Helicobacter pylori colonization in the larynges of patients with hoarseness.

OBJECTIVES: Vocal nodules and polyps are two common noninfectious causes of hoarseness. Patients with persistent hoarseness often require microscopic laryngeal surgery to excise mass lesions of the larynx despite extensive voice resting and modification of voice use behavior. Helicobacter pylori has recently been reported to present in the upper aerodigestive tract. This study applies the rapid urease test to determine the colonization of Helicobacter pylori in surgical specimens of patients with vocal nodules and polyps. METHODS: In this prospective study, 53 consecutive patients with vocal …

Hoarseness -As a presenting feature of aortic arch aneurysm.

Hoarseness due to left recurrent laryngeal nerve paralysis (LRLN) caused by identifiable cardiovascular disease is described as Ortner's syndrome or Cardiovocal syndrome. This was first described by Ortner in 1897 to describe left recurrent laryngeal nerve palsy secondary to Mitral Stenosis. The common conditions which gave rise to Ortners' syndrome include mitral stenosis, mitral valve prolapse, aortic aneurysm, septal defect, following cardiothoracic surgery, high altitude pulmonary hypertension, ductal aneurysm, aortic dissection etc. Herewith we are presenting an unusual presentation of Ao…

[An eye on the vocal cords: family practitioner and hoarseness].

In four patients with hoarseness, men aged 53 and 67 years, and women aged 8 and 37 years, indirect laryngoscopy revealed, respectively, a squamous cell carcinoma of the vocal cord, recurrent nerve paralysis due to pulmonary carcinoma, irritative noduli due to forced use of the voice, and psychic stress as the cause. In a patient with existing hoarseness over a period of 3 to 6 weeks, the vocal cords will have to be examined with indirect laryngoscopy. Where the general practitioner is not equipped to handle this procedure, he can make a referral to an ear, nose and throat specialist. It would…

[Farber disease: a cause of hoarseness of the voice in children].

Farber's disease (disseminated lipogranulomatosis) is an autosomal recessive disease characterized by deposition of glycolipid ceramide in different tissues due to deficiency of lysosomal acid ceramidase. The disease starts to manifest at the age of four months by a hoarse cry or swollen tender joints followed by subcutaneous nodules. This disease is fatal in the first years of life and no treatment is known until now. This study presents four cases of Farber's disease who all presented by hoarseness of voice, polyarthritis and subcutaneous nodules. After clinical examination, the diagnosis wa…

A benign parathyroid cyst presenting with hoarse voice.

Parathyroid tumours and cysts are rare and, when presenting as neck masses, can be clinically misdiagnosed as thyroid lesions. Symptoms may be caused by compression of the surrounding structures or hormonal overactivity. This paper describes a patient with recurrent hoarseness owing to the pressure effects of a parathyroid cyst on the recurrent laryngeal nerve.

Hoarseness as the initial manifestation of systemic lupus erythematosus.

A 51-year-old woman complained of hoarseness of two years duration. The patient's past medical history was significant for autoimmunological hepatitis and arthritis for which she had not received treatment. Laryngoscopy and laryngeal stroboscopy revealed 'bamboo joint-like nodules' on both true vocal folds. These nodules resembled rheumatoid nodules and were suggestive of a collagen disease. Previous reports have documented that the treatment for such conditions related to collagen diseases is surgical resection. However, we initially attempted to treat the laryngeal lesions systemically with …

Histiocytosis: an unusual cause of dysphagia, hoarseness and stridor.

We present a patient with established histiocytosis who developed dysphagia, retching, regurgitation, hoarseness and stridor. These symptoms were managed with carbon dioxide laser vaporization, electively on three occasions, and once as an urgent procedure, while awaiting radiotherapy, to control her airway. Histiocytosis is a rare cause of a number of otolaryngological syndromes, but there has been no previous record of this disease causing laryngopharyngeal symptoms. This paper discusses the classification of histiocytosis, and describes our management of this rare and intriguing case.

Voice perturbations of children with perceived nasality and hoarseness.

Cycle-to-cycle variations in voice fundamental frequency (jitter) and amplitude (shimmer) were derived by electroglottography for 10 children with velopharyngeal insufficiency (VPI). Jitter was found to be positively correlated with ratings of perceived nasality, whereas shimmer was found to be positively correlated with ratings of perceived hoarseness. Theoretic implications for a regulatory model of speech aerodynamics are discussed. Additionally, clinical applications, in terms of using electroglottography as a supplemental assessment procedure, are suggested.

Evaluation of gastroesophageal reflux as a cause of idiopathic hoarseness.

Eleven patients presenting to an ear, nose, and throat specialist were diagnosed as having idiopathic hoarseness and prospectively evaluated for evidence of gastroesophageal reflux (GER) to determine if an association existed. Testing for GER included voice analysis, EGD, esophageal manometry, Bernstein test, and ambulatory 24-hr pH monitoring. Six of the 11 (55%) hoarse patients studied had GER by pH monitoring (mean score 105 +/- 23), and most reflux episodes were supine and prolonged (20.9 +/- 8.2% supine pH less than 4.0, longest 129 min). All patients with abnormal pH monitoring had endos…

Bilateral Horner's syndrome and hoarseness complicating lumbar epidural anesthesia.

A case of bilateral Horner's syndrome associated with hoarseness resulting from single-dose lumbar epidural analgesia for relief of low back pain in a 57-year-old man is reported. The pathophysiology of Horner's syndrome and hoarseness is discussed.

Primary mediastinal mucormycosis presenting with hoarseness: a case report.

BACKGROUND: Mucormycosis is a rare, rapidly progressive fungal infection caused by molds of the order Mucorales, most commonly Rhizopus species. While it predominantly affects immunocompromised patients, mediastinal mucormycosis is exceptionally rare, with fewer than 50 cases reported, and has a high mortality rate. Diagnosis is challenging owing to nonspecific clinical and radiologic findings, often mimicking malignancies such as lymphoma or thymoma. This study aims to report a rare case of primary mediastinal mucormycosis presenting with hoarseness and to highlight the diagnostic challenges …

Giant aortic arch aneurysm presenting with hoarseness of voice.

A middle-aged male with hypertension and dyslipidaemia presented with hoarseness of voice due to a left vocal palsy. The chest radiograph revealed a saccular opacity silhouetting the left border of the aortic knuckle, suggestive of a large aortic arch aneurysm. Routine investigations, such as a chest radiograph do occasionally identify potentially fatal pathology, even when perhaps not strictly indicated; uncommon presentations do occur!
