Humidification during high-frequency oscillation ventilation is affected by ventilator circuit and ventilatory setting.

BACKGROUND: High-frequency oscillation ventilation (HFOV) is an accepted ventilatory mode for acute respiratory failure in neonates. As conventional mechanical ventilation, inspiratory gas humidification is essential. However, humidification during HFOV has not been clarified. In this bench study, we evaluated humidification during HFOV in the open circumstance of ICU. Our hypothesis is that humidification during HFOV is affected by circuit design and ventilatory settings. METHODS/MATERIALS: We connected a ventilator with HFOV mode to a neonatal lung model that was placed in an infant incubat

Effect of oral high frequency ventilation by jet or oscillator on minute ventilation in normal subjects.

Normal subjects were asked to breathe through an open ended tube while high frequency oscillations were superimposed on tidal breathing via a side arm, either an eight inch (20 cm) loudspeaker or a jet ventilator being used. Both systems were comfortable and well tolerated. Spontaneous minute ventilation fell by 19-46% at frequencies up to 33 Hz without a rise in transcutaneous PCO2. Maximum ventilatory savings occurred at 1.6 Hz with the jet ventilator (p less than 0.01) and at a frequency corresponding to respiratory system resonance with the loudspeaker. This suggests that during oral high

Carbon Dioxide Levels in Extremely Preterm Neonates Ventilated With High Frequency Jet Ventilation.

BACKGROUND: Close monitoring of carbon dioxide (CO2) levels is crucial in extremely preterm neonates; however, a consensus on the optimal CO2 thresholds is lacking. We aimed to evaluate the impact of PaCO2 levels during the first 24 h of life on the outcomes of extremely preterm neonates ventilated with first-intention high frequency jet ventilation (HFJV). METHODS: We retrospectively examined neonates ≤26 weeks of gestational age who were ventilated with first-intention HFJV, during 2022-2024, comparing the lowest PaCO2, peak PaCO2, PaCO2 fluctuations, and PaCO2 deviation from the target (40

A comparison of the effects of high frequency--low tidal volume and low frequency--high tidal volume mechanical ventilation.

Ten neonates with severe lung disease were studied while mechanically ventilated with standard volume preset infant ventilators, using two different ventilatory patterns. Slow ventilatory rates and high tidal volumes were alternated with rapid rates and low tidal volumes; minute ventilation, FIo2, PEEP, and I:E ratios were held constant. Peak inspiratory pressure, mean airway pressure, expiratory time, Pao2, Paco2, pH, and arterial blood pressure were measured and compared for each frequency-tidal volume combination. The best arterial oxygenation occurred at the combination of settings that pr

Non-invasive high-frequency ventilation in newborn infants with respiratory distress.

BACKGROUND: Respiratory distress occurs in up to 7% of newborns, with respiratory support (RS) provided invasively via an endotracheal (ET) tube or non-invasively via a nasal interface. Invasive ventilation increases the risk of lung injury and chronic lung disease (CLD). Using non-invasive strategies, with or without minimally invasive surfactant, may reduce the need for mechanical ventilation and the risk of lung damage in newborn infants with respiratory distress. OBJECTIVES: To evaluate the benefits and harms of nasal high-frequency ventilation (nHFV) compared to invasive ventilation via …

Lung Recruitment Strategies During High Frequency Oscillatory Ventilation in Preterm Lambs.

Background: High frequency oscillatory ventilation (HFOV) is considered a lung protective ventilation mode in preterm infants only if lung volume is optimized. However, whilst a "high lung volume strategy" is advocated for HFOV in preterm infants this strategy is not precisely defined. It is not known to what extent lung recruitment should be pursued to provide lung protection. In this study we aimed to determine the relationship between the magnitude of lung volume optimization and its effect on gas exchange and lung injury in preterm lambs. Methods: 36 surfactant-deficient 124-127 d lambs co

High-frequency oscillatory ventilation with volume guarantee: a single-centre experience.

OBJECTIVE: High-frequency oscillatory ventilation (HFOV) is widely used in neonatology. The Dräger Babylog VN500 ventilator offers volume-guaranteed HFOV (HFOV-VG) mode when the high-frequency tidal volume (VThf) to be delivered can be set. We investigated how HFOV-VG maintains VThf in the short and longer term and how it affects other ventilator parameters and blood gases. METHODS: We downloaded ~3.2 million seconds (36.7 days) of ventilator data from 17 infants ventilated using HFOV-VG during clinical care with 1 Hz sampling rate. To process and analyse the data, we used the Python computer

Use of high frequency oscillatory ventilation (HFOV) in neurocritical care patients.

INTRODUCTION: Adult respiratory distress syndrome (ARDS) can be a common problem associated with the treatment of acute brain injury. High frequency oscillatory ventilation (HFOV) is a developing therapy for the treatment of ARDS in adult patients that can be life saving. However, often patients with acute, severe brain injury demonstrate intracranial hypertension (hICP) due to a variety of injuries (e.g., traumatic brain injury, mass lesion, acute hydrocephalus). There is concern over the use of HFOV due to its effects on intracranial pressure in patients with hICP. METHODS: Retrospective ca

The role of high-frequency ventilation in neonates: evidence-based recommendations.

High-frequency ventilation (HFV) uses small tidal volumes and extremely rapid ventilator rates. Despite the wealth of laboratory and clinical research on HFV, there are no established guidelines for prioritizing the use of HFV versus conventional mechanical ventilation (CMV) in neonatal respiratory failure. Examination of the currently available randomized controlled trials and meta-analysis of HFV versus CMV does not demonstrate any clear benefit of HFV either as a primary mode or as a "rescue" mode of ventilation in neonates who have respiratory insufficiency. The current literature does sup…

Safety and efficacy of high-frequency jet ventilation in neonatal transport.

OBJECTIVE: To evaluate the safety and efficacy of high-frequency jet ventilation for transporting critically ill hypoxic neonates to an extracorporeal membrane oxygenation (ECMO) center. STUDY DESIGN: We conducted a retrospective cohort study of 38 transported neonates. Safety was assessed by the comparison of cardiopulmonary variables before and after transport from referring hospital to our ECMO unit. Efficacy was assessed as the effect on ventilation and efficiency of pulmonary gas exchange after conversion from a conventional mechanical ventilator or a high-frequency oscillator to a high-

The role of high-frequency oscillatory ventilation in paediatric intensive care.

Mechanical ventilation during acute respiratory failure in children is associated with development of ventilator-induced lung injury. Experimental models of mechanical ventilation that limit phasic changes in lung volumes and prevent alveolar overdistension appear to be less damaging to the lung. High-frequency oscillatory ventilation, using very small tidal volumes and relatively high end-expiratory lung volumes, provides a safe and effective means of delivering mechanical ventilatory support with the prospect of reducing the development of ventilator-induced lung injury. Despite theoretical

Heliox administration during high-frequency jet ventilation augments carbon dioxide clearance.

We report the combined use of heliox and high-frequency jet ventilation to augment carbon dioxide clearance, with a focus on the important technical considerations. Our case is a 5-month old infant with acute respiratory failure associated with gas trapping, hypercarbia, respiratory acidosis, and air leak. Despite maximal conventional ventilation, bronchodilator therapy, corticosteroids, and sedation, the infant continued to demonstrate worsening gas exchange necessitating an escalation of support to high-frequency oscillatory ventilation. After the development of an air leak and continued dif

High frequency percussive ventilation in burn patients: hemodynamics and gas exchange.

High frequency percussive ventilation (HFPV) is a recent ventilatory mode, which combines conventional cycles with high frequency percussions. HFPV was initially instituted as salvage therapy after acute respiratory failure following smoke inhalation injury achieving in each case a dramatic improvement of blood oxygenation, PaCO(2) and ventilatory pressures. This study investigates the influence of HFPV on hesmodynamics, blood oxygenation and ventilatory parameters in eight stable ICU burn patients requiring artificial ventilatory support during a postoperative period following traumatic injur

High frequency oscillatory ventilation during repair of neonatal congenital diaphragmatic hernia.

We evaluated the use of high frequency oscillatory ventilation (HFOV) during congenital diaphragmatic hernia repair. After preoperative stabilization, 22 newborn infants were ventilated with HFOV during surgery. Ventilatory settings, blood gas values and oxygenation index were recorded before, during and after surgical repair. No differences were noted for these variables. No complications related to ventilation were recorded. According to the surgeon, diaphragmatic repair during HFOV is facilitated. This study confirms that CDH can be safely repaired using HFOV during anaesthesia.

[High-frequency oscillatory ventilation in pediatric patients. protocol and preliminary results].

BACKGROUND: High-frequency oscillatory ventilation (HFOV) constitutes an important advance in the management of children with respiratory failure. Although it has been used mainly as "lung rescue therapy", pediatric indications for HFOV can be broader. The principal advantages of this modality compared with conventional ventilation are the lower incidence of barotrauma, volutrauma, atelectrauma and biotrauma. To date, experience with HFOV in our country has been scarce and limited to neonatal patients. AIM: To describe the HFOV protocol for pediatric patients and to report the preliminary res

Respiratory distress syndrome and tracheoesophageal fistula: management with high-frequency ventilation.

An 1180-g infant with esophageal atresia and tracheoesophageal fistula developed life-threatening respiratory distress syndrome. Conventional mechanical ventilation resulted in gastric perforation and pneumoperitoneum. High-frequency ventilation stabilized the infant, permitting distal occlusion of the esophagus with a Silastic band. Fistula ligation was subsequently performed under more optimal physiologic conditions.
