Total hip arthroplasty in chronic unreduced hip fracture-dislocation.

We treated 15 patients with chronic unreduced hip fracture-dislocations at our hospital; all patients sustained the fracture-dislocations in motor vehicle accidents. All presented to our institution more than 6 months after initial treatment at their local hospitals with uncontained femoral heads; all underwent 1-stage total hip arthroplasty with bone grafting. These patients were monitored for a mean of 71.5 months (range, 36-96 months). All patients had significantly decreased pain, increased function, and increased range-of-motion scores using the Merle d'Aubigné scoring system. All grafts

Do hip replacements improve outcomes for hip fracture patients?

BACKGROUND: Hip fracture is a common problem among older Americans. Two types of procedures are available for repairing hip fractures: hip replacement and open or closed reduction with or without internal fixation. The assumption has been that hip replacement produces better functional outcomes. Although that is the common wisdom, outcome studies evaluating hip replacement for treatment of hip fracture are few and have not clearly documented its superiority. OBJECTIVES: To compare outcomes of hip fracture patients who receive hip replacement versus another stabilizing procedure (open or close

Fragility Fractures in the Hip and Distal Radius: Difference in the Age-groups and Its Implication in Preventing Hip Fractures.

BACKGROUND: Fragility fractures are commonly seen in the distal radius and hip in osteoporotic individuals. While hip fractures have high mortality rates, distal radius fractures are known to cause morbidity in the form of wrist stiffness. This study aims to compare the chronological age of patients who sustain distal radius and hip fractures following a trivial injury. METHODS: The study was done by reviewing available in-patient medical records between January 2019 and July 2024 of patients aged more than 50 years. The data was analysed on Jamovi software version 2.6.17 by using a Mann-Whit

Relative survival following hemi-and total hip arthroplasty for hip fractures in Sweden.

BACKGROUND: Hip fractures are a common problem in the ageing population. Hip arthroplasty is the common treatment option for displaced intracapsular neck of femur fractures. Even though hip replacements are successful in restoring mobility, reducing pain and diminishing loss of health-related quality of life, the potential impact of a hip fracture on life expectancy as well as the postoperative mortality need consideration. The purpose of this study was to describe the mid-term relative survival rate for a cohort of Swedish patients whom underwent total- or hemiarthroplasty surgery following h

Prior fracture and refracture among patients admitted with hip fracture: Data from a regional hip fracture registry in Sri Lanka.

AIM: Fragility hip fracture has become a major global health concern. Data related to recurrent fragility fractures are sparse in the South Asian region. METHOD: We assessed the prevalence of prior fragility fracture among patients admitted with new hip fracture to a tertiary care hospital in southern Sri Lanka. Also the incidence of refracture was assessed among hip fracture survivors followed up for 24 months after discharge. RESULTS: Three hundred and nine patients with incident hip fracture were admitted during the study period (June 2014-February 2015) and 11 patients died while in the

Impact of COVID-19 on hip fracture care in Ireland: findings from the Irish Hip Fracture Database.

PURPOSE: To describe the impact of COVID-19 on hip fracture care during the first 6 months of the pandemic. METHODS: A secondary analysis of 4385 cases in the Irish Hip Fracture Database from 1st June 2019 to 31st August 2020 was conducted. RESULTS: Hip fracture admissions decreased by 15% during the study period (p < 0.001). Patient characteristics were largely unchanged as the majority of cases occurred in females over 80 years admitted from home. Adherence to many of the Irish Hip Fracture Standards (IHFS) changed following the COVID-19 pandemic. There was an increase in patients admitted

External validation of the Hip Fracture Prognosis Tool to predict the probability of gait recovery after hip fracture.

PURPOSE: The Hip Fracture Prognosis (HF-Prognosis) Tool was developed using data from the Spanish National Hip Fracture Registry (SNHFR) to predict 30-day recovery of ambulation. Our study's aim was to externally validate the model in a later cohort (2021-2022) to ensure its continued predictive accuracy and clinical utility. METHODS: Data were obtained from the SNHFR, a multicenter, prospective registry. Patients aged ≥ 75 years admitted for fragility hip fracture with information on pre-fracture ambulation were included. The primary outcome was recovery of pre-fracture walking ability at 1

Outcomes of Fast-Track Multidisciplinary Care of Hip Fractures in Veterans: A Geriatric Hip Fracture Program Report.

BACKGROUND: Hip fractures are a significant cause of morbidity and mortality in the elderly. Fast-track multidisciplinary co-management of these patients, rapid preoperative optimization, early surgery, and expeditious rehabilitation may minimize morbidity and mortality. In this study, we evaluated outcomes of fixation of hip fractures in the elderly patients managed by Geriatric Hip Fracture Program at a military hospital in India. METHODS: A total of 114 patients above 60 years of age with hip fractures were enrolled. They were comanaged by a team of specialists and fast-tracked to surgery.

Improving morbidity and mortality in hip fragility fractures.

PURPOSE OF REVIEW: Hip fragility fractures (HFF) carry high morbidity and mortality for patients and will increase in frequency and in proportion to the average patient age. Provision of effective, timely care for these patients can decrease their morbidity and mortality and reduce the large burden they place on the healthcare system. RECENT FINDINGS: There are associative relationships between prefracture frailty, postoperative delirium and increased morbidity and mortality. The use of a multidisciplinary approach to HFF care has shown improved outcomes in care with focus on modifiable facto

Waterproof Hip Spica Casts for Pediatric Femur Fractures.

BACKGROUND: Hip spica casting is the treatment of choice for femur fractures in children ages 6 months to 5 years. Traditional spica (TS) casting utilizes cotton padding that precludes patient bathing. Waterproof (WP) casting has inherent advantages, including clearance for bathing and improved family satisfaction. This study examines the safety and efficacy of WP hip spica casting for the treatment of pediatric femur fractures. METHODS: This is a retrospective, matched cohort study of patients ≤5 years with a femur fracture treated with hip spica casting. Patients with WP casts were matched

Novel Anticoagulants and Hip Fractures in the Elderly.

Introduction Early surgery is recommended in hip fractures to reduce morbidity and mortality. Surgery is often delayed in patients on novel direct oral anticoagulants (DOACs). The purpose of our study was to investigate the impact of DOACs on patients with hip fractures. Methods A retrospective comparative analysis was performed. A total of 766 patients presented with neck of femur fractures in the study period. Patients under the age of 60, those managed conservatively and those on alternative anticoagulation (including warfarin, clopidogrel and aspirin) were excluded. Forty-seven (6.1%) pati

Comparison of two frailty indexes in hip fractures.

AIM: Modified-Krishnan's frailty index (FI) is an FI calculation method developed by Krishnan et al. in 2014. This study aimed to compare the effectiveness and correlation of the FIs from Krishnan and the Canadian study of health and aging (CSHA) in predicting postoperative outcomes of elderly patients with hip fracture. METHODS: Based on clinical follow-up and observation, we utilized these two instruments to predict 3-month mortality, hip function, and recovery of daily activities. The area under the curve (AUC) and the Pearson correlation coefficient were used to compare the two scales' pr

Morphological abnormalities of the hip in acetabular fractures.

Poor prognosis factors in surgical treatment of acetabular fracture-dislocations have been well established but there is little information about how morphological abnormalities of the hip may affect the surgical outcome. Hip anatomy has a wide range of variations. Morphological abnormalities of the hip can also be observed in patients with acetabular fractures. We present a case of a complication in a patient with a complex acetabular fracture, acetabular retroversion and femoroacetabular impingement. A 31-year old male patient was transferred to our trauma center following a high speed road

The Influence of Capsulotomy for Intracapsular Hip Fractures.

Young patients with femoral neck fractures undergo operative stabilization urgently to prevent complications of avascular necrosis (AVN) and nonunion. The necessity of capsulotomy to prevent these sequelae is disputed. A retrospective chart review performed on 113 patients who underwent fixation of a femoral neck fracture found 62 patients following exclusion criteria, who were analyzed for demographic and fracture characteristics, operative variables, and radiographic outcomes. Sixty-two patients (age 18-85), with a mean follow-up of 21.2 months, had a mean time to union of 5.3 months. Capsul
