Health Behavior Survey Among People Who Use Opioids: Protocol for Implementing Technology-Based Rapid Response Surveillance in Community Settings.

BACKGROUND: In 2018, 2 million Americans met the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition diagnostic criteria for an opioid use disorder, and 9.9 million Americans had misused prescription pain relievers the previous year. Despite a rapid increase in opioid misuse, opioid use disorders, and overdoses, data are limited on the behavioral and contextual risks as well as the protective factors fueling the opioid epidemic in some hard hit US cities-Atlanta, Los Angeles, and Las Vegas. Opioid use also contributes to the risk of other health problems such as HIV and hepatiti

How Personal Perspectives Shape Health Professionals' Perceptions of Fetal Alcohol Spectrum Disorder and Risk.

This article examines how health, allied health and social service professionals' personal perspectives about alcohol and the risks associated with alcohol consumption become non-clinical factors that may influence their professional practice responses in relation to fetal alcohol spectrum disorder (FASD). It presents findings derived from a qualitative, interview-based study of professionals from a range of health, allied health and social service professions in New Zealand. The data derived from these interviews revealed four frames of reference that practitioners use when thinking about alc

Examining the health and well-being profile of people experiencing primary homelessness: the social work perspective at a major metropolitan teaching hospital.

Objective The literature has established a significant increase in morbidity and mortality among people experiencing primary homelessness, along with a pattern of using acute hospitals in place of primary healthcare services. This study examined how health services can engage and support people experiencing homelessness. Methods Participants were prospectively identified by acute hospital social workers in the course of providing usual care. All participants identified as experiencing primarily homelessness immediately before acute inpatient hospital admission. This study used the Vulnerabilit

Emergency ambulance dispatch: is there a case for triage?

Emergency telephone calls for an ambulance (999 calls) are usually dealt with first-come first-served. We have devised and assessed criteria that ambulance dispatch might use to prioritize responses. Data were collected retrospectively on consecutive patients presenting to an accident and emergency (A&E) department after a 999 call. An unblinded researcher abstracted data including age, date, time, caller, location, reason for call and A&E diagnosis and each case was examined for ten predetermined criteria necessitating an immediate ambulance response--namely, cardiac arrest; chest pain; short

Alcohol and Suicide Risk: Examining the Role of Meaning-Making.

OBJECTIVE: Conceptual understandings of meaning-making suggest that it may have protective value in regard to alcohol misuse and suicidal behavior. Accordingly, the aim of this study was to examine whether meaning-making attenuated the relationship between alcohol misuse and suicide risk severity in a population of active-duty service members. METHODS: The sample (N = 97) was recruited while presenting for emergency behavioral health services in circumstances indicative of high-risk suicidality: endorsing current suicidal ideation with intent to die. Those who reported ideation with a lifetim

Differences in 5-year survival after a 'homeless' or 'housed' drugs-related hospital admission: a study of 15--30-year olds in Scotland.

BACKGROUND: Young drug misusers and the homeless both have a greater risk of death than their peers. This study sought to estimate the additional impact of homelessness on the risk of death for young drugs misusers. METHODS: From all admissions to NHS hospitals in Scotland between 1986 and 2001, those that were: drug misuse related, for people born between 1970 and 1986 and aged over 15 years (n=13 303), were selected. All subsequent admissions and registrations of death were linked to this dataset. Each admission was coded as homeless if the health board of residence was coded as 'no fixed a

Co-morbidity and cannabis use in a mental health trust in South East England.

AIM: The prevalence of co-morbidity (severe mental illness and substance) may be less in rural and semi-rural areas than inner cities. The aims were therefore to measure the prevalence of co-morbidity among patients of attending a mental health service in a semi-rural area South East England. DESIGN AND PARTICIPANTS: Cross-sectional prevalence survey of 1,808 patients with detailed assessments from a representative sample of 373 patients identified as having a combination of severe mental illness and substance misuse. Interviews with key workers were performed using validated methods from the

"My home is (now) at peace": Evaluating the relevance, acceptability and potential scalability of a guided self-help intervention for male refugees in Uganda.

In humanitarian crises, male refugees face significant mental health challenges, including high rates of depression and alcohol misuse. However, access to adequate mental health services is limited. In this study, we evaluate a combined intervention that addresses both mental health and alcohol misuse among male refugees in Uganda, focusing on its relevance, acceptability, and potential for scalability. We conducted a qualitative study, using process evaluation data, to evaluate a combined guided self-help intervention for mental health and alcohol use among male refugees in Uganda's Rhino and

Effectiveness of interventions targeting frequent users of emergency departments: a systematic review.

STUDY OBJECTIVE: Frequent users of emergency departments (EDs) are a relatively small group of vulnerable patients accounting for a disproportionally high number of ED visits. Our objective is to perform a systematic review of the type and effectiveness of interventions to reduce the number of ED visits by frequent users. METHODS: We searched MEDLINE, EMBASE, CINAHL, PsychINFO, the Cochrane Library, and ISI Web of Science for randomized controlled trials, nonrandomized controlled trials, interrupted time series, and controlled and noncontrolled before-and-after studies describing intervention

Variation in suicide rates between Health Board areas.

This paper examines variations in suicide in the eight Health Boards of the Republic of Ireland for the years 1976 to 1995. It is found that while all have experienced a rise in male suicide, it has been much less pronounced in the Eastern Health Board which is somewhat surprising when one considers Dublin's much-publicised problems with homelessness and hard drug misuse. Since the mid-eighties, female rates have been somewhat higher in the southern half of the country, comprising the Southern, Mid-Western and South-Eastern Health Boards. This variation may reflect a difficulty with contacting

Suicide in South Wales: comprehensive review of suicides reported to the South Wales Central Coroner's Service, 2016-2023.

AIMS AND METHOD: An analysis of coronial inquest data provides contextual information rarely available in routine surveillance systems, including a nuanced understanding of proximal circumstances preceding death of value to mental health services. Inquest records of more than 750 suicides occurring within the South Wales Central Coroner's jurisdiction between 2016 and 2023 were reviewed, including the COVID-19 pandemic years, to assess the effects of lockdown and social isolation. Detailed information was extracted, including medical records, toxicology, police reports, witness statements and

The association between somatic complaints and past-30 day opioid misuse among justice-involved children.

BACKGROUND: Individuals in the criminal justice system are especially vulnerable to the adverse effects of opioid misuse. Research on justice-involved children (JIC) is needed to uncover the variables that predict opioid misuse initiation to prevent misuse or reduce harm in this population. Somatic symptoms are symptoms experienced in the body, such as physical sensations, movements or experiences, which can cause severe distress and dysfunction. These include pain, nausea, dizziness, and fainting. In this study, we hypothesize that somatic complaints will be associated with a higher likelihoo

Health needs and co-morbidity among detainees in contact with healthcare professionals within police custody across the London Metropolitan Police Service area.

AIMS: Detainees requiring access to healthcare services in police custody have been shown to suffer from poor physical and mental health, often exacerbated by substance misuse. This study examines the extent and nature of health needs in police custody across the Metropolitan Police Service (MPS), London. METHODS: A survey (n = 1657) was administered by Healthcare Professionals (HCP) for one month in 2015 across all MPS custody suites representing a 73% response rate. A logistic regression model was created using four binary outcomes (whether a detainee was a drug user, had mental health issu

Stakeholder perspectives on the scalability of a psychological intervention for alcohol misuse and psychological distress in wartime: A qualitative study in Ukraine.

The war in Ukraine has intensified mental health vulnerabilities, including alcohol misuse among conflict-affected men. The CHANGE intervention, a transdiagnostic mental health programme building on WHO's Problem Management Plus (PM+), addresses alcohol misuse and common mental disorders among conflict-affected populations. This study explores stakeholder perspectives on the scalability of CHANGE under active wartime conditions in Ukraine. Here, scalability refers to the potential for future scale-up rather than retrospective evaluation of actual scale-up, feasibility or effectiveness. Guided

[Optimal care needs daring doctors].

Doctors should deliver optimal care to patients, i.e. care in which it is the well-being of the person rather than the disease that is the focus. The present financial management structure together with the domination of guidelines and the increase in defensive medicine pushes doctors to deliver maximal rather than optimal care, resulting in overdiagnosis and overtreatment. In order to provide optimal care doctors have to act daringly.
