Pancreaticobiliary diversion is a surgical procedure undertaken for obesity. Crohn's disease may involve any part of the alimentary tract, including the stomach and duodenum. We report herein the case of a 22 year-old male in whom Crohn disease was diagnosed due to weight loss in relation with a stenosis of the first and second parts of the duodenum. A gastrojejunostomy was performed with a good subsequent result. The initial management of a Crohn's disease with involvement of the duodenum is medical. When there is an indication for surgery, a gastroenterostomy is preferred, albeit with a high incidence of outlet obstruction and marginal ulceration.

Experience in treatment of 168 patients with burns of the esophagus including 42 (25%) with consequences of combined chemical burns of the esophagus and the stomach is presented. In combined burn strictures of the esophagus and a pyloroantral part of the stomach with a large area of the lesion the method of repair of enteral alimentation was successfully applied. Laparoscopic Roux-en-Y gastric bypass was recently introduced as an alternative surgical treatment for morbid obesity. The technique involves placement of a 21-mm anvil transorally down to the gastric pouch for creation of the gastroenterostomy anastomosis using an EEA stapler placed transabdominally. Gastric outlet obstruction (GOO) in locally advanced pancreatic cancer (LAPC) can be managed with endoscopic duodenal stenting (EDS), gastrojejunostomy (GJ), or EUS-guided gastroenterostomy (EUS-GE). In our meta-analysis of LAPC patients, EDS enabled quicker recovery and chemotherapy but had higher re-intervention rates, while GJ offered greater durability. Our systematic review showed EUS-GE had high success and low re-intervention in malignant GOO, though LAPC-specific data were lacking. Further comparative studies are needed to guide individualized treatment.

A 51-year-old man, 25 years after undergoing Billroth II gastrojejunostomy with antrectomy and vagotomy, came to our hospital with acute relapsing pancreatitis. At operation he was found to have an inflammatory polypoid lesion at the duodenal stump that consisted of suture and talc granuloma from his previous gastric surgery. The polyp intermittently obstructed the pancreatic duct, causing symptomatic pancreatitis. The patient has remained without symptoms 2 years after submucosal resection of this polyp. This complication of Billroth II gastroenterostomy has not been recognized previously.

A 19-year-old man diagnosed with diffuse large B-cell lymphoma undergoing chemotherapy presented for recurrent emesis and weight loss. Imaging studies of the abdomen demonstrated features of superior mesenteric artery syndrome. The patient deferred conservative treatment options and was deemed not to be a surgical candidate. Four patients were admitted to the Surgical Ward because of massive bleeding from a duodenal ulcer. In three the ulcer was induced by non-steroid anti-inflammatory drugs. On operation, following pylorotomy and suture of the bleeding artery, neither drainage nor stump closure could be affected safely. BACKGROUND AND STUDY AIMS: Surgical gastrojejunostomy (S-GJ) is standard treatment for gastric outlet obstruction (GOO), but is associated with significant morbidity, high cost, and prolonged recovery. Endoscopic ultrasound-guided gastroenterostomy (EUS-GE) provides a minimally invasive alternative; however, the anastomosis often closes after removal of the lumen-apposing metal stent (LAMS), limiting its role to palliation.
