[Experience in using normofundin and sterofundin solutions for the correction of hypernatremia].

The efficacy of Sterofundin, Normofundin, and physiological solutions in correcting hypernatremia and impaired acid-base balance was analyzed in patients with sellar region tumors. Twenty-one patients were enrolled in the study; three groups were formed. Physiological solution added by 4% of potassium chloride and 25% magnesium sulfate solution was used in Group 1 (n = 7). Normofundin was employed in Group 2 (n = 7). Sterofundin was applied to Group 3 (n = 7). The volume of administered fluid corresponded to the sum of volumes of basic needs and those of fluid losses. The study revealed that t

Topical therapy for dry eyes.

'Artificial tear' solutions, perhaps more accurately termed 'ocular wetting' solutions, have been the mainstay of topical therapy for keratoconjunctivitis sicca (KCS). These solutions have been designed with water-soluble polymers in an attempt to increase their ocular retention time or ability to wet the ocular surface. Treatment with ocular wetting solutions has been non-specific in that there is no evidence that dry spot formation is essential to the induction of surface disease in KCS patients, nor is there evidence that water-soluble polymers are beneficial to epithelium. In KCS, tear fil

Ethanol dependence as a determinant of fluid preference.

Rats made dependent on ethanol by a schedule-induced polydipsia procedure preferred 5% ethanol to an increasing concentration of dextrose solution to a greater extent than animals on a non-dependent, non-polydipsic procedure which allowed an equivalent opportunity to drink ethanol, confirming a previous study. Two corresponding groups of animals drinking isotonic (0.9%) NaCl rather than 5% ethanol behaved similarly to the latter group, changing to a dextrose preference at a lower dextrose concentration than the ethanol polydipsic group. Therefore, neither the intermittent food regimen (polydip

Colloids versus crystalloids for fluid resuscitation in critically ill patients.

BACKGROUND: Colloid solutions are widely used in fluid resuscitation of critically ill patients. There are several choices of colloid and there is ongoing debate about the relative effectiveness of colloids compared to crystalloid fluids. OBJECTIVES: To assess the effects on mortality of colloids compared to crystalloids for fluid resuscitation in critically ill patients. SEARCH STRATEGY: We searched the Injuries Group specialised register, Cochrane Controlled Trials Register, MEDLINE, EMBASE and BIDS Index to Scientific and Technical Proceedings and checked the reference lists of trials and

I.V. fluid therapy. Part 2. I.V. fluid selection.

I.v. fluid selection depends on the estimated fluid loss, the primary fluid compartment involved, the patient's underlying problem and the physiological and haemodynamic impact of the i.v. solution. Clinically, the most important problem is intravascular fluid volume deficit, which is associated with hypotension, inadequate tissue oxygenation and hypoperfusion of essential organs. Intravascular volume resuscitation is therefore of primary importance. Crystalloid solutions have the disadvantage of only small amounts remaining in the IVS whereas colloids are known as plasma volume expanders due

Intravital imaging of mucus transport in asthmatic mice using microscopic optical coherence tomography.

Asthma is one of the most common chronic diseases. Mucus overproduction is consistently linked to asthma morbidity and mortality. Despite the knowledge of the importance of mucus, little data exist on how mucus is transported in asthma and the immediate effects of therapeutic intervention. We therefore used microscopic optical coherence tomography (mOCT) to study spontaneous and induced mucus transport in an interleukin-13 (IL-13)-induced asthma mouse model and examined the effects of isotonic (0.9% NaCl) and hypertonic saline (7% NaCl), which are used to induce mucus transport in cystic fibro

A balancing act: drifting away from the reflexive use of "ab"normal saline.

Maintenance intravenous fluids are the most frequently ordered medications for hospitalized children. Since the American Association of Pediatrics published national guidelines, there has been an increased reflexive use of isotonic solutions, especially 0.9% saline, as a prophylaxis against hyponatremia. In this educational review, we discuss the potential deleterious effects of using 0.9% saline, including the development of hyperchloremia, metabolic acidosis, acute kidney injury, hyperkalemia, and a proinflammatory state. Balanced solutions with anion buffers cause relatively minimal harm wh…

Maintenance fluids in prehospital care: crystalloid versus dextrose solutions--is there a difference?

D5W is the maintenance fluid often used in prehospital care when transporting patients with cardiac or central nervous system processes. However, there is evidence that dextrose solutions are potentially harmful, and that suggests isotonic crystalloid solutions are the preferred maintenance fluid in treating emergent patients regardless of their underlying disease. Dextrose solutions may exacerbate cellular ischemic damage and they cannot be used to resuscitate hypotensive patients. Crystalloids do not cause fluid overload when used at maintenance rates and are effective resuscitative agents i

Gastric emptying after different surgical procedures for duodenal ulcer.

Gastric emptying has been studied in patients operated upon for duodenal ulcer by either 1) parietal cell vagotomy without or 2) with pyloroplasty, 3) truncal vagotomy combined with pyloroplasty or 4) antral resection, 5) gastric resection a.m. Billroth I or 6) Billroth II. Isotonic saline and 10% glucose solution have been used as test meals. Volumes of meal remaining 10 and 30 minutes after the instillations of the saline and the glucose meal respectively have been estimated. After parietal cell vagotomy without pylorplasty (PCV) the gastric emptying rate of 10% glucose solution was signific

Monoclonal antibody delivery to intraperitoneal tumors in rats: effects of route of administration and intraperitoneal solution osmolality.

Monoclonal antibody (MAb) transport in peritoneal tissue is dominated by convection, which is dependent on the net driving force of i.p. hydrostatic and osmotic pressure. To test the hypothesis that the i.p. osmolality has significant effects on IgG delivery to the tumor during the acute period after injection, solid tumors (FEMX-II) were transplanted into the anterior abdominal wall of nude rats. The wall is subject to well-defined pressure forces from the solution in the cavity. MAb 96.5, which specifically binds to FEMX-II cells, was simultaneously injected i.v. Stability of methacholine chloride in isotonic sodium chloride using a capillary electrophoresis assay.

OBJECTIVE: To investigate the stability of methacholine chloride in 0.9% sodium chloride solutions. METHOD: Methacholine powder was mixed with diluent to a final concentration of 5 and 10 mg/ml. Duplicates of each admixture were divided and stored in glass vials at 25 degrees C, 4 degrees C and -20 degrees C for 12 months. At appropriate times intervals, samples were removed from solutions and analysed. Methacholine concentrations were measured using a high performance capillary electrophoresis assay. RESULTS: No colour or other visual changes were seen in any sample. However, an additional

Hypertonic-hyperoncotic solutions improve cardiac function in children after open-heart surgery.

OBJECTIVES: Hypertonic-hyperoncotic solutions are used for the improvement of micro- and macrocirculation in various types of shock. In pediatric intensive care medicine, controlled, randomized studies with hypertonic-hyperoncotic solutions are lacking. Hypertonic-hyperoncotic solutions may improve cardiac function in children. The primary objective of this controlled, randomized, blinded study was to evaluate the hemodynamic effects and safety of hypertonic-hyperoncotic solution infusions in children shortly after open-heart surgery for congenital cardiac disease. The secondary objective was

[Transfusion-infusion therapy in modern wars].

All previous experiences have shown that the application of blood, blood components and intravenous solutions presents an unreplaceable therapeutical measure in modern surgical-resuscitative management of war injuries. Together with the broad application of the whole blood, there have been also used other blood components (even such as cryoprecipitate and platelet rich plasma). Among intravenous solutions the most frequently mentioned were isotonic saline, Ringer's lactate solution, glucosaline, 5% dextrose solution, dextran solutions and, recently, human albumin solutions. Due to a high risk

Differential responses in adrenal and renal nerves to CNS osmotic stimulation.

Hypertonic solutions act in the central nervous system (CNS) to increase mean arterial blood pressure (MAP) by activation of the sympathoadrenal axis. However, adrenal nerve activity (pre- and postganglionic nerve fibers) has not been determined during central osmotic stimulation. Therefore, these experiments evaluated adrenal (AdSNA) and renal (RSNA) sympathetic nerve activity, MAP, and heart rate (HR) following CNS administration of isotonic, hypertonic, and hypotonic sodium chloride solutions in chloralose-anesthetized rats. Injection of isotonic saline (5 microliters) did not alter MAP, HR

Terminology used to describe volume expanding resuscitation fluid.

Many published reports state that the type of resuscitation fluid to be used for volume expansion should be "isotonic crystalloid" or "isotonic fluid". Generally, this terminology refers to normal saline (NS) and lactated Ringer's (LR) solution. This terminology is suboptimal technically since not all "isotonic" crystalloids are sufficient volume expanders. A total of 48 reference books from the areas of general pediatrics, pediatric emergency medicine, emergency medicine, pediatric critical care, pediatric surgery, general surgery, internal medicine, family medicine, adult critical care, obst

Resuscitation Targets, Fluids, and Vasoactives in Septic Shock.

Intravenous fluids and vasoactive medications represent the foundation of hemodynamic resuscitation for septic shock. Based on current evidence, initial fluid resuscitation of roughly 30 mL/kg with balanced crystalloids is reasonable for most patients. Carefully monitored peripheral vein administration of the first-line vasopressor, norepinephrine, appears safe. Ongoing multimodal assessment and therapy titration integrating blood pressure, lactate clearance, capillary refill time, and dynamic measures of fluid responsiveness yields outcomes equivalent to protocolized goal-directed resuscitati

Burn State of the Science: Fluid Resuscitation.

Recognition of fluid creep has driven a large amount of the scientific investigation in the area of acute fluid resuscitation for burn patients. The role of colloids in ameliorating fluid creep is controversial, despite the fact that a fluid-sparing effect of colloids has been recognized for some time. All but one of the available prospective studies using colloids are more than a decade old, and a modern randomized controlled trial (RCT) comparing crystalloids to colloids is long overdue. While urinary output continues to be the main endpoint for fluid titration, there has been a moderate amo

Crystalloid and colloid therapy.

Fluid therapy is a cornerstone of emergency medicine, but equine practitioners should be aware of recent developments that have modified previous recommendations. First, new emphasis on the avoidance of hyperchloremia suggests that crystalloids with a lower chloride concentration may be more appropriate for use. Second, modifications to the understanding of the Starling equation suggest that the benefits of colloids may be more limited than previously thought. In addition, the negative effects of fluid overload on morbidity and mortality are becoming increasingly recognized. Although more spec
