterior wall of the frontal sinus located close to the infundibulum. For osteomas of the anterior wall and those located laterally or for very large osteomas we prefer the osteoplastic ap…
