This paper discusses the role of endogenous opioids in response to hyperthermia and the alterations observed in drug or alcohol addicts. Endorphins, rather than enkephalins are involved in adaptation to heat in the central nervous system. The pituitary secretion of beta endorphin together with ACTH in response to thermal stress provided the opportunity to measure the opioid reactivity to hyperthermia in health and toxicomania, as an index of opioid function, in adaptation to heat. The review of the data in the literature shows absent responses of beta endorphin and its related hormone ACTH to …

Hyperthermia is an uncontrolled elevation of body temperature exceeding the body's ability to dissipate heat. Hyperthermia can result in dangerously high core temperatures and can rapidly become fatal. Common causes include heat stroke, malignant hyperthermia, serotonin syndrome, neuroleptic syndrome, a few endocrine emergencies as well as numerous intoxications. Rapid diagnosis and prompt cooling are pivotal, since the condition triggers a cascade of metabolic events which may progress to irreversible injury or death. Ice-water immersion and evaporative cooling are the methods of choice.

The purpose of this article is to provide an overview on the current clinical application of hyperthermia combined with conventional treatment modalities (e.g. ionizing radiation, chemotherapy) in the treatment of malignant disease. The clinical application of hyperthermia with increase of tissue temperatures (range 40-44 degrees C) has been integrated in multimodal anti-cancer strategies. This review describes selected phase I or II (n = 17) and phase III trials (n = 16) investigating the effect of hyperthermia combined with radiotherapy (n = 10 trials), chemotherapy (n = 15 trials), or both …

Occurring chiefly in epidemics during hot, humid summer weather, heatstroke dramatically demonstrates the consequences of uncontrolled body temperature elevation. It also illuminates the mechanism and management of unusual hyperthermic disorders associated, for example, with neuroleptic and anesthetic drugs.

We describe the pathogenesis and the typical clinical and laboratory features of the hyperthermia syndromes that may develop during neuroleptic drug treatment, environmental exposure or following the use of inhalational anesthetics. Since the features are similar and confusion is possible we emphasize the need for prompt diagnosis so that appropriate treatment, can be started without delay. Hyperthermic reactions have common pathophysiological mechanisms and they may be seen as an abnormal individual response to organic and functional stressors.

Malignant hyperthermia is a rare, subclinical pharmacogenetic syndrome leading to potentially life-threatening skeletal muscle hypermetabolism. Providing a safe and trigger-free anesthesia in predisposed individuals is essential to avoid serious harm to the patient. Especially the management of malignant hyperthermia predisposition in the context of pregnancy poses a huge challenge to the attending anesthesiologist. In May 2019 the European Malignant Hyperthermia Group published a guideline on malignant hyperthermia during pregnancy. The article summarizes and discusses the recommendations and…

Drug-related causes of hyperthermia can often be overlooked in the setting of elevated body temperature. This article reviews the pathophysiology, presentation, and treatment of several drug-induced hyperthermia syndromes: malignant hyperthermia, neuroleptic malignant syndrome, sympathomimetic poisoning, and anticholinergic toxicity. Although the general approach is similar, specific management strategies may be required for each syndrome.

Drugs with antidopaminergic properties and those capable of stimulating serotonin release can be responsible for hyperthermia syndromes such as neuroleptic malignant syndrome and serotonin syndrome. Dopamine and serotonin are important neurotransmitters in temperature regulation and it is likely that these reactions result from drug-induced changes in neurotransmitter levels. We describe three cases of drug-induced hyperthermia, discuss their aetiology and management, with both general measures and therapies designed to redress neurotransmitter imbalance.

Clearly the malignant hyperthermia crisis offers a significant challenge in the well cosmetic surgical patient. Aggressive early treatment is mandatory in these cases. Any delay can be disastrous. At times, the crisis may be aborted due to such therapy, leaving some doubt as to the current diagnosis. With such a disease entity, this is the hopeful scenario.
