Epidemiology and Management of Postoperative Infections in Hepatectomy and Pancreaticoduodenectomy.

Infections remain a major source of postoperative morbidity and mortality following complex bile duct surgeries, namely hepatectomy and pancreaticoduodenectomy. Infection rates remain high-ranging from 24% to 61%-and are influenced by multiple factors, including patient comorbidities, surgical complexity, and perioperative variables such as prolonged operative time, excessive blood loss, and preoperative biliary drainage. Disruption of the biliary barrier due to drainage or stent placement promotes bile stasis, microbial colonization, and biofilm formation, facilitating the emergence of multid…

Laparoscopic hepatectomy for the treatment of hepatic alveolar echinococcosis.

BACKGROUND: At present, laparoscopy is relatively mature as a minimally invasive technique, but there are few reports on this approach for the radical treatment of hepatic alveolar echinococcosis (AE). In this study, we aimed to evaluate the safety and feasibility of laparoscopic hepatectomy (LH) for AE treatment. RESULTS: A retrospective review of medical records obtained from 13 patients diagnosed with AE between January 2018 and December 2019 and treated with laparoscopic hepatectomy was conducted at the First Affiliated Hospital of Xinjiang Medical University. All patients (n = 13) underw…

The safety and efficacy of laparoscopic hepatectomy in obese patients.

BACKGROUND: Obesity is generally reported to increase the risk of surgical complications. There have been few reports of laparoscopic hepatectomy (LH) in obese patients. The purpose of this study was to compare the safety and efficacy of (1) LH versus open hepatectomy (OH) in obese patients and (2) LH in obese patients versus LH in non-obese patients. METHODS: We introduced LH at our institution in April 2014. LH was performed in 63 obese patients and 108 non-obese patients from April 2014 to May 2017. OH was performed in 79 obese patients from January 2010 to May 2017. This study retrospecti…

Marginal hepatectomy in the rat: from anatomy to surgery.

OBJECTIVE: Based on the 3-dimensional visualization of vascular supply and drainage, a vessel-oriented resection technique was optimized. The new surgical technique was used to determine the maximal reduction in liver mass enabling a 50% 1-week survival rate. BACKGROUND DATA: Determination of the minimal liver mass is necessary in clinical as well as in experimental liver surgery. In rats, survival seems to depend on the surgical technique applied. Extended hepatectomy with removal of 90% of the liver mass was long regarded as a lethal model. Introduction of a vessel-oriented approach enabled…

[Anesthesia for hepatectomy in a patient with glycogen storage disease].

We successfully anesthetized a 19-year-old female with type I glycogen storage disease for hepatectomy. She had hypoglycemia and severe metabolic acidosis before surgery. General anesthesia was performed with epidural anesthesia. We used acetated Ringer solution mainly as intraoperative fluids instead of lactated Ringer solution and controlled administration of glucose determining blood glucose. The patient's plasma lactate levels, pyruvate and base excess during and after operation, were unchanged compared with those before operation. Administration of sodium bicarbonate was not necessary. In…

Volumetric Remodeling of the Left Liver After Right Hepatectomy: Analysis of Factors Predicting Degree of Hypertrophy and Post-hepatectomy Liver Failure.

BACKGROUND: This study investigated the volumetric remodeling of the left liver after right hepatectomy looking for factors predicting the degree of hypertrophy and severe post-hepatectomy liver failure (PHLF). METHODS: In a cohort of 121 right hepatectomies, we performed CT volumetrics study of the future left liver remnant (FLR) preoperatively and postoperatively. Factors influencing FLR degree of hypertrophy and severe PHLF were identified by multivariate analysis. RESULTS: After right hepatectomy, the mean degree of hypertrophy and kinetic growth rate of the left liver remnant were 25% a…

Long-term outcome of hepatectomy following conversion therapy for initially unresectable hepatocellular carcinoma versus direct hepatectomy for early-stage hepatocellular carcinoma.

BACKGROUND: Although emerging evidence has reported the efficacy and safety of sequential surgery following targeted and immunotherapy conve … tors and anti-angiogenic targeted drugs, with a median duration of five cycles, followed by radical hepatectomy. One hundred and eleven patients with Barcelona Clinical Liver Cancer (BCLC) stage A HCC only underwent direct hepatectomy. Clinical characters, surgical features and survival outcomes of the two groups were compared. RESULTS: In this cohort study …

Abnormal Distribution and Upregulation of Tenascin-C mRNA Expression in the Liver of 90% Hepatectomy Rats are Related to Post-hepatectomy Liver Failure.

BACKGROUND: Post-hepatectomy liver failure (PHLF) is the most serious complication after extended hepatectomy, with a high perioperative mortality rate. Matricellular protein tenascin-C (TNC) has been related to blood flow change and inflammation. We aimed to explore the hepatic changes of TNC and its relationship with PHLF. MATERIALS AND METHODS: Rats were divided into seven groups: Sham operation, 90% partial hepatectomy (90PH) as a PHLF model, 85% (85PH), and 68% (68PH) hepatectomy without or with lipopolysaccharide (LPS) treatment. The distribution of TNC protein, the mRNA expression of T…

A systematic review and meta-analysis of associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) versus traditional staged hepatectomy.

It is still controversial whether associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) or traditional staged hepatectomy such as portal vein embolization (PVE) and 2-staged hepatectomy (TSH) is better. The aim of this study was to compare these 3 available strategies in extended hepatectomy.Trials were identified by searching MEDLINE, PubMed, the Cochrane Library, and Embase and additional articles were identified by hand searching. Comparative clinical studies reporting volumetric changes, mortality, morbidity, and feasibility of the second stage about ALPPS ver…

Current status of associating liver partition with portal vein ligation for staged hepatectomy: Comparison with two-stage hepatectomy and strategies for better outcomes.

Since its introduction in 2012, associating liver partition with portal vein ligation for staged hepatectomy (ALPPS) has significantly expanded the pool of candidates for liver resection. It offers patients with insufficient liver function a chance of a cure. ALPPS is most controversial when its high morbidity and mortality is concerned. Operative mortality is usually a result of post-hepatectomy liver failure and can be minimized with careful patient selection. Elderly patients have limited reserve for tolerating the demanding operation. Patients with colorectal liver metastasis have normal l…

Synchronous hepatectomy and splenectomy vs hepatectomy for selected patients with hepatocellular carcinoma and clinically significant portal hypertension: A systematic review and meta-analysis.

This review aimed to evaluate the safety, feasibility, and effectiveness of synchronous hepatectomy and splenectomy for selected patients with hepatocellular carcinoma and clinically significant portal hypertension compared to hepatectomy alone. A systematic search in electronic databases was conducted. A total of 8 studies including 1445 patients were reviewed. While perioperative safety was satisfactory, synchronous hepatectomy and splenectomy were associated with a significant improvement of 5-year overall survival (odds ratio = 1.81; 95% confidence interval: 1.35, 2.42; P < 0.0001).

Progressive hepatopathy after central division hepatectomy in a dog.

The consequences of large-volume hepatectomy on the remaining liver in the dog are not documented. This case report documents a progressive hepatopathy after central division hepatectomy in a 10-year-old female Lhasa Apso dog. Serum biochemistry at presentation for chronic urinary incontinence indicated a hepatopathy, with a liver mass identified on subsequent ultrasound. Referral for computed tomography (CT) (day 76) demonstrated a central division liver mass (~204cm3). A central division hepatectomy (right medial and quadrate lobes; gallbladder) was performed, with microscopic examination co…

Microwave ablation and portal vein embolization for staged hepatectomy.

Associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) enables resection of extensive colorectal liver metastases (CRLM) by inducing rapid hypertrophy of the future liver remnant (FLR). However, the classical approach is often associated with significant morbidity, particularly in patients with compromised liver parenchyma. We present a case utilizing the associating microwave ablation and portal vein ligation for staged hepatectomy (AMAPS) technique as a modified ALPPS procedure in a patient with chemotherapy-induced liver injury and bilobar CRLM.
