Ten patients referred late with oesophagopleural fistulae were managed by T tube intubation of the oesophageal defect. Seven survived and six had radiographic evidence of closure of the defect. One patient who removed his T tube early was discharged to convalesce with a small oesophagocutaneous fistula. Three patients died as a consequence of pre-existing renal failure, upper gastrointestinal haemorrhage and adult respiratory distress syndrome. Patients presenting late with multisystem complications and ongoing sepsis secondary to oesophagopleural fistulae pose a major management problem. Our

We present a case of a patient with recurrent squamous cell cancer of the base of the tongue and right tonsil who developed severe haematemesis. An elongated blood cast of the upper gastrointestinal tract was noted in the emesis. The patient required emergent intubation and blood transfusions. A CT scan with contrast revealed the presence of a pseudoaneurysm of the right lingual artery. Successful control of bleeding was achieved with coil embolisation of the pseudoaneurysm and its feeding artery. This case highlights that oropharyngeal bleeding can mimic gastrointestinal bleeding.

OBJECTIVE: The nasogastric tube is the chosen nutritional technique in premature infants. However, it is not without complications. The aim of this study is to compare our experience in iatrogenic complications caused by nasogastric tube (especially in very low birth weight infants) to a review of the most recent literature.
METHODS: From january to december of 2008, in the Department of Neonatal Pathology at the Hospital of Treviso, 118 premature patients were treated. 110 of them had a body weight less than 1,500gr: serious complications caused by nasogastric tube occurred in two of these v

BACKGROUND: Service models for gastrointestinal endoscopy sedation must be safe, as endoscopy is the most common procedure performed under sedation in many countries. The aim of this prospective cohort study was to determine the patient risk profile, and incidence of and risk factors for significant unplanned events, in adult patients presenting for gastrointestinal endoscopy in a group of university-affiliated hospitals where most sedation is managed by anaesthetists.
METHODS: Patients aged ≥18 yr presenting for elective and emergency gastrointestinal endoscopy under anaesthetist-managed sed

Gastroparesis often precludes gastric enteral nutrition (EN) in critically ill patients. Our aim was to determine the feasibility of bedside microendoscopic placement of nasointestinal feeding tubes to facilitate enteral nutrition in critically ill patients with poor gastric emptying. Nine mechanically ventilated patients with proven gastroparesis underwent 10 nasointestinal intubations using a microendoscope. These were compared with 35 patients who underwent pH sensor-guided intubation. Blind pH-guided intubation was faster than microendoscopic placement (21.4 +/- 10.7 v 32 +/- 11.6 min, P =

Nasogastric tubes (NGTs) are commonly used in the intensive care unit (ICU) and are often inserted blindly at the bedside. Previous studies have highlighted various complications associated with NGT misplacement, including epistaxis, pneumothorax and even fatal perforations. To reduce the incidence of complications, guidelines recommend confirming the correct position of the NGT through radiography, pH testing, end-expiratory carbon dioxide monitoring, ultrasonography, etc. Herein, we present the case of a 78-year-old man who experienced sudden dyspnoea, was brought to the ICU and subsequently

A 74-year-old male with acute obstructive suppurative cholangitis due to common bile duct stones (CDBS) underwent an urgent endoscopic biliary decompression. A 7 F endoscopic nasobiliary drainage (ENBD) tube with a pigtail shape was temporarily placed in the common bile duct. The patient had also been diagnosed with an acute cerebral infarction several days before the endoscopic procedure and was under antiplatelet drug therapy.

OBJECTIVE: To describe the use of temporary palatopexy procedure as a treatment for brachycephalic dogs in severe respiratory distress.
MATERIALS AND METHODS: In this prospective pilot study, seven client-owned brachycephalic dogs in severe respiratory distress, requiring emergent intubation or failing extubation, had a temporary palatopexy placed using a novel suture pattern.
RESULTS: Six out of seven dogs were successfully extubated less than 2 hours post palatopexy and survived to discharge. One dog failed extubation secondary to worsening lower airway disease and laryngeal collapse. That

Enterobacterial and pseudomonal carriage in the oropharynx, stomach and gut is uncommon in healthy individuals (apart from E. coli in the gut). Underlying intrinsic disease is the main cause of oropharyngeal and gastrointestinal carriage of 'abnormal' flora. Interventions including intubation, catheterisation and drugs impairing peristalsis may promote this abnormal carriage in the critically ill. The oropharyngeal and gastrointestinal carrier state is the first stage in the development of nosocomial infections. Procedures aimed at the prevention of hospital infections via the abolition of the

This case report describes the airway course of a 52-year-old male who required emergent exploratory laparotomy for suspected gastrointestinal perforation. The patient had underlying subglottic stenosis and unilateral vocal cord paralysis, as well as a history of significant cardiopulmonary disease. Attempts at intubation using standard tube sizes were unsuccessful. A 6.0 endotracheal tube was eventually passed beyond a point of resistance, allowing the operation to proceed. He remained intubated after surgery and was successfully extubated several days later. The report emphasizes the need fo

A new method was developed for nonsurgical peroral positioning of an esophageal prosthesis by using bougie under X-ray control in 1983. I referred to it as "through-bougie esophageal prosthesis intubation". The external diameter of a bougie is equal to the internal diameter of an esophageal prosthesis, so intubation can be performed safely and smoothly. This method was carried out in 44 cases (40 cases of cancerous stenosis, and 4 cases of cicatrical stenosis), and insertion of an esophageal prosthesis was possible in all cases. The improvement rate of peroral feeding was 88.6%. This new metho

Iatrogenic injuries to the upper gastrointestinal tract in neonates are rare but may lead to significant morbidity and mortality if undiagnosed. The clinical presentation of such injuries is usually nonspecific and symptoms may be overlooked, particularly in sick preterm neonates. Therefore, it is important to recognize the findings on plain chest radiographs obtained regularly in neonatal intensive care units (NICUs) on intubated patients. The purpose of this pictorial essay is to describe the imaging findings of various iatrogenic injuries to the pharynx and esophagus in the neonatal period

Enteral feeding is an essential intervention for patients unable to meet their nutritional needs orally, but it carries risks that require skilled nursing recognition and management. This article provides a practical overview of the most common complications encountered across the range of enteral tube types - including refeeding syndrome, nausea and vomiting, aspiration, diarrhoea and constipation, tube blockage, and tract infection - and outlines the nursing management strategies supported by current evidence and guidance. Prompt identification of complications, accurate documentation, early

To address the problems of traditional naso-jejunal tube replacement in post-pyloric feeding, such as high blind insertion failure rate, high radiation exposure and high risk of complications, this study proposed a naso-jejunal tube replacement technique with real-time ultrasound guidance and zebra guide wire, and applied it to the clinical practice of a 67-year-old male patient with severe traumatic brain injury. The patient needed long-term post-pyloric enteral nutrition support due to severe traumatic brain injury, but blind tube insertion was difficult (the first tube insertion was assiste
