Pneumonia remains a concern for persons with long-standing HIV infection. We present a case of a 43-year-old HIV-infected woman with bilateral pneumonia whose presentation suggested the cause was a bacterial pathogen. A chest of radiograph and CT scan of the chest revealed infiltrates and adenopathies, but this did not help in the differential diagnosis. A Gram stain of a sputum specimen revealed gram-positive filamentous rods, and infection with Nocardia asteroides was diagnosed. The patient was started on a regimen of ceftriaxone and trimethoprim/sulfamethoxazole and experienced significant

Fournier's disease--gangrene of the penis and scrotum--is an uncommon condition. During the past 6 years, five patients, whose cases are described, were admitted to Toronto General Hospital with this diagnosis. Four had preceding trauma (ischiorectal abscess, puncture wound, surgery) and four had pre-existing debilitating problems (diabetes, rectal carcinoma, acute lymphocytic leukemia, alcoholic cirrhosis). Appropriate treatment must include urgent radical surgery to remove all necrotic tissue, and combination antibiotic therapy directed against the likely organisms, which are aerobic gram-ne

Two issues that have become clinically relevant to the treatment of pneumonia over the past few years are the development of antibiotic resistance among respiratory pathogens and the increasing importance of the atypical respiratory pathogens---Mycoplasma pneumoniae, Chlamydia pneumoniae and Legionella spp. Resistance has become an important issue in Streptococcus pneumoniae, methicillin-resistant Staphylococcus aureus and Gram-negative rods. The ways by which bacteria become resistant to antibiotics include production of antibiotic-modifying enzymes, reduced access to target sites, efflux of

We used a decision analysis model to estimate the clinical and economic implications of testing the gram-stained smears to select the effective antibiotics at the initial visit on women with lower urinary tract infections. A strategy of the initial therapy of antibiotics without routine testing was compared with the initial testing of urine gram stain and selecting antibiotics according to the results. Varying assumptions were used for prevalence of etiologic agents, operating characteristics of gram stain test, treatment efficacy and required days for microbiological tests. We concluded that

The aim of this study was to evaluate the frequency of isolation and antimicrobial resistance testing of bacterial strains isolated from clinical specimens from patients hospitalized in three Intensive Care Units in Wrocław. The susceptibility of bacteria (107 strains) to selected antibiotics was determined. The results clearly show that non-fermentative rods were identified as the main agents causing pneumonia (58% of isolates). The second commonest pathogens were Gram-positive cocci (29%). The P. aeruginosa and E. cloacae strains were resistant to ampicillin, amoxicillin/clavulanate, cefurox

OBJECTIVE: This purpose of this study was to investigate the association between vaginal microflora, concentrations of interleukin-1beta (IL-1beta), and its natural receptor antagonist (IL-1ra) in the cervicovaginal discharge, and spontaneous preterm birth. Study design Vaginal samples collected at 18 to 22 weeks' gestation from 207 women were analyzed to study qualitative and quantitative microbiologic aspects of vaginal microflora and IL-1beta and IL-1ra concentrations.
RESULTS: Among women colonized with anaerobic Gram-negative rods and/or Gardnerella vaginalis, an elevated IL-1beta concen

The course of intrauterine infections are illustrated and 5 maternal deaths reported. Early lung complications were predominant, described as shock lung or septic pneumonia. Renal complications or disseminated intravascular coagulation were absent or developed late. In contrast to gram negative sepsis there was a frequent association with premature rupture of the membranes, which indicates the danger of this event. On microbiology gram positive rods were found more frequently than a mixed flora. The morphology of intrauterine infection is similar to those of puerperal sepsis although the clini

The emergence and dissemination of drug-resistant Gram-negative bacilli have been recognized as a serious health concern in worldwide. The isolation rates of Extended-Spectrum β-lactamases (ESBL) and AmpC β-lactamases (AmpC) producing gram negative rods are increasing in our hospital. In the present study, we evaluate the availability of the antimicrobial resistance testing by the direct disc methods using AmpC/ESBL differential discs. One hundred and ten strains of Enterobacterales were isolated during the observation period, of which 19 strains (17%) were ESBL-positive and 6 strains (5%) wer

Actinobaculum species are anaerobic Gram-positive rods that have previously been associated with urinary tract infection (UTI) in the elderly. We report 12 patients with Actinobaculum bacteremia. Only 40% of blood cultures were clinically considered significant by the treating physicians, but most patients were treated for UTI, suggesting a possible urinary source of bacteremia. Clinicians should be aware of the pathogenic potential of Actinobaculum spp.

During 1991 to 1993 12,064 blood cultures from patients with suspected bacteraemia were examined in a newly established Microbiology Department of the Brno-Bohunice Faculty Hospital. A total of 1,790 (14.8%) blood cultures were positive, 746 (6.2%) were contaminated. The incidence of positive blood cultures was 7.1/1,000 patients admitted in 1991, 10.0/1,000 in 1993. From the positive blood cultures 853 different microbes were isolated: 486 gram-negative bacteria (56.9%), 315 gram-positive ones (36.9%), 9 anaerobes (1.1%) and 44 yeasts (5.2%). In 69 cases (3.8%) more than one agent was recover

BALB/cA mice were examined for the effects of chronic isoproterenol treatment or submandibular-sublingual gland ablation on the natural patterns of oral bacterial colonization on tooth surfaces. Indigenous microflora on the tooth surfaces of BALB/cA mice was relatively simple. The predominant bacterial groups were Enterobacteriaceae (45.9%), enterococci (29.4%) and staphylococci (15.7%). Isoproterenol, which resulted in the induced synthesis of proline-rich proteins, caused a decrease in the total cultivable bacteria on the tooth surfaces. The proportion of Enterobacteriaceae in the isoprotere

10 patients with posttraumatic bacterial meningitis were treated in the Department of Infectious Diseases of the Jagielloniam University--Collegium Medicum during the period of 63 months. Traffic accidents were responsible for 80% of all cases of trauma. The most common place of injury was the base of anterior cranial fossa. Infecting agent was established in 8 cases. Gram-positive aerobic cocci (Streptococcus pneumoniae, Enterococcus faecalis, and Enterococcus spp.) were found in 4 patients, Gram-negative aerobic cocci (Neisseria meningitidis) in 2 patients, and Gram-negative aerobic rods (Ac

Using colony morphology on selected agars, Gram-stain morphology, and a number of 1-step biochemical or enzymatic tests, skilled microbiologists can identify the species of the majority of isolates seen routinely in a clinical laboratory. These results are often available more quickly than and are as accurate as those derived from conventional methods. The National Committee for Clinical Laboratory Standards has produced a guideline that describes tests that can be used to identify a number of aerobic gram-negative rods and gram-positive cocci, a number of commonly isolated anaerobes, and 3 sp

Brevibacteria are Gram-positive rods found in human skin flora and dairy products. Although generally not considered human pathogens, case reports have implicated Brevibacterium species as rare causes of bacteremia, endocarditis, peritonitis, and osteomyelitis. We report a case of Brevibacterium tibial osteomyelitis in an immunocompetent individual with implanted hardware and highlight the challenge of identifying the organism and recognizing it as a potential pathogen.

The enterotoxaemia syndrome in Belgian Blue calves is characterised by a high case fatality rate, sudden death, lesions of haemorrhagic enteritis of the small intestine and, quite often an absence of other clinical signs but its cause has not been yet identified. As a first step in this identification, the aerobic and anaerobic intestinal flora of a population of 78 calves, originating from farms located in southern Belgium and that died in circumstances defined as "calf enterotoxaemia" (study population) and of 64 calves that died in other circumstances (control population) were studied quali

Gas gangrene developed from a chronic foot ulcer in the absence of periferal vascular disease or diabetes mellitus in a hospitalized patient undergoing parenteral antibiotic therapy. Within a 6 hour period the patient developed profound toxemia necessitating emergency and life saving leg amputation. Classically clostridial myonecrosis is diagnosed by the clinical course and the gram stain. In this case, 2 preoperative gram stains failed to show gram-positive rods. At the time of surgery, frank fasical and muscle necrosis in the peroneal compartment dictated extending the below knee amputation

Urinary tract infections (UTI) due to gram-positive bacteria are fairly uncommon. In order to investigate the efficacy of treatment for UTI secondary to gram-positive rods, we performed a non-comparative study on the effect of cefotaxime in 64 patients with gram-positive UTI. Patients with a history of hypersensitivity to cephalosporins and penicillin as well as patients who had received antibiotic treatment within 48 hours after the administration of cefotaxime, patients with hepatic disease and patients with fatal progressive disease were excluded from the study. UTI was confirmed by positiv

Rapid and accurate identification of the pathogens involved in bloodstream infections is crucial for the prompt initiation of appropriate therapy, as this can decrease morbidity and mortality rates. A PCR-reverse blot hybridization assay for sepsis, the reverse blot hybridization assay (REBA) Sepsis-ID test, was developed; it uses pan-probes to distinguish Gram-positive and -negative bacteria and fungi. In addition, the assay was designed to identify bacteria and fungi using six genus-specific and 13 species-specific probes; it uses additional probes for antibiotic resistance genes, i.e., the

A 53-year-old woman with non-Hodgkin lymphoma underwent an autologous bone marrow transplant (BMT). Incomplete reconstitution necessitated the use of a long-term central venous catheter. One year after BMT she presented with fever. Echocardiography revealed vegetations on the tricuspid valve. Gram-positive rods grown from blood cultures and catheter tip were identified as Oerskovia xanthineolytica. We report the first case of native valve endocarditis caused by this organism.
