Impact of insurance status on healthcare resource utilization and outcomes in adolescent patients presenting with spinal cord injuries.

OBJECTIVE: Insurance disparities have been suggested to influence the medical and surgical outcomes of adult patients with spinal cord injury (SCI), with a paucity of studies demonstrating their impact on the outcomes of pediatric and adolescent SCI patients. The aim of this study was to assess the impact of insurance status on healthcare utilization and outcomes in adolescent patients presenting with SCI. METHODS: An administrative database study was performed using the 2017 admission year from 753 facilities using the National Trauma Data Bank. Adolescent patients (11-17 years old) with cer

Factors associated with age at operation for children with congenital heart disease.

OBJECTIVE: Previous studies have shown that children with congenital heart disease (CHD) who live in nonurban areas or who do not have private insurance are at risk for delayed referral to a pediatric cardiologist. However, the effect of these factors on the age at which cardiac surgery is performed has not been evaluated. This study is designed to evaluate the factors that influence the age at which definitive surgical repair is performed. METHODS: Data on hospital discharges for 1995 and 1996 in California were obtained from the Office of Statewide Health Planning and Development database.

Validity of breast cancer surgery treatment information in a state-based cancer registry.

PURPOSE: Surgery is an important part of early stage breast cancer treatment that affects overall survival. Many studies of surgical treatment of breast cancer rely on data sources that condition on continuous insurance coverage or treatment at specified facilities and thus under-sample populations especially affected by cancer care inequities including the uninsured and rural populations. Statewide cancer registries contain data on first course of cancer treatment for all patients diagnosed with cancer but the accuracy of these data are uncertain. METHODS: Patients diagnosed with stage I-III

Does managed care make a difference? Physicians' length of stay decisions under managed and non-managed care.

BACKGROUND: In this study we examined the influence of type of insurance and the influence of managed care in particular, on the length of stay decisions physicians make and on variation in medical practice. METHODS: We studied lengths of stay for comparable patients who are insured under managed or non-managed care plans. Seven Diagnosis Related Groups were chosen, two medical (COPD and CHF), one surgical (hip replacement) and four obstetrical (hysterectomy with and without complications and Cesarean section with and without complications). The 1999, 2000 and 2001--data from hospitals in New

Outcomes of cervical spinal stenosis surgery in patients aged ≥ 65 years based on insurance status: a single-center cohort study from a tertiary center in Germany.

OBJECTIVE: The prevalence of degenerative disorders of the spine, such as cervical spinal stenosis with cervical spine myelopathy (CSM) in the geriatric population, has rapidly increased worldwide. To date, there has been no systematic analysis comparing outcomes in older patients suffering from progressive CSM and undergoing surgery depending on their health insurance status. We sought to compare the clinical outcomes and complications after anterior cervical discectomy and fusion (ACDF) or posterior decompression with fusion in patients aged ≥ 65 years with multilevel cervical spinal canal s

Socioeconomic Risk Factors for Poor Outcomes of Developmental Dysplasia of the Hip.

OBJECTIVE: To examine the relationship between socioeconomic factors and outcomes of developmental dysplasia of the hip (DDH). STUDY DESIGN: A retrospective review of patients with DDH at a tertiary pediatric hospital from 2003 to 2012 with 2 years minimum follow-up was conducted. The relationship between socioeconomic factors with late presentation, treatment, and outcomes was examined. Socioeconomic factors included insurance status, language, and ethnicity. RESULTS: In total, 188 patients met criteria. Patients with late presentations were more likely to be Hispanic (P = .02). However, pu

Disparities in Treatment Decisions for Abnormal Uterine Bleeding.

Background: Abnormal uterine bleeding (AUB) affects ∼1.4 million women in the United States each year. This study sought to identify differences in treatment decisions for managing noncancer-related AUB. Materials and Methods: IRB approval was obtained. This retrospective study analyzed data for all women 21-60 years old, diagnosed with AUB from July 1, 2013 through June 30, 2017, in an Obstetrics and Gynecology residents' clinic serving mostly underinsured patients. Patients with a diagnosis of postmenopausal bleeding or any gynecologic cancer were excluded. Two multivariable logistic regress

Procedural trends in the treatment of peripheral arterial disease by insurer status in New York State.

BACKGROUND: Type or lack of insurance may affect access to care, treatment, and outcomes. We evaluated trends for surgical management of all peripheral arterial disease (PAD) in-hospital admissions by insurer status in New York State. STUDY DESIGN: Statewide Planning and Research Cooperative System (SPARCS) data were obtained and cross-referenced for diagnostic and procedure codes. Data from 2001 to 2002 were averaged and used as a baseline. Change in indication, volume of admissions, procedures, and amputations were calculated for the years 2003 to 2008 and were analyzed by insurer status.

Prevalence and incidence of hypoparathyroidism in the United States using a large claims database.

Hypoparathyroidism is a rare endocrine disorder whose incidence and prevalence have not been well defined. This study aimed to 1) estimate the number of insured adult patients with hypoparathyroidism in the United States and 2) obtain physician assessment of disease severity and chronicity. Prevalence was estimated through calculation of diagnoses of hypoparathyroidism in a large proprietary health plan claims database over a 12-month period from October 2007 through September 2008 and projected to the US insured population. Incidence was also calculated from the same database by determining t

High Deductibles are Associated With Severe Disease, Catastrophic Out-of-Pocket Payments for Emergency Surgical Conditions.

BACKGROUND: Out-of-pocket spending has risen for individuals with private health insurance, yet little is known about the unintended consequences that high levels of cost-sharing may have on delayed clinical presentation and financial outcomes for common emergency surgical conditions. METHODS: In this retrospective analysis of claims data from a large commercial insurer (2016-2019), we identified adult inpatient admissions following emergency department presentation for common emergency surgical conditions (eg, appendicitis, cholecystitis, diverticulitis, and intestinal obstruction). Emerging advances in non-surgical ablation for early-stage breast cancer.

Recent advances in diagnostic imaging have led to an increase in the detection of early-stage breast cancer, prompting growing interest in minimally invasive breast surgery. These approaches include the omission of axillary lymph node dissection, use of endoscopic or robot-assisted surgery, and application of non-surgical ablation techniques. Non-surgical ablation therapy aims to eradicate tumor tissue in situ by delivering localized thermal or cryogenic energy under imaging guidance without surgical resection. Among these, radiofrequency ablation (RFA) and cryoablation have been the most exte

Factors associated with subsequent surgery after septic arthritis of the knee in children.

BACKGROUND: Septic arthritis of the knee in children is a challenging problem. Surgical debridement is an established treatment, but there is a paucity of literature on long-term prognosis. AIM: To determine the rates and factors associated with return to surgery (RTS) and readmission after index surgical debridement for septic arthritis of the knee in children. METHODS: This is a retrospective cohort study that utilizes data from the Healthcare Cost and Utilization Project (HCUP). We included patients between ages 0 to 18 years that underwent surgical debridement for septic arthritis of the

Social determinants of health and outcomes in proximal humerus fractures based on surgery type.

BACKGROUND: Proximal humerus fractures (PHFs) account for 5% to 6% of all adult fractures. The optimal surgical management for PHFs remains under debate, with 2 of the most common operations being open reduction and internal fixation (ORIF) and reverse total shoulder arthroplasty (rTSA). Social determinants of health (SDOHs) have gained particular attention in many medical fields because of their relationship to health outcomes, with the Social Vulnerability Index (SVI) as an example of an adopted measure of geographic disadvantage. The purpose of this study was to investigate the associations

Navigating payment and policy barriers to gender-affirming care for transmasculine individuals: A qualitative study and policy assessment.

Over the past decade, access to and insurance coverage for gender-affirming medical and surgical treatment for transgender (trans) individuals in the U.S. has improved. Despite this, trans individuals continue to experience insurance-related barriers to gender-affirming care (GAC)-and in particular, transmasculine individuals may face gender-specific barriers resulting from regulations on masculinizing hormones. In this study, we interviewed transmasculine individuals from across the U.S. about their gender-affirming care experiences with a focus on insurance, payment and policy-related barrie

Demographic Disparities amongst Patients Receiving Carpal Tunnel Release: A Retrospective Review of 92,921 Patients.

UNLABELLED: Despite strong evidence supporting the efficacy of surgical release for carpal tunnel syndrome (CTS), previous studies have suggested that surgery is not performed equally amongst races and sex. The purpose of this study was to investigate potential socioeconomic disparities in the surgical treatment for CTS. METHODS: Adult patients (≥18) were identified in the New York Statewide Planning and Research Cooperative System database from 2011 to 2018 by diagnosis code for CTS. All carpal tunnel surgery procedures in the outpatient setting were identified using Current Procedural Termi

[Robotic-Assisted Surgery for Renal Cancer and Cancer of the Renal Pelvis and Ureter].

Recently, minimally invasive urological surgery has become increasingly common as years pass, with the rapid development of laparoscopic, minimal wound, mono-sectional, and robot-assisted surgeries. Robot-assisted surgery's indication for renal, renal pelvis, and ureter cancers has been expanded, and in Japan, robot-assisted partial nephrectomy(RAPN)has been covered by insurances since 2016. Additionally, robot-assisted radical nephrectomy(RARN)and robot-assisted nephroureterectomy(RANU)were covered by insurances in 2022. Compared to that of conventional laparoscopic surgery, robot-assisted su
