The Hyoid Bone Kinematics in Dysphagic Stroke Patients: Instantaneous Velocity, Acceleration and Temporal Sequence Matters.

Hyoid bone excursion (HBE) is one of the most critical events in the pharyngeal phase of swallowing. Most previous studies focused on the total displacement and average velocity of HBE. However, HBE during swallowing is not one-dimensional, and the change of velocity and acceleration is not linear. This study aims to elucidate the relationship between the instantaneous kinematics parameters of HBE and the severity of penetration/aspiration and pharyngeal residue in patients with stroke. A total of 132 sets of video-fluoroscopic swallowing study images collected from 72 dysphagic stroke patient…

Mechanisms of hyoid bone fracture after modelling: evaluation of anthropological criteria defining two relevant models.

Several studies have attempted to describe the morphology of the hyoid bone, while other authors have focused on discovering the role of this bone in the occurrence of fractures. Hyoid fractures are known to be dependent on the force applied against the bone, or on the location at which the force is applied. We wished to assess the value of defining one or more models of the hyoid bone by analyzing variations in the size and angle of the various component parts of the bone relative to the sex and morphology of an individual (height and weight) in a sample of 72 bones obtained during forensic a…

Functional Masticatory Angle and Hyoid Bone Position: A Pilot Study on Occlusal Symmetry and Morphofunctional Adaptation.

Background: The hyoid bone is a key anatomical structure involved in the functional coordination of the stomatognathic system. Although its position may vary in response to masticatory patterns, its relationship with functional occlusion remains insufficiently studied in orthodontics. Objective: This pilot study aimed to explore the association between masticatory type and hyoid bone position and to assess the clinical utility of the Functional Masticatory Angle of Planas (AFMP) in classifying masticatory patterns. Materials and Methods: A descriptive, observational, cross-sectional study was …

Treatment of hyoid bone syndrome with surgical excision: our monocentric experience in the past five years.

PURPOSE: In 1968, Steinmann described the hyoid bone syndrome as a degenerative and inflammatory insertion tendinosis. It causes unilateral pain in the neck, increased by swallowing and palpation. The treatment is conservative, but when this fails, surgery is an option. Our aim is to assess the post-operative outcome of patients for whom a surgical hyoid bone resection procedure was performed in the University Hospital of Antwerp (UZA). METHODS: A retrospective study was performed. Patients were seen at the laryngology clinic in the UZA from 2018 until 2023. They were diagnosed with the hyoid…

Evaluation of the influence of hyoid bone position, volume, and types on pharyngeal airway volume and cephalometric measurements.

OBJECTIVES: We aimed to explore the position, morphological, and morphometric properties of the hyoid bone (HB) and to investigate the effect of HB on the pharyngeal airway (PA) volume and cephalometric measurements. METHODS: A total of 305 patients with CT images were included in the study. DICOM images were transferred to the InVivoDental three-dimensional imaging software. The position of the HB was determined based on the cervical vertebra level and in volume render tab, the bone was classified into six types after all structures around the HB were removed. Also, final bone volume was rec…

Hyoid bone displacement as parameter for swallowing impairment in patients treated for advanced head and neck cancer.

Reduced hyoid displacement is thought to contribute to aspiration and pharyngeal residues in head and neck cancer (HNC) patients with dysphagia. To further study hyoid elevation and anterior excursion in HNC patients, this study reports on temporal/kinematic measures of hyoid displacement, with the additional goal to investigate correlations with clinical swallowing impairment. A single-blind analysis of data collected as part of a larger prospective study was performed at three time points before and after chemoradiotherapy. Twenty-five patients had undergone clinical swallowing assessments a

[Analysis of a change in the position of the hyoid bone when displacing the lower jaw in patients with dentofacial malformations].

OBJECTIVE: To estimate the magnitude of a change in the position of the hyoid bone when displacing the lower jaw during surgical treatment in patients with dentofacial abnormalities. MATERIAL AND METHODS: Fifty patients with dentofacial abnormalities (25 and 25 patients with distal and mesial occlusion, respectively) were examined and treated. All the patients underwent multislice spiral computed tomography before and 6 months after surgery. The authors developed linear quantities to analyze the position of the hyoid bone with respect to the skull base and cervical spine. The paired Wilcoxon …

Traumatic fracture of the hyoid bone: three case presentations of cardiorespiratory compromise secondary to missed diagnosis.

Hyoid bone fractures secondary to blunt trauma other than strangulation are rare (ML Bagnoli et al., J Oral Maxillofac Surg 1988; 46: 326-8), accounting for only 0.002 per cent of all fractures. The world literature reports only 21 cases. Surgical intervention involves airway management, treatment of associated pharyngeal perforations, and management of painful symptomatology. The importance of hyoid fracture, however, rests not with the rarity of it, but with the lethal potential of missed diagnosis. We submit three cases with varying presentations and management strategies. All three of our …

Mandibular prognathism: a cineradiographic analysis of mandibular and hyoid bone movements before and after surgical correction.

A study was undertaken to investigate the effect of surgical treatment of mandibular prognathism on mandibular and hyoid movements during speech and swallowing in fourteen individuals. The material consisted of cineradiographic records of adult prognathic patients before and after the surgical correction. The records were obtained with a 16 mm, cinecamera at a speed of thirty-two frames per second. Image intensification was used, and the head of the patient was positioned in a cephalostat during the recording. Postoperative recordings were taken at about 1 month after the removal of intermaxil

The role of the sphenoid bone in the connection between the temporal-mandible-tongue-hyoid system and the cervical spine.

The cranio-cervical-mandibular system is a functional complex whose understanding has evolved significantly, moving from a purely mechanical model to an integrated view that includes myofascial and postural interdependencies. Recent studies, particularly the work of Messina, have led to the definition of the Temporal-Mandible-Tongue-Hyoid (TMTH) system, a biomechanical unit in which the temporal bone acts as a fulcrum. This article aims to analyze the often-underestimated role of the sphenoid bone as a key element of anatomical and functional connection between the TMTH system and the upper ce

Forensic value of soft tissue detachments from the hyoid bone in death due to strangulation asphyxia.

BACKGROUND: There are no unequivocal histopathological findings for the diagnosis of fatal asphyxia due to neck compression. From the observation of a series of asphyxiation cases, we noted, during microscopic analysis, a high frequency of "detachment" of soft tissues from the hyoid bone. This specifically refers to the presence of an optical space between the surface of the hyoid bone and soft tissues. OBJECTIVES: We aimed to evaluate the detachment of soft tissues from the hyoid bone as specific histological evidence of death due to strangulation asphyxia. MATERIAL AND METHODS: Ten blocks …

Hyoid Bone Fracture Pattern Assessment in the Forensic Field: The Importance of Post Mortem Radiological Imaging.

Post mortem hyoid bone fracture findings may be attributable to various factors, including both the onset of acute mechanical asphyxia as it happens in manual strangulation and in charred corpses. In forensic practice, the discovery of corpses burned after death to hide their real cause of death is not uncommon: in these cases, the diagnostic challenge is even greater, as the action of flames is capable of both masking previously generated lesions and/or generating new ones, as occurs for hyoid bone fractures. The case concerns a 76-year-old man found charred in his bedroom. Almost complete bo…

A complex fracture of the hyoid bone and the larynx after a bicycle accident - Case report and review of literature.

INTRODUCTION AND IMPORTANCE: Reports about laryngeal trauma and fractures of the hyoid bone are rare in the literature. Most cases are forensic cases and the results of postmortem analysis. Traumatic larynx and hyoid bone fractures represent a rare but important differential diagnosis of the common symptom hoarseness. CASE PRESENTATION: A 60-year-old female patient presented with unclear dysphonia and globus sensation following intubation for a surgical treatment for a lower leg fracture after a bicycle accident two months ago. Endoscopy and the computed tomography (CT) of the neck revealed a…

The role of the styloid apophysis of the temporal bone in the biomechanics of the tongue, mandible, hyoid system: a case study.

Temporomandibular disorders are characterized by ear pain, tinnitus, difficulty in chewing, dysphagia and dizziness, cervical limitation of range of movement (ROM) and other less common disorders. Numerous recent studies associate the presence of these signs and symptoms with an elongated styloid apophysis, as in the case of Eagle syndrome. The purpose of this clinical case is to support the thesis of a common syndrome that temporomandibular joint (TMJ) disorders with an elongated styloid process affirming the relationship between temporal bone, mandible, hyoid bone and tongue. The patient was…
