Coma Reversal After Intravenous Lipid Emulsion Therapy in a Trazodone-Poisoned Patient.

A 77-year-old woman developed a deep coma (Glasgow Coma Scale score at 3/15) after a trazodone overdose (maximal ingested dose, 4500 mg), and orotracheal intubation was required for mechanical ventilation. In addition, she presented sinus bradycardia (<40/min) with QTc prolongation, whereas arterial blood pressure was preserved. The administration of intravenous lipid emulsion (1.5 mL/kg as a bolus followed by a continuous infusion of 0.5 mL/kg per minute for 30 minutes) resulted in a rapid improvement of her neurological condition, with a Glasgow Coma Scale score rising from 3 to 10/15 at the

[Head trauma in a general surgery department: observations, diagnostic and therapeutic indications].

UNLABELLED: The authors reviewed the records of 927 patients admitted to Surgical Clinic University of L'Aquila from November 1986 to July 1990 with head trauma. The 5.6% (52 patients) had skull fractures. 23 (2.4%) patients sustained significant intracranial sequelae from their injuries, but only 4 (17.3%) of these also sustained fractures, 17 did not. Of the four fractures 1 were simple, 2 was depressed and 1 was basilar. The patients (17) without a skull fracture and positive CT were transferred to a neurosurgical department, where 12 underwent operation. The patients (4) with a skull fract

The neuropathology of traumatic brain injury.

Traumatic brain injury, a leading cause of mortality and morbidity, is divided into three grades of severity: mild, moderate, and severe, based on the Glasgow Coma Scale, the loss of consciousness, and the development of post-traumatic amnesia. Although mild traumatic brain injury, including concussion and subconcussion, is by far the most common, it is also the most difficult to diagnose and the least well understood. Proper recognition, management, and treatment of acute concussion and mild traumatic brain injury are the fundamentals of an emerging clinical discipline. It is also becoming in

Effect of Low Frequency Repetitive Transcranial Magnetic Stimulation (rTMS) Combined with Hyperbaric Oxygen (HBO) on Awakening of Coma Patients with Traumatic Brain Injury.

Источник пометил публикацию как отозванную

Coma caused by craniocerebral injury is a common condition of neurosurgical acute injury. There is no specific method to promote awakening in a clinic. Early comprehensive treatment may be helpful to patients. The common methods are hyperbaric oxygen (HBO) and low-frequency repetitive transcranial magnetic stimulation (rTMS). However, the application effect and mechanism of rTMS combined with HBO on coma patients with traumatic brain injury need to be further studied. The brain stem auditory evoked potential (BAEP) is examined by the Kennedy coma recovery scale (CRS-R), the recovery of brain f

Neural Response to Familiar Names Predicts Outcome of Comatose ICU Patients: A Prospective Observational Cohort Study.

Predicting the outcome of comatose patients in the intensive care unit (ICU) can inform decision making but remains challenging. Recent studies suggest that task-state electroencephalography (EEG) can detect covert cognition and facilitate patient prognosis. This study aimed to predict the outcome of comatose patients, by assessing covert processing of familiar names using a state-of-the-art EEG frequency tagging approach. Eighty-nine comatose patients following acute brain injury were recruited from five ICUs. Patients were presented with a rapid stream of familiar names and acoustically matc

Updating Traumatic Brain Injury Classification for Surgeons: Integrating the CBI-M Framework Into Trauma and Acute Care Practice.

BACKGROUND: Traumatic brain injury (TBI) remains a major cause of death and disability worldwide. Traditional classification based on the Glasgow Coma Scale (GCS) provides a shared clinical language but insufficiently captures the biological heterogeneity, imaging variability, and contextual modifiers that influence outcomes. In January 2024, the National Institute of Neurological Disorders and Stroke (NINDS) convened a multidisciplinary working group to propose the Clinical-Biomarkers-Imaging-Modifiers (CBI-M) framework. This review aims to summarize the rationale, structure, and potential cl

Past, present and future perspectives of traumatic brain injury.

Despite extensive research, traumatic brain injury (TBI) imposes significant mortality and morbidity burden globally. This narrative review provides a comprehensive overview of how TBI classification, surgical and critical care management, and physiological treatment goals have evolved based on recent evidence, shaping future directions for TBI care. We highlight the conceptual shift in TBI classification from the Glasgow Coma Scale to a new framework that integrates clinical and imaging features, biomarkers, and modifiers to improve disease characterization and describe disease trajectories.

[Neurosurgical Management of Traumatic Brain Injury].

The neurosurgical management of traumatic brain injury (TBI) plays a critical role in ensuring acute survival and mitigating secondary brain damage, which significantly impacts patients' quality of life. TBI is defined as an external force impacting the skull, leading to brain injuries and subsequent functional impairments. It is a leading cause of mortality and morbidity, particularly among young individuals. The initial clinical examination is crucial, with external signs like scalp injuries, hematomas, nasal fluid leakage, skull deformities, and neurological deficits providing important clu

Prediction of functional outcome after traumatic brain injury: a narrative review.

PURPOSE OF REVIEW: To synthesize current evidence on prognostic factors, tools, and strategies influencing functional outcomes in patients with traumatic brain injury (TBI), with a focus on the acute and postacute phases of care. RECENT FINDINGS: Key early predictors such as Glasgow Coma Scale (GCS) scores, pupillary reactivity, and computed tomography (CT) imaging findings remain fundamental in guiding clinical decision-making. Prognostic models like IMPACT and CRASH enhance early risk stratification, while outcome measures such as the Glasgow Outcome Scale-Extended (GOS-E) provide structure

Can modern therapy influence the prognosis of brain injuries in childhood?

A standard therapy and the outcome following severe head injury in 51 children are presented. The Glasgow-Coma-Scale was used for classification. Initial therapy included intubation, hyperventilation, ICP monitoring, and barbiturate coma. 14 children had a normal ICP recording, 13 showed moderate (-35 mmHg) ICP elevations, and 23 had recurrent ICP elevations above 35 mmHg. Only one patient survived in a vegetative state. Overall mortality was 21%. All but one of the survivors were controlled one year after the accident. School performance, social integration and the Glasgow-Outcome-Scale were
