BACKGROUND: Histoplasma capsulatum is a globally distributed dimorphic fungal pathogen endemic to the Americas, Africa, and Asia. It causes histoplasmosis, a disease acquired via inhalation of spores from contaminated environments. It thrives in nitrogen-rich soils and is disseminated by avian and chiropteran reservoirs. Extrapulmonary Histoplasma capsulatum infections in the immunocompetent population are rare and pose a diagnostic challenge. A sandwich enzyme immunoassay (EIA) for the detection of Histoplasma antigens (Ag) in urine, developed by Optimum Imaging Diagnostics (OIDx) was evaluated. A verification using a standardized reference panel of urine samples found sensitivity of 92%, specificity of 32% and accuracy of 51%. In this study, the OIDx Histoplasma urinary Ag EIA displayed high sensitivity, however, in non-histoplasmosis cases this EIA displayed false-positive results in 68% of specimens tested.

A patient with progressive disseminated histoplasmosis was noted to have an increase in urine Histoplasma antigen level during monitoring of her disease. The patient revealed she had inadequately hydrated, and her urine volume was low and subjectively concentrated. Following hydration, urine antigen was retested and became undetectable.

The primary objective of the study was to investigate the risk factors for Histoplasma capsulatum fungemia. We conducted a retrospective case-control study among patients with histoplasmosis seen at Mayo Clinic in Rochester from 1 January 1991 through 31 December 2005. Histoplasmosis is an endemic mycosis in some regions in the US, but it can be more and more often imported to Europe. One of the clinical characters of the infection is the variety of shapes it can have, many similar to tuberculosis. It can be associated to HIV infection, being the most frequent mycosis in these patients in endemic regions. Diagnosis is based on evidence of mycotic filaments on microscopy or culture. Treatment consists of Amphotericin B and Itraconazole, the latter being also the foundation of primary and secondary prevention.

Human infection with Histoplasma capsulatum runs the gamut from asymptomatic to disseminated disease. CT-directed fine-needle aspiration of bilaterally enlarged adrenal glands has been used in diagnosing serious infections with this ubiquitous organism. Three cases have previously been reported in which H. capsulatum infection caused unilateral adrenal enlargement; this enlargement was diagnosed post-mortem. The epidemiological, clinical and therapeutical findings are described in a case of recurrent pulmonary histoplasmosis due to Histoplasma capsulatum. Using Immuno-Mycologics (IMMY; Norman, Okla.) histoplasmal yeast (HY) and mycelial (HM) antibody complement fixation test antigens, we retested 1,386 samples that were initially tested with Meridian Diagnostics, Inc. Previously reported cases of Histoplasma capsulatum infection of prosthetic heart valves have been first diagnosed at autopsy. A patient had an H capsulatum infection that was diagnosed by serologic means 1 1/2 years before involvement of the patient's prosthetic mitral valve was found at surgery.
