Hyperbilirubinemia and neonatal jaundice.

Jaundice is a common physiologic problem seen in both term and preterm infants. Normal transitional changes in bilirubin metabolism lead to physiological jaundice in many infants. In some infants these normal changes at birth may be exaggerated, such as occurs with immaturity, or may interact with health alterations (pathologic jaundice), resulting in the accumulation of excess bilirubin and development of hyperbilirubinemia. Caregivers must appreciate the processes and the basis for physiologic jaundice and hyperbilirubinemia and recognize infants at risk for these disorders. This article rev…

Neonatal jaundice in Asia.

Neonatal jaundice is a major clinical problem globally, especially in the Asian and south-east Asian regions. There is no universal definition of hyperbilirubinaemia, and comparisons of management and control of hyperbilirubinaemia in infants at different centres are difficult. G6PD deficiency, ABO incompatibility, low birth weight and sepsis are the common causes of neonatal jaundice, but there is a group of babies whose cause of neonatal jaundice has yet to be found. Genetic factors may be responsible for ethnic differences in the ability to eliminate bilirubin, while unidentified environmen…

Management of neonatal jaundice.

Jaundice is the most commonly encountered neonatal clinical problem; 80 percent of neonates become clinically jaundiced, while 5 percent develop serum bilirubin levels above currently recommended treatment standards. This article outlines theories about the pathophysiology of neonatal jaundice and presents a logical approach to its management. First, the health care provider must distinguish between physiologic, exaggerated and pathologic jaundice. The treatment modalities of exchange transfusion, phototherapy and cessation of nursing are discussed.

Jaundice and chronic pancreatitis.

A study was conducted on a group of 48 patients presenting with jaundice during the course of chronic pancreatitis. Two different patterns of symptoms could be opposed: (1) Short-lasting cholestatic jaundice which is accompanied by pain, but free from pruritus and fever, is linked to a medium consumption of alcohol, occurs in the course of advanced chronic pancreatitis and seems to be the consequence of a stricture of the main bile duct due to pancreatic lesions. (2) Protracted, intense jaundice which is linked to a high alcohol consumption, is encountered among less-advanced cases of chronic …

Factors affecting neonatal jaundice.

Plasma bilirubin was estimated on 690 term infants on about the 6th day of life. Perinatal factors were recorded and the results analysed. Hyperbilirubinaemia was defined as a level greater than 205 micromol/1 (12 mg/100 ml) and this was present in 20% of cases. Three factors--epidural analgesia, breast feeding, and poor weight recovery--showed highly significant associations with jaundice. The relative importance of these is discussed and compared with recent reports. Induction of labour, for reasons other than postmaturity, and a gestational age less than 39 weeks showed a slightly increased…

Jaundice after renal allotransplantation.

Of 567 patients receiving renal transplantation at the University of Minnesota between October 1967 and October 1975, 22 developed clinical jaundice. Of these 22, nine died with their initial episode of hepatitis, six died within three months of causes associated with liver malfunction, four developed evidence of chronic hepatic failure and only three totally recovered from their illness. Five had clear evidence of Australia antigen positive hepatitis B, four of cytomegalovirus hepatitis, two of herpes hominis hepatitis, one of varicella zoster hepatitis and three of hepatic failure associated…

Jaundice in pregnancy--1976.

An integrative survey is given of three disease processes, in which recent progress of a fundamental nature has been made, primarily affecting the liver, either coincident with or caused by the gravid state. The three conditions considered include (1) recurrent cholestasis of pregnancy (RCP), (2) viral hepatitis coincident with pregnancy, and (3) acute fatty liver of pregnancy (AFLP). In addition to an assessment of our present knowledge with respect to RCP and AFLP, new genetic hypotheses are proposed. In the latter, the proposal of an ornithine transcarbamylase deficiency, similar to that se
