Changes in pattern of immunoglobulin heavy chain gene rearrangement and MIB-1 staining before and after eradication of Helicobacter pylori in gastric mucosa-associated lymphoid tissue (MALT) lymphoma.

Gastric low-grade mucosa-associated lymphoid tissue (MALT) lymphomas are strongly associated with infection by Helicobacter pylori (H. pylori). Antibiotic treatment for H. pylori induces a sustained remission in a significant number of patients. We report here the outcome in 13 patients with gastric low-grade MALT lymphomas or suspected gastric lesions, treated for eradication of H. pylori. Patients were followed closely with sequential histological studies, polymerase chain reaction (PCR) amplification of the immunoglobulin heavy chain (IgH) gene and immunohistochemistry for MIB-1. Antibiotic

Analysis of the immunoglobulin heavy chain gene of secondary diffuse large B-cell lymphoma that subsequently developed in four cases with B-cell chronic lymphocytic leukemia or lymphoplasmacytoid lymphoma (Richter syndrome).

The immunoglobulin heavy chain gene (IgH gene) was analysed in four cases of B-cell Richter syndrome, in order to determine whether a secondary diffuse large B-cell lymphoma (DLBCL) could arise from the same clone as the initial B-cell chronic lymphocytic leukemia (B-CLL) and lymphoplasmacytoid lymphoma (LPL) or be a de novo event, and whether secondary DLBCL shows an intraclonal microheterogeneity. Both the initial B-CLL and secondary DLBCL in two cases expressed CD5 antigen. Both samples of the initial B-CLL or LPL and the secondary DLBCL in three cases were examined for comparison. The poly

Pulmonary extranodal marginal zone lymphoma that presented with macroglobulinemia and marked plasmacytic cell proliferation carrying the t(14;18)(q32;q21)/MALT1-immunoglobulin heavy-chain fusion gene in pleural fluid.

An 80-year-old man presented with the accumulation of pleural fluid in the right thoracic cavity. Serum electrophoresis revealed an M-component and immunofixation confirmed IgM/λ. The level of IgM was 1,526 mg/dL. Imaging studies showed an infiltrative condition of the ipsilateral lung parenchyma. The fluid contained abundant neoplastic cells with the morphological and immunophenotypic features of plasma cells, which expressed IgM/λ monoclonal immunoglobulins on the cell surface and in the cytoplasm. The karyotype was 48,XY,+3,add(9)(p13),+12,add(14)(q32),del(16)(q22),-18,+mar, and a series of

Allogeneic transplant with reduced intensity conditioning regimens may overcome the poor prognosis of B-cell chronic lymphocytic leukemia with unmutated immunoglobulin variable heavy-chain gene and chromosomal abnormalities (11q- and 17p-).

PURPOSE: To evaluate the efficacy of reduced intensity conditioning (RIC) allogeneic transplant in 30 patients with poor-prognosis chronic lymphocytic leukemia (CLL) and/or high-risk molecular/cytogenetic characteristics. EXPERIMENTAL DESIGN: Eighty-three percent of patients had active disease at the moment of transplant. That is, 14 of the 23 patients analyzed (60%) had unmutated immunoglobulin variable heavy-chain gene (IgV(H)) status; 8 of 25 patients (32%) had 11q-, with four of them also displaying unmutated IgV(H); and six (24%) had 17p- (five were also unmutated). RESULTS: After a med

Detection of clonally restricted immunoglobulin heavy chain gene rearrangements in normal and lesional skin: analysis of the B cell component of the skin-associated lymphoid tissue and implications for the molecular diagnosis of cutaneous B cell lymphomas.

A monoclonal B cell population is the hallmark of B cell neoplasms including cutaneous B cell lymphomas (CBCLs). We modified and tested several polymerase chain reaction (PCR)-based assays involving amplification of immunoglobulin heavy chain (IgH) gene rearrangements to optimize assays specifically for cutaneous lymphoid infiltrates. We achieved greatest sensitivity with an assay employing IgH consensus primers complementary to the framework 3 portion of the upstream variable region and the downstream joining region. We studied 12 CBCLs, 6 nodal lymphomas and 7 cell lines. In 17/25 of these B

[Study of the clonal characteristics of autoantibodies in chronic idiopathic thrombocytopenic purpura].

OBJECTIVE: To define the clonality characteristics of autoantibodies in chronic idiopathic thrombocytopenic purpura (ITP). METHODS: The heavy-chain and light-chain phenotype composition of the glycoprotein (GP)-specific IgG antibodies were analysed with a modified monoclonal antibody specific immobilization of platelet antigens (MAIPA) technique. The immunoglobulin heavy-chain gene rearrangement was analysed by PCR amplification. RESULTS: 16 out of the 43 patient sera reacted with least one of the following five GPs, namely GPIIb/IIIa, GPIb, GPIa, GPIV and GPV. Eight of 11 (73%) GP-specific

Immunoglobulin and T cell receptor gene rearrangement and expression in B cell acute lymphoblastic leukemia.

Immunoglobulin and T cell receptor gene rearrangement and expression were investigated within B cell acute lymphoblastic leukemias (ALLs) that are thought to be representative of the early stages of normal human B cell development. Although the immunoglobulin heavy chain locus is rearranged in leukemic cells from all of these patients, only cells from pre-B ALL patients express mu chain protein. We have shown, however, that immunoglobulin heavy chain transcripts are expressed at levels similar to those found in normal B cells in all of these leukemic cell types and, in addition, some of the le

Uniform detection of immunoglobulin-gene rearrangement in benign lymphoepithelial lesions.

The term "benign lymphoepithelial lesion" is used to describe the salivary-gland lymphocytic infiltration and epithelial changes typically found in association with Sjögren's syndrome. We used Southern blot hybridization techniques to examine the immunoglobulin genes in salivary-gland tissue derived from eight patients with benign lymphoepithelial lesions. Three of these patients had intrasalivary non-Hodgkin's lymphoma complicating the lesions, whereas the lesions in the remaining five were all histologically benign. Ten samples from the eight patients all revealed rearrangement of both the h

Acute lymphoblastic leukemia with the (4;11) translocation: combined cytogenetic, immunological and molecular genetic analyses.

Five cases of acute leukemia (AL) with the t(4;11) translocation were investigated for the immunoglobulin heavy chain, kappa, lambda, TCR beta and TCR gamma gene rearrangements. All patients presented with high-risk features and had survival times of less than two years. Two cases were classified by immunological phenotyping as acute null-AL(L), one case as pre B-cell ALL (CD10+) and two cases expressed both immature B-cell markers CD19 and CD24 and myelomonocytic markers CD15 and CD14, suggesting mixed lineage leukemia. In two cases more than two rearranged fragments for the immunoglobulin he

Classical Hodgkin's disease and follicular lymphoma originating from the same germinal center B cell.

PURPOSE: Classical Hodgkin's disease and non-Hodgkin's B-cell lymphoma occasionally occur in the same patient. To clarify whether these different diseases share a common precursor cell, we analyzed the immunoglobulin rearrangements in tumor cells of the classical Hodgkin's disease and the follicular lymphoma that developed in the same patient 2 years apart. PATIENTS AND METHODS: Polymerase chain reaction (PCR) for the detection of rearranged immunoglobulin genes was carried out on single Reed-Sternberg cells and on whole tissue DNA extracted from the follicular lymphoma. PCR products were seq

Lack of expression of surface immunoglobulin light chains in B-cell non-Hodgkin lymphomas.

We describe 10 cases of B-cell non-Hodgkin lymphoma (NHL) that did not express immunoglobulin kappa or lambda light chains by dual-color flow cytometry. Cases were identified from 298 consecutive cases of B-cell NHL and included follicular center cell lymphoma, diffuse large B-cell lymphoma, small noncleaved cell lymphoma, and small lymphocytic lymphoma. One case did not express any immunoglobulin heavy chain (IgH) as well; however, isolated expression of IgG heavy chain was seen in another case. Immunoglobulin heavy chains were not part of the lymphoma panel in other cases. All 3 cases in whi

Kappa light chain gene rearrangement in T-cell acute lymphoblastic leukemia.

Immunoglobulin and T-cell receptor gene rearrangement assays are sensitive methods of detecting clonality in lymphoproliferative disorders. The lack of lineage specificity of immunoglobulin heavy chain and T-cell receptor beta and gamma gene probes in acute leukemia is well established. However, immunoglobulin light chain gene rearrangement traditionally has been considered a highly specific indicator of a clonal B-lineage process. The authors describe a case of T-cell acute lymphoblastic leukemia in which Southern blot hybridization studies showed rearrangement of the T-cell receptor beta cha

[Proliferation of specific cytotoxic T lymphocytes induced by immunoglobulin heavy chain framework region-derived antigenic nonapeptides].

OBJECTIVE: To identify that immunoglobulin heavy chain framework regions-derived peptides function as cytotoxic T lymphocytes epitopes. METHODS: Seven IgHV gene families were respectively amplified by PCR and directly sequenced for 108 acute lymphoblastic leukemia cases. Sequences available were translated into amino acid sequences. Bioinformatics were applied for analyzing recombination patterns and gene mutations of the IgHV genes. Softwares SYFPEITHI and BIMAS were used for predicting the T cell epitopes in the immunoglobulin heavy chain variable regions. To determine whether the predicted

Variable rate of detection of immunoglobulin heavy chain V-D-J rearrangement by PCR: a systematic study of 41 B-cell non-Hodgkin's lymphomas and leukemias.

The use of clonospecific probes has recently been employed for the detection of minimal residual disease in B- and T-cell acute lymphoblastic leukemias. However, these methods are predicated upon the successful amplification of the V-D-J rearrangement in the genome of the tumor cells by the polymerase chain reaction (PCR). In order to determine whether the type of B-cell lymphoid malignancy influenced the rate of success of amplifying the region of the immunoglobulin heavy chain gene rearrangement in these lesions, we studied 41 morphologically and immunologically well characterized B-cell neo

Can polymerase chain reaction help distinguish benign from malignant lymphoid aggregates in bone marrow aspirates?

OBJECTIVE: Although morphologic and immunologic clues are helpful in distinguishing benign from malignant lymphoid aggregates in bone marrow biopsies, there remain some cases in which it is not possible to arrive at a definitive diagnosis. Since the malignant aggregates are monoclonal B-cell proliferations, we sought to determine whether performing polymerase chain reaction for the immunoglobulin heavy-chain locus would be helpful in distinguishing these 2 entities. METHODS AND RESULTS: Scrapings from unstained bone marrow aspirate smears or touch preparations of bone marrow biopsies from 15

Compartment analysis of T cell receptor gamma--and immunoglobulin V(H) rearrangements by microdissection in patients with malt lymphoma and chronic gastritis with lymphatic hyperplasia.

The interpretation of clonality within H. pylori-associated gastritis and low-grade MALT lymphoma remains controversial. Due to the observation of MALT lymphoma regression after H. pylori eradication, new definitions concerning the border between benign reactive lesions and malignant gastric lymphoma are needed. Gene rearrangements for immunoglobulin heavy-chain in low-grade MALT lymphoma (N= 12) and H. pylori-associated chronic gastritis with lymphatic hyperplasia (N= 13) were analyzed by microdissection and polymerase chain reaction. Furthermore, T cell receptor-gamma chain rearrangements we

Infectious mononucleosis: a polyclonal B cell transformation in vivo.

We obtained spontaneous formation of Epstein-Barr virus-transformed B lymphocyte colonies from the blood of four patients with mononucleosis with the use of soft-agar medium. The colonies were propagated into separate cell lines and analyzed for their immunoglobulin secretion. Of 52 such lines, 44 produced immunoglobulin composed of a single class of heavy chain and single type of light chain. Among these clonal transformants, 27 (62%) of 44 produced mu-chain, 13 (29%) of 44 produced gamma-chain, and 4 (9%) of 44 produced alpha-chain. Analysis of light-chain secretion revealed that of the 44 c

[Immunohistochemical and immunogenetic analysis of ocular adnexal lymphoid proliferations].

We examined 20 cases (21 specimens) of ocular adnexal lymphoid proliferations, using the histological, immunohistochemical and molecular genetic methods. The latter two protocols were performed to detect the light chains restriction of immunoglobulin with peroxidase-antiperoxidase (PAP) methods, and the clonality of immunoglobulin heavy chain gene with the hemi-nested polymerase chain reaction (PCR) method, respectively. Although in 8 cases it could not be morphologically determined whether they were neoplastic or not, clonality was revealed in 1 case with immunohistochemistry and in 4 cases w
