Intercostal Nerve Transfers to the Musculocutaneous-A Reliable Nerve Transfer for Restoration of Elbow Flexion in Birth-Related Brachial Plexus Injuries.

Introduction  There is consensus on the need for early microsurgical reconstruction in birth palsies involving three or more roots, that is, extensive partial palsies and total palsies. The fundamental principles of these operations are complete exploration and judicious use of the ruptured stumps by nerve grafting to suitable distal targets. The frequent observation of root avulsions in such cases makes it imperative to look for extraplexual nerve donors for some functions. Intercostal nerves are readily available in such patients. Materials and Methods  This is a study of 50 patients of exte…

Restoration of elbow flexion in brachial plexus avulsion injury: comparing spinal accessory nerve transfer with intercostal nerve transfer.

This study was performed to compare the clinical outcome of 2 types of commonly used nerve transfers, the spinal accessory nerve transfer and the intercostal nerve transfer. This study was a prospective randomized parallel trial involving 205 patients presenting between 1989 and 1994. All patients were males ranging in age from 16 to 43 years. All patients underwent surgery within 6 months of injury. Spinal accessory nerve transfer was performed in 130 patients; better results were obtained in terms of less operative time, fewer blood transfusions, fewer immediate complications, and better mot

Intercostal nerve cryoablation for control of pain after sternal fracture repair: A case series.

INTRODUCTION: Sternal fractures resulting from blunt chest trauma, can present with significant pain and respiratory compromise, particularly in patients with polytrauma. While isolated, nondisplaced fractures are typically managed conservatively, displaced or painful fractures are higher risk and may warrant surgical intervention. Intercostal cryoneurolysis (IC) has emerged as a promising adjunct, offering enhanced pain control in thoracic procedures. However, no studies have characterized the role of IC in the management of traumatic sternal fractures. CASE REPORT: We present two cases of p

Rehabilitation strategies for abdominal wall reinnervation after intercostal nerve injury: A narrative review.

The abdominal wall musculature, including the rectus abdominis, transversus abdominis, and internal and external obliques, is a complex, multifunctional unit essential for trunk stability, postural control, and respiratory function. Injury to the intercostal nerves innervating these muscles, which may occur following rib fractures, thoracic surgery, trauma, or iatrogenic interventions, can lead to significant motor and respiratory deficits, including abdominal wall bulging, reduced trunk stability, impaired cough, and diminished forced expiration. Surgical interventions such as intercostal ner…

Pain management after pneumothorax surgery: intercostal nerve block or thoracic epidural analgesia.

OBJECTIVES: In patients undergoing video-assisted thoracoscopic surgery for pneumothorax, the benefits and risks of single-shot intercostal nerve block as loco-regional analgesia are not well known. We retrospectively compared the effectiveness of intercostal nerve blocks as a viable alternative to thoracic epidural analgesia (TEA) regarding pain control and enhanced recovery. METHODS: A retrospective multicentre analysis with single-centre propensity score matching was performed in patients undergoing video-assisted thoracoscopic surgery for pneumothorax receiving either TEA or intercostal n

Cost and outcomes of intercostal nerve cryoablation versus thoracic epidural following the Nuss procedure.

BACKGROUND: Pectus excavatum is the most common congenital chest wall abnormality, with the Nuss procedure being the most commonly performed repair. Pain control is the predominant factor in the postoperative treatment of these patients. This study aims to compare the cost and outcomes of intercostal nerve cryoablation (INC) and thoracic epidural (TE) in patients undergoing the Nuss procedure. METHODS: A retrospective chart review was conducted at our institution for all patients who underwent the Nuss procedure for pectus excavatum from 2002 to 2020. Patients were stratified by pain manageme

Intercostal Nerve Block Using an Innovative Intraneedle Ultrasound Transducer: A Proof-of-Concept study.

Intercostal nerve block is a widely used and effective approach to providing regional anesthesia in the thoracic region for pain relief. However, during ultrasound-guided intercostal nerve block, inaccurate identification of the anatomic structures or suboptimal positioning of the needle tip may result in complications and blockade failure. In this study, we designed an intraneedle ultrasound (INUS) system and validated its efficacy in identifying anatomic structures relevant to thoracic region anesthesia. The 20-MHz INUS transducer comprised a single lead magnesium niobate-lead titanate cryst

Intercostal Neuralgia Occurring as a Complication of Splanchnic Nerve Radiofrequency Ablation in a Patient with Chronic Pancreatitis.

UNLABELLED: Our intent is to report a case of intercostal neuralgia occuring as a complication of splanchnic radiofreqency ablation (RFA), due to a breach in the integrity of the insulating sheath of the RFA needle.A 48-year-old man presented to our pain clinic with upper abdominal pain due to chronic pancreatitis, recalcitrant to medical management. We decided to perform bilateral splanchnic nerve RFA in this patient. After confirmation of bilateral correct needle placement under fluoroscopic guidance and sensorimotor testing, RFA was performed on the right side uneventfully. However, during

Assessment and follow-up of intercostal nerve damage after video-assisted thoracic surgery.

OBJECTIVE: Chronic pain is a common complication after thoracic surgery. The most important factor appears to be intercostal nerve damage. The purpose of this prospective study was to objectively evaluate intercostal nerve damage associated with post-thoracotomy pain after three surgical procedures using current perception threshold testing. METHODS: The 32 patients were classified into three groups: the video-assisted thoracic surgery group (n=7), the video-assisted minithoracotomy with metal retractors group (n=15), and the conventional thoracotomy group (n=10). Intercostal nerve function w

Sensory recovery of the hand with intercostal nerve transfer following complete avulsion of the brachial plexus.

BACKGROUND: Restoration of sensory function is imperative when prehensile function is restored after irreparable brachial plexus injury. However, there are few reports that focus on the results of sensory recovery after brachial plexus reconstruction. The purpose of this article is to report the results of sensory recovery of the hand with intercostal nerve transfer following complete brachial plexus injury. METHODS: Seventeen patients with complete brachial plexus injury underwent sensory reconstruction of the hand with intercostal nerve transfer to the median or ulnar nerve. All patients un

Outcomes of surgical treatment for chronic postoperative breast and abdominal pain attributed to the intercostal nerve.

BACKGROUND: Postoperative breast and abdominal pain arising from lesion of the intercostal nerves is an often overlooked and misdiagnosed clinical entity. Typically, affected patients experience a high degree of frustration in addition to persistent pain. Surgical options for management of this pain are not widely reported. STUDY DESIGN: Medical records were reviewed of all consecutive patients of the senior author who had undergone surgical treatment for postoperative breast and abdominal pain since May of 2003. All patients underwent surgical excision of the involved nerve or nerves proxima…

Electromyographic comparison of various exercises to improve elbow flexion following intercostal nerve transfer.

We compared the quantitative electromyographic activity of the elbow flexors during four exercises (forced inspiration, forced expiration, trunk flexion and attempted elbow flexion), following intercostal nerve transfer to the musculocutaneous nerve in 32 patients who had sustained root avulsion brachial plexus injuries. Quantitative electromyographic evaluation of the mean and maximum amplitude was repeated three times for each exercise. We found that mean and maximum elbow flexor activity was highest during trunk flexion, followed by attempted elbow flexion, forced inspiration and finally fo

Respiration-related activity in the biceps brachii muscle after intercostal-musculocutaneous nerve transfer.

Needle EMG and polygraphic recordings of respiration and biceps activity were obtained in 7 patients who had undergone intercostal to musculocutaneous nerve transfer because of severe traumatic injury to the brachial plexus. EMG activity during expiration and inspiration was assessed during quiet breathing, deep breathing, and during sustained inspiration and expiration, and compared to the clinical strength of the biceps muscle. Biceps activity was consistently found during both expiration and inspiration, and in either case separately related to the flexion force levels. Initially this relat

Posterior intercostal nerve block for pain relief after cholecystectomy. Anatomical basis and efficacy.

The human intercostal space has been studied by excision of the posterior part of the rib cage at autopsy, followed by fixation, decalcification, section and staining. Injection of India ink was used to simulate local anaesthetic. At a point 7 cm from the midline, the distance from the posterior aspect of the rib to the pleura averaged 8 mm. The intercostal nerve usually comprises a number of small bundles without any enclosing fascial sheath. The bundles lie in a triangular space bounded by the rib, the posterior intercostal membrane and the intercostalis intima muscle. The first two appear i

Prospective randomized evaluation of intrathoracic intercostal nerve block with bupivacaine on postoperative ventilatory function.

This prospective and randomized study demonstrates the beneficial effect of intrathoracic intercostal block with bupivacaine on the recovery of pulmonary function after thoracotomy. A significantly (p less than 0.05) better forced expiratory volume was seen in the treated group in the early post-operative period. Similar findings were observed in forced vital capacity. Also, the requirements for analgesics were decreased in the treated group. We believe that intrathoracic intercostal block with bupivacaine when properly utilized can be a good adjuvant in the improvement of pulmonary function a
