Characterising Hand Tremor in Adult Kidney Transplant Recipients: Its Impact on Daily Activities and Relationship With Tacrolimus Administration and Exposure.

AIM: This study aimed to characterise hand tremor and examine its relationship with tacrolimus administration time and systemic exposure in adult kidney transplant recipients early post-transplantation. METHODS: Quantitative and qualitative data on hand tremor were collected from patients receiving immediate-release tacrolimus, approximately 1 month post-transplantation. Smartphone accelerometry was used to objectively assess tremor pre- and 2 h post-tacrolimus administration and values were compared using a Wilcoxon signed-rank test. A Welch's analysis of variance (ANOVA) was used to determi

Utilizing the same incision for staged renal transplant in patients with polycystic kidney disease requiring hand-assisted laparoscopic nephrectomy.

BACKGROUNDS: Many autosomal dominant polycystic kidney disease (ADPKD) patients undergo nephrectomy and subsequent renal transplantation. We report our outcomes after hand-assisted laparoscopic nephrectomy (HALN) where a Rutherford-Morrison incision is used as a hand-port site and kidney extraction site, as well the future incision site for staged transplantation. METHODS: A retrospective review was performed on all adult nephrectomies for ADPKD by the Transplant Surgery department at Westmead Hospital between June 2011 and June 2021. Outcomes were compared between HALN, laparoscopic nephrect

A Lifesaving View of Vascularized Composite Allotransplantation: Patient Experience of Social Death Before and After Face, Hand, and Larynx Transplant.

INTRODUCTION: Most solid organ transplantation is viewed as lifesaving, whereas vascularized composite allotransplantation (VCA) has been viewed as life enhancing. This article challenges the latter and argues that "social death" evident in severe face, hand, and larynx disfigurement can be potentially treated via VCA. Social death (from a social science perspective) consists of a combination of 7 components: social isolation, loneliness, ostracism, loss of personhood, change of role and identity, harm, and disfigurement. METHODS: In February 2016, PubMed and Google were searched for case rep

Demonstration of reversed flow in segmental branches of the portal vein with hand-held color Doppler ultrasonography after hematopoietic stem cell transplantation.

Hepatic veno-occlusive disease (VOD) is a severe complication of hematopoietic stem cell transplantation (SCT). When monitored with hand-held color Doppler ultrasonography during day -7 to +35 around SCT, reversed blood flow in the segmental branches of the portal vein was detected in nine of 56 patients who had undergone SCT. Three of nine patients had clinical evidence of VOD, but six patients did not fulfill the criteria for diagnosis of VOD initially. Two patients progressed to clinical VOD at a later date and the reversed portal flow disappeared with or without treatment for VOD in the ot

Hand-Assisted Laparoscopic Native Nephrectomy for Polycystic Kidney Disease Before Transplantation: A Single-Centre Case Series with a Technique-Stratified Focused Scoping Review.

Background: Native nephrectomy is frequently required in autosomal-dominant polycystic kidney disease (ADPKD) before or during transplantation, but the optimal minimally invasive approach is unsettled, and it is unclear whether the pattern of post-operative complications tracks with operative technique. We report a retrospective single-centre experience of hand-assisted laparoscopic (HAL) native nephrectomy and, alongside it, a technique-stratified scoping review of complications. Methods: We retrospectively reviewed all consecutive adults undergoing HAL native nephrectomy-with an infra-umbili

Hand-Assisted Laparoscopic Subtotal Colectomy for Ogilvie Syndrome Associated With Idiopathic Fibrosis of Colon After Simultaneous Pancreas Kidney Transplant.

Ogilvie syndrome (acute colonic pseudo-obstruction) is a rare, acquired, life-threatening disorder for which treatment plans vary from simple observation to surgical intervention. Ogilvie syndrome has been reported in patients after renal or liver transplant, but its occurrence after simultaneous pancreas-kidney transplant is rare. Herein, we present the case of a 45-year-old female recipient of a deceased donor simultaneous pancreas-kidney transplant who developed Ogilvie syndrome 10 days after a previous fecal ileus that had resolved at posttransplant week 3. She demonstrated Ogilvie syndrom

Infrared imaging of lymphatic function in the upper extremity of normal controls and hand transplant recipients via subcutaneous indocyanine green injection.

OBJECTIVES: The goal of this study was to define the parameters of movement of indocyanine green in the upper extremity of normal control and hand transplant recipients. The purpose was to establish a non-invasive method of determining the level of lymphatic function in hand transplant recipients. In hand transplantation (and replantation), the deep lymphatic vessels are rarely repaired, resulting in altered lymphatic connections. In most cases, the relatively rapid inosculation of superficial lymphatic networks and drainage via the venous systems results in sufficient interstitial fluid and l

The United Kingdom Hand and Upper Limb Transplantation Service: A report on the first twelve years of a single-center, single-protocol clinical cohort.

BACKGROUND: Following the world's first case (1998), proof of concept was established for hand and upper limb transplantation (HAUL-T). By 2012, there was sufficient evidence for the United Kingdom HAUL-T service to be established on a standard of care basis. A commitment was made to report in detail on this prospective, single-center, single-protocol series. We provide the first comprehensive report on the procedures, surgical, functional, immunological and psychosocial outcomes of the United Kingdom (UK) HAUL-T service. METHODS: Potentially suitable patients undergo sequential psychological

Electrothermal Lymphatic Sealing (LigaSure) Versus Conventional Hand-Tie Ligation for Control of Perivascular Lymphatics in Kidney Transplant Recipients: A Mixed Prospective-Retrospective Cohort Study.

BACKGROUND: Ligation of iliac vessel lymphatics is essential for preventing lymphocele in kidney transplant (KT) recipients. Electrothermal bipolar vessel sealer (EBVS) such as LigaSure (Medtronic) may offer technical advantages. This study aimed to evaluate the safety and efficacy of lymphatic sealing using LigaSure in KT recipients compared to conventional hand-tie ligation. METHODS: A mixed prospective-retrospective cohort study was conducted at a single center. The 'hand-tie group' included data collected from 300 KT recipients transplanted between May 2017 and May 2020 using hand-tie lym

Intensified fractional CO2 laser-assisted photodynamic therapy vs. laser alone for organ transplant recipients with multiple actinic keratoses and wart-like lesions: a randomized half-side comparative trial on dorsal hands.

BACKGROUND: Photodynamic therapy (PDT) is a well-documented treatment for actinic keratosis (AK), but achieves inferior efficacy in organ transplant recipients (OTRs), particularly in acral regions. Ablative fractional laser (AFXL) intensifies the PDT response and may improve the efficacy of AK clearance when used as monotherapy. OBJECTIVES: To compare the efficacy of a single treatment with AFXL-assisted PDT vs. AFXL alone for difficult-to-treat AKs and wart-like lesions (WLLs) in OTRs. METHODS: Ten OTRs were included with a total of 680 AKs (severity grade I-III) and 409 WLLs on the dorsal

Replantation versus transplantation: Where do we stand?

PURPOSE: Despite advances in extremity trauma care and reconstructive microsurgery, management of the traumatic amputations remains a challenge. The majority of patients will forever experience some level of disability even with replantation or advanced prosthetics. The goal of this article is to familiarize hand and reconstructive surgeons with the current state of upper extremity transplantation, so they better can educate their amputee patients regarding this as an option following limb loss. METHODS: Current literature, in addition to the international registry on hand and composite tissu

Combined third and fourth toe transplantation.

Multiple toe transplantation has been established as an effective technique for restoring prehensile ability in the metacarpal hand. The combined third and fourth toe transplant is a new toe transplant that has been developed for bilateral metacarpal hand reconstruction. Since 1991, the combined third and fourth toe transplant was added to the armamentarium of toe transplants for metacarpal hands, although its role in metacarpal hand reconstruction has not been described previously. This article introduces the indications for combined third and fourth toe transplant, outlines relevant vascular

Great Toe Transplantation.

Despite being relatively uncommon in the general population, thumb amputations cause severe disability. More than 3,300 thumb amputations occurred in the United States. The thumb makes up around 40% of the function of the hand. Therefore, losing it would result in significant medical, hospital, and societal costs. Thumb reconstruction surgery's primary goal is to restore grip strength, including the range of motion, fine and tripod pinch, power grasp, strength, and sensibility, while secondary goals include restoring hand aesthetics. In cases of thumb replantation, like-for-like replacement is
