Hypertension in the intensive care unit.

PURPOSE OF REVIEW: The severity of hypertensive crises is determined by the presence of target organ damage rather than the level of blood pressure. Hypertensive urgencies with no signs of organ dysfunction can therefore be distinguished from hypertensive emergencies in which the presence of severe end-organ damage requires prompt therapy. Hypertensive emergencies include acute aortic dissection, hypertensive encephalopathy, acute myocardial ischaemia, severe pulmonary oedema, eclampsia, and acute renal failure. RECENT DEVELOPMENTS: Malignant hypertension is a severe form of hypertensive emer

Infection in the intensive care unit.

An epidemic of infection associated with Serratia marcescens and other Gram-negative organisms resistant to aminoglycosides and other chemotherapeutic agents occurred in the Intensive Care Unit, and spread to other areas of the hospital. This paper describes the problems of sepsis in the critically ill patient, outlines the occurrence of organisms in the patients concerned in this epidemic, and discusses the policies adopted to control the incidence of life-threatening infection caused by bacteria resistant to all other agents.

Use of Point-of-Care Ultrasound by Intensive Care Unit Triage Teams in Evaluating Unstable Patients Outside Intensive Care Units.

Introduction Point-of-care ultrasound (POCUS) has become an integral asset in intensive care units (ICUs). However, there is limited literature on the value of POCUS in evaluating deteriorating patients outside the ICU. In this study, we sought to investigate the use and impact of POCUS by ICU triage teams in hospitals outside of the ICU setting. Methods ICU triage fellows were provided a portable ultrasound to use as part of their evaluations during consultations and hospital code activations. Fellows were asked to fill out a survey on how ultrasound was used and its impact on patient managem…

Neuromonitoring in neonatal intensive care units-an important need towards individualized neuroprotective care.

Neuromonitoring has been widely accepted as an important part in neonatal care. Amplitude-integrated EEG (aEEG) and near-infrared spectroscopy (NIRS) are often mentioned in this context, though being only a part of the fully array of methods and examinations that could be considered neuromonitoring. Within the broad array of medical conditions that could be encountered in a neonatal patient, it is important to be aware of the indications for neuromonitoring and especially which neuromonitoring technique to use best for the individual condition. aEEG is now a widely accepted neuromonitor in neo

[Quality of care in intensive care units. Retrospective study on long-term patients].

Nursing care must be evaluated in order to determine its effectiveness and identify deficiencies, which makes it possible to introduce corrective measures to optimize quality. The quality of care given to patients in our unit was evaluated with a retrospective analysis of 915 records in the Nursing Care Plan corresponding to 59 patients with a mean ICU stay of 15.5 days. Six areas of care and indicators of deficient practice were identified: orotracheal intubation: accidental disconnection, displacement, obstruction, and pressure sores on lips; arterial catheterization: accidental disconnectio

[Post-Resuscitation care in the intensive care unit].

Despite major advances, morbidity and mortality after cardiac arrest remain high. Postresuscitation care requires a multimodal, time-coordinated approach. The International Liaison Committee on Resuscitation (ILCOR) model describes four phases of brain injury, explaining the failure of single-target therapies. Cardiac arrest centers consolidate expertise and improve survival and neurological outcomes in observational studies, although randomized trials remain inconclusive. Hypoxia must be avoided, while hyperoxia should not be pursued; a MAP of 60-65 mm Hg is adequate. Immediate PCI is mandato

Urinary catheter care in the intensive care unit.

Urinary catheters are associated with a number of complications, and nurses are ideally suited to minimize the associated risks by utilizing the available research in their practice. Urine tract infections caused by urine catheters are associated with increased mortality; however, urine catheter care is a nursing procedure, the importance of which is sometimes overlooked. This study reviews recommended guidelines on urine catheter care and current published literature on the subject. The aim of the study was to identify recommended practice and compare it with the current research and literatu

Length of hospitalization in intensive care units: A cross-sectional study.

Prolonged hospitalizations in intensive care units (ICUs) due to critical conditions are a significant challenge to the healthcare system. The primary objective within the ICU is to reduce the length of stay (LOS), which ultimately could enhance the quality of healthcare, minimize complications, and reduce healthcare-associated costs. This study aims to identify factors affecting the LOS in ICUs. This is a retrospective cross-sectional study that utilized electronic records from a tertiary hospital, analyzing data from 7628 ICU admissions between 2012 and 2022. A total of 7628 ICU patients wer…

Respiratory Physiotherapy Techniques in Neonatal Intensive Care Units: A Scoping Review.

BACKGROUND AND PURPOSE: Certain physiological characteristics increase the susceptibility of newborns, particularly preterm infants, to pulmonary collapse. In addition, newborns are prone to the accumulation of airway secretions, which, even in small volumes, can compromise respiratory function. Consequently, breathing therapy plays a crucial role in promoting airway clearance and supporting effective ventilation in this population. This scoping review aimed to map the breathing therapy methods used for newborns admitted to intensive care units and also to identify the clinical outcomes assess…

Lung recruitment manoeuvre strategies in paediatric intensive care units across Europe.

INTRODUCTION: In severe paediatric acute respiratory distress syndrome (PARDS) lung recruitment manoeuvres (LRMs) may be applied to improve oxygenation, but their application is still controversial. The aim of this survey is to report what the current practice is across European paediatric intensive care units (PICUs). METHODS: An online survey was distributed to PICUs in 19 European countries targeting paediatric intensivists, nurses and respiratory therapists. One reply per unit was allowed. RESULTS: 151 PICUs out of 276 (54.8%) responded. Of those, 75.9% have more than 300 admissions per …

Fostering safer intensive care units: How incident reporting drives cultural change.

BACKGROUND: Patients admitted to intensive care units (ICUs) are more prone to patient safety incidents (PSIs), with substantial negative effects. Patient safety culture (PSC) is seen as a crucial pillar to address these safety problems. The aim of our study was to evaluate the impact of the implementation of a PSIs reporting system on PSC in ICUs. METHODS: We conducted a quasi-experimental study in 15 ICUs in two Tunisian university hospitals. One hospital was chosen as the intervention site (10 ICUs), where a PSI reporting system was implemented, while the other served as the control (five …

Analysis of Candidozyma auris - an emerging threat in intensive care units.

INTRODUCTION: Candidozyma (C.) auris (formerly Candida auris) is an emerging fungus posing a serious global health threat due to its multidrug resistance and difficulty in identification with standard laboratory methods. It causes severe illness in hospitalized patients, with risk factors including recent surgery, diabetes, and the use of broad-spectrum antibiotics and antifungals. Colonization of the respiratory tract, catheters, and skin with C. auris can lead to serious infections, particularly through central venous catheters, urinary catheters, and tracheostomy tubes, resulting in fungemi

Delirium management and current practice among Intensive Care Units Doctors, Khartoum.

Delirium is a brain dysfunction characterized by attention and cognitive disturbances in a fluctuating manner. The international guidelines recommend daily screening for delirium. The Confusion Assessment Method for the Intensive Care Unit (CAM-ICU) and Intensive Care Delirium Screening Checklist (ICDSC) are the most commonly used methods for assessing delirium. This study aimed to identify barriers and gaps in knowledge and practice. This was a hospital-based Cross-Sectional study. Stratified random sampling was used in this study. 72 ICU doctors were randomly selected. Statistical analyses w
