Outcomes Following Posterior and Posterolateral Plating of Distal Fibula Fractures.

Ankle fractures are common orthopaedic injuries often requiring open reduction and internal fixation. A variety of positions for plate placement exist for surgical fixation of distal fibula fractures, including direct lateral, posterior, or posterolateral. Direct lateral is more common, despite evidence that posterior and posterolateral placement is mechanically superior. However, concern for peroneal tendon injury from posterior plating remains. Our study investigates clinical and functional outcomes of patients who underwent operative fixation of distal fibula fractures with posterior or pos

Imaging evaluation of condylar remodeling after open reduction and internal fixation of unilateral mandibular condylar fracture in pediatric patients.

OBJECTIVE: This study aimed to evaluate the radiographic changes in damaged condyles after open reduction and internal fixation (ORIF) in pediatric patients with unilateral condylar fractures. STUDY DESIGN: We retrospectively analyzed patients aged ≤ 18 years who underwent ORIF for unilateral condylar fractures. The study group comprised fractured condyles, while contralateral healthy condyles served as controls. We assessed post-ORIF condylar remodeling and analyzed the influencing factors. We also compared condylar remodeling before and after internal fixation hardware removal. RESULTS: Th

Management of device-related wound complications in deep brain stimulation surgery.

OBJECT: Wound complications are uncommon following deep brain stimulation (DBS) surgery. However, certain key technical steps can be performed in each procedure to minimize this still troublesome risk. The authors reviewed the incidence and management of all hardware-related wound dehiscences and infections in a large patient series. METHODS: All patients undergoing new DBS hardware implantation surgery between 2002 and 2010 by a single surgeon (R.K.S.) were entered into a database after undergoing verification by cross-referencing manufacturer implantation records. All hardware-related compl

Clinical Outcomes of Bioresorbable Versus Titanium Fixation in Anterior Maxillary Segmental Osteotomy: A Randomized Clinical Trial.

This study aims to compare the clinical outcomes of bioresorbable and titanium fixation systems used in anterior maxillary segmental osteotomy (AMSO) procedures, with a focus on skeletal stability, postoperative complications, and hardware-related morbidity. A prospective, randomized clinical trial was conducted on 50 patients undergoing AMSO. Participants were randomly assigned to 2 groups: bioresorbable fixation using poly- L -lactic acid/polyglycolic acid (PLLA/PGA) (n=15) and conventional titanium miniplates (n=35). Standardized panoramic and cephalometric radiographs were taken preoperati

Risk Factors for Complications in Ulnar Shortening Osteotomies: A Multicenter Retrospective Review.

PURPOSE: Ulnar shortening osteotomy (USO) is commonly performed to alleviate pathologies causing ulnar-sided wrist pain. Surgical complications include nonunion and hardware removal, with rates up to 18% and 45%, respectively. The primary objective of the study was to report the overall complication rate of USO. The secondary objective was to identify risk factors for complications. METHODS: A retrospective multicenter cohort review was undertaken, including six Canadian cities over a 6-year period (January 2013-December 2018). Chart review was used to collect demographic data, surgical techn

Are resorbable plating systems comparable to conventional titanium plating in paediatric maxillofacial trauma patients? A systematic review and meta-analysis.

This systematic review and meta-analysis evaluated the effectiveness of resorbable versus titanium plating systems in the management of paediatric maxillofacial fractures. A comprehensive search of PubMed, Embase, Scopus, and specialty journals up to December 2024 identified six retrospective studies published between 2010 and 2024, involving 1,584 patients aged ≤18 years. Both resorbable and titanium systems provided satisfactory fracture stability and healing. Titanium fixation was consistently associated with higher complication rates, ranging from 8%-12% whereas resorbable systems demonstr

Extraction of Broken Intramedullary Nails Using Olive-Tipped Guidewire and Minifragment Screws.

In nonunion surgery, removing a broken intramedullary nail can be technically challenging and time-consuming, often requiring either creating a bone window or an open approach to retrieve the broken nail. A simple, effective, and minimally traumatic technique is presented for removing the difficult-to-access part of broken canulated intramedullary nails. First, the olive-tipped guidewire is inserted and advanced down the nail until the olive end protrudes out of the broken nail. A minifragment screw is inserted through a locking screw hole in the broken part under fluoroscopy. The guidewire is

Retrieval of entrapped intramedullary fragments using laparoscopic instruments for fixation of femur and tibia fractures.

Cortical fragments within the intramedullary canal can pose a challenge during intramedullary nail treatment of tibial and femoral shaft fractures. Complications including reamer entrapment, malunion, and inability to pass final implants have all been reported. To limit soft tissue injury and avoid an open approach, laparoscopic instrumentation can be a useful adjunct to manipulate or retrieve cortical fragments at distant sites. Our objective is to outline a method to remove cortical fragments during treatment of tibial and femoral shaft fracture and characterize the standard shaft lengths di

Superior compared with anteroinferior plating for mid-shaft fractures of the clavicle : a meta-analysis.

AIMS: Superior plating has long been the routine surgical approach for mid-shaft fractures of the clavicle. However, concerns about the prominence of the plate and neurological risk have led to an increased interest in anteroinferior plating. Whether anteroinferior plating offers comparable or improved outcomes without compromising the stability of fixation has been investigated in several studies. The aim of this meta-analysis was to evaluate the clinical outcomes, complications, and complication-related outcomes following superior compared with anteroinferior plating of mid-shaft fractures o

Migration of Spinal Hardware Into the Popliteal Vein: A Case Report.

CASE: A 41-year-old woman with prior T12-L4 instrumentation for adolescent lumbar scoliosis presented with left posteromedial thigh pain initially diagnosed as an adductor strain. Two years later, she re-presented with knee stiffness, lower leg mottling, and paresthesia suspected to be reflex sympathetic dystrophy. Radiographs later revealed a 13.6-cm radiopaque object in the popliteal fossa and rod absence from the lumbar spine. The spinal rod was removed from the popliteal vein by orthopaedic surgery. CONCLUSION: Radiographs are an easily obtained, low-cost, and high-yield resource that are

Predictors of Persistent Pain After Fixation of Distal Clavicle Fractures in an Active Military Population.

Patients who undergo open reduction and internal fixation of distal clavicle fractures have a high rate of hardware removal and persistence of symptoms, particularly when attempting to return to high-demand activities. This study evaluated the outcomes of military servicemembers after surgical treatment of distal clavicle fractures. The authors performed a retrospective analysis of active duty servicemembers who underwent open reduction and internal fixation of Neer type II distal clavicle fractures between October 17, 2007, and July 20, 2012, with a minimum of 2-year clinical follow-up. Secondary retrieval and complication management for failed separated instrument removal in the apical third in curved root canals: a case series.

BACKGROUND: Separated instruments in the apical third of the infected root canals can lead to persistent periapical lesions. Retrieval requires appropriate preoperative assessment and a skilled technique, especially in cases of separated instruments in the apical third of curved root canals. Otherwise, retrieval will lead to removal failure and concomitant complications. CASE PRESENTATION: Herein, we report two cases of separated instrument retrieval failure with complications due to inadequate preoperative assessment and an incorrect technical approach. In these cases, we re-evaluated the fe

Removal of cranial springs after spring-mediated cranioplasty.

Cranial spring hardware is generally removed 3 months after placement for spring-mediated cranioplasty. Spring removal is performed as an outpatient procedure under general anesthesia in approximately 15 minutes through the incision locations of the index procedure. Herein, the authors provide a multimedia demonstration of cranial spring hardware removal after spring-mediated cranioplasty for sagittal craniosynostosis. The video can be found here: https://vimeo.com/511179695.

Extraction of cannulated percutaneous screw from scaphoid: a simplified technique.

Removal of a cannulated headless screw following scaphoid fixation, when necessary, can prove to be difficult due to loss of bony integrity and the shape of the screw. This can result in greater exposure and more extensive soft tissue dissection for removal of the hardware. We describe a simple technique for percutaneous extraction of this hardware that can be done reliably with no additional equipment.

Does instrumentation removal cause curve progression in patients with adolescent idiopathic scoliosis?

Adolescent idiopathic scoliosis (AIS) is usually instrumented using a posterior approach. Hardware removal may be performed for specific clinical reasons. Little data is available on whether removal influences curve magnitude. The aim of the paper is to evaluate the impact of instrumentation removal on curve progression, and the safety and efficacy of the procedure. We analyzed 59 patients who underwent instrumentation removal. Curve types, reasons for removal, period between procedures, and Cobb angles: at baseline, immediately after correction, after removal and in follow-up were evaluated.

Editorial Commentary: Locking Plate Removal Is Safe and May Improve Function 1 Year After Medial Opening Wedge High Tibial Osteotomy.

The development of angular stable locking late fixation in realignment osteotomy has resulted in the ability to be more aggressive with weight bearing and rehabilitation after high tibial osteotomy. One of the downsides of some of these plate fixators is the degree of soft-tissue irritation and discomfort that can be experienced by many patients, resulting in the need for hardware removal. Studies have shown greater than 50% of patients requiring hardware removal with no resulting loss of correction, although there may be cultural reasons for the higher number, not solely attributable to the s

Review of Calcaneal Osteotomies Fixed With a Calcaneal Slide Plate.

BACKGROUND: The calcaneal slide osteotomy is a common procedure used for the surgical correction of heel varus and valgus deformities. A variety of fixation methods exist including screws and plates. The literature shows a high rate of hardware prominence with screws leading to subsequent removal of hardware. Few studies have examined the use of plates for fixation of a calcaneal osteotomy. The purpose of this study was to assess outcomes following fixation of a calcaneal osteotomy with a calcaneal slide plate. METHODS: This is a retrospective consecutive case series of all patients who under

A meta-analysis comparing complications of anterior versus dorsal osteotomy and plating for distal radial malunions.

This systematic review and meta-analysis compare the incidences of complications, reoperations and hardware removal between anterior and dorsal plating after corrective osteotomy for dorsally angulated distal radial malunions. A total of 403 patients were included; 253 patients underwent anterior corrective osteotomy and 150 underwent dorsal corrective osteotomy. Mean follow-up was 30 months. Anterior plating was associated with fewer reoperations (9% vs. 28%), less hardware removal (3% vs. 18%) and comparable major complications (5% vs. 6%) compared to dorsal plating. The adjusted model showe

Tibial interference screw removal following anterior cruciate ligament reconstruction.

A small number of patients developed pain and tenderness at the tibial tunnel following anterior cruciate ligament reconstruction. Twenty-three knees in 22 patients underwent removal of the tibial interference screw. Ten knees had a preoperative flexion contracture and underwent a concomitant procedure to address the loss of motion at the time of hardware removal. In the 13 knees with full extension, the interval between ligament reconstruction and screw removal averaged 16 months. Eleven of these knees also underwent arthroscopy, but no intra-articular causes of pain were identified. Roentgen
