Menstrual cup use and intrauterine device expulsion in a copper intrauterine device randomized trial.

OBJECTIVE: To evaluate menstrual cup use and intrauterine device (IUD) expulsion. STUDY DESIGN: We performed a secondary analysis of a 3-year contraceptive efficacy trial comparing two copper 380 mm2 IUDs. Investigators randomized participants approximately 1:4 to the TCu380A or NTCu380-Mini IUD. Approximately 12 months after enrollment began, we advised participants against menstrual cup use due to observed IUD expulsions in cup users. We evaluated IUD expulsion (including spontaneous partial and complete expulsion and accidental self-removal) at 12 and 36 months. We used multivariable logis

Carrying a Pregnancy to Term With an Intrauterine Device in Place: A Case of an Intraabdominal Intrauterine Device Migration.

Long-acting intrauterine contraceptives such as intrauterine devices (IUD) are popular due to their high rates of long-term efficacy, ease of use, and reversibility. Though rare, these devices can incur complications such as uterine perforation. Signs and symptoms are often vague abdominal and pelvic pain, and patients rarely present with a surgical emergency. This uterine perforation can happen immediately upon IUD placement or in a delayed manner. This case details an example of an IUD uterine perforation with abdominal migration two years after placement. The patient's history is complicate…

The evaluation of 318 intrauterine pregnancy cases with an intrauterine device.

OBJECTIVE: To determine the factors affecting the pregnancies with intrauterine devices. DESIGN AND METHODS: The intrauterine device (IUD) locations in pregnancies with IUDs referring to SSK Aegean Maternity Hospital were determined by ultrasonography (USG) in a 6 year period. Three hundred and eighteen pregnancy cases (pregnancy group) were observed together with intrauterine devices. All intervally applied IUDs were CeT380A. All demographic characteristics of the cases together with determination of IUD localizations were recorded, and these cases were compared with 300 cases (control group…

Risk of uterine perforation with levonorgestrel-releasing and copper intrauterine devices in the European Active Surveillance Study on Intrauterine Devices.

OBJECTIVES: The objectives were to identify and compare the incidence of uterine perforation and other medically adverse events associated with levonorgestrel-releasing intrauterine systems (LNG-IUSs, releasing 20 mcg LNG daily) and copper intrauterine devices (IUDs) under routine conditions of use in a study population representative of typical users. METHODS AND MATERIALS: This is a multinational, prospective, non-interventional cohort study with new users of LNG-IUSs and copper IUDs. In addition to a baseline questionnaire, women and their treating health care professional completed a sing…

Misoprostol for intrauterine device placement.

RATIONALE: Identifying effective approaches to reduce pain and improve providers' ease of intrauterine device (IUD) placement may reduce barriers to IUD access and use. The cervical softening and dilation effects of misoprostol might make IUD placement less painful for women and technically easier for providers. However, evidence suggests that misoprostol does not improve many outcomes and may only be helpful for some patients (e.g. those with a recent failed IUD placement). OBJECTIVES: To examine the effect of misoprostol for routine IUD placement on patient and provider outcomes compared to

Rectum migration of an intrauterine device.

Intrauterine device (IUD) is a well-accepted means of contraception. Although it is safe and effective, some serious complications may occur. It should be paid attention to a 45-year-old female admitted to the hospital for aggravated abdominal pain and dyspareunia for 2 months. She was found to have two IUDs in her body, one in the uterine cavity and the other outside. They were removed through laparoscopic and hysteroscopy. When IUD perforation occurs, whether symptomatic or not, surgical removal is necessary. Laparoscopy is thought to be the first choice. However, when serious adhesions coex

Ultrasound Assessment of the Intrauterine Device.

As the use of intrauterine devices (IUDs) has expanded in the United States, both as a highly effective form of long-acting reversible contraception (LARC) and as a tool for effective management of other gynecologic conditions, effective imaging of IUDs has become increasingly important. Ultrasound is a cost-effective, accurate, safe, and accessible imaging solution for the assessment of IUDs. This article explores the role of ultrasound in evaluation of IUDs as well as the literature regarding ultrasound guidance for device placement. It also examines the evaluation and management of IUDs wit…

The intrauterine device and its dynamics.

In the past decade, attention has shifted from family planning (often made available through population programs) to reproductive health--a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity, in all matters related to the reproductive system and its function and processes. Reproductive health has three components: the ability to procreate, regulate fertility and enjoy sex; the successful outcome of pregnancy through infant and child survival and growth; and the safety of the reproductive process. According to Mitchell et al., the followi

The intrauterine device: dispelling the myths.

The worldwide popularity and usage of intrauterine devices (IUDs) plummeted in the 1970s, when grim reports of septic abortions and pelvic inflammatory disease were published. Although the Dalkon Shield ultimately was determined to be the culprit for these problems, the reputation of all IUDs was damaged, and their popularity spiraled downward. The stigma continues, despite the proven safety and efficacy of newer IUDs, particularly the ParaGard T 380A and the Progestasert, which are now the only two IUDs approved for use in the United States. This article will review how the IUD works and will…

The intrauterine device in the present and future.

Good counseling and better selection of candidates would result in the intrauterine device's (IUD) safer use. Age and parity of the IUD acceptor do not seem to be associated with an increased risk of pelvic inflammatory disease (PID). The newer generation of copper- and hormone-releasing IUDs are associated with improved safety and efficacy.

Intravesical migration of intrauterine device.

Intrauterine devices have been plagued by many early and late complications, including uterine perforation and migration into adjacent structures. To our knowledge only 18 cases have been reported in the literature of migration of an intrauterine device into the bladder. We report on a 38-year-old woman in whom an intrauterine device eroded from the uterus 3 years after placement. The device remained asymptomatic in the pelvis for an additional 13 years before the patient presented with urinary symptoms. The literature is reviewed.

The intrauterine device and ectopic pregnancy.

Based on data given by case-control and cohort studies the relationships between current and past IUD use, duration of IUD use and the type of IUD were evaluated to determine the risk of ectopic pregnancy among IUD users. The results of this review indicate that current and past IUD users do not have an increased risk of ectopic pregnancy. No relationship was found between the duration of IUD use, for either current or past IUD users, and the risk of ectopic pregnancy. Pooled data from clinical studies of different types of IUD showed that the lowest risk of ectopic pregnancy was for users of

Failure to insert an intrauterine device.

Numerous studies have reported IUD-related events in evaluating the rates of pregnancy, expulsion, and removal for bleeding and pain, but the inability to successfully insert an IUD has been overlooked. This study analyzed data on 51 unsuccessful insertions from 22 international centers collected on standard forms. The incidence of insertion failures was estimated to be between 2.3 and 8.3/1000 attempted insertions. No statistically significant risk factors in patient characteristics were delineated through a case-control analysis. The incidence of insertion failures among postpartum cases was
