Ligamentum teres reconstruction with labrum and capsule repair after posterior acetabular wall fracture: a case report.

The aim of this case study is to present arthroscopic treatment of recurrent hip instability after acute post-traumatic posterior hip dislocation with a fracture of the posterior acetabular wall. A male patient aged 35 suffered a dislocation of the right hip joint with a fracture of the posterior acetabular wall due to an accident. The fracture was stabilized during emergency surgery with a locking compression plate, and the patient was released home in a hip brace. Multiple dislocations of the hip joint followed with the implant being confirmed as stable. Decision was made to qualify the pati

[Frozen shoulder: A long-lasting and misunderstood clinical problem].

Frozen shoulder (FS), also known as adhesive capsulitis, is a painful inflammatory fibrotic disease of the glenohumeral joint capsule. While it's frequently self-limiting, patients can be symptomatic for years. The clinical course is often divided into three phases: the freezing phase with predominantly pain, the frozen phase with mainly stiffness, and the thawing phase during which the complaints slowly resolve. Diagnosing FS can be challenging during the freezing phase as the symptoms in this phase are similar to other common shoulder conditions (such as subacromial pain syndrome). Treatment

Cadaveric Evaluation of Dorsal Intermetatarsal Approach for Plantar Plate and Lateral Collateral Ligament Repair of the Lesser Metatarsophalangeal Joints.

BACKGROUND: Access to the plantar plate has been described using either a plantar approach or an extensive dorsal approach that required complete joint destabilization and often a metatarsal osteotomy. Clinical scenarios related to plantar plate tear vary and the pathologies in early stages are frequently limited to unilateral soft tissue structures; a less invasive operative approach may be possible. A novel approach requiring a release of only the lateral collateral ligament and the lateral half of the plantar plate is presented in this cadaver model; the extent of joint exposure possible is

Surgical managements of subluxation and dislocation of the temporomandibular joint: clinical and experimental studies.

Subluxation and dislocation of the temporomandibular joint (TMJ) are very unpleasant and distressing conditions to patients; they are not common diseases, but large groups of people in countries such as Yemen and Somalia are affected owing to daily chewing of Qat for several hours every day, but in Iraq, the conditions are not common. These clinical studies were conducted on 133 patients: 87 men and 36 women; their age ranged between 18 and 72 years (mean, 45 y), and their complains were difficulties of chewing food, speech, laughing, or yawning. Patients were divided into 3 groups: the first

Assessing the relationship of the peroneal nerve to the posterolateral corner of the knee and influence of race on its dimensions- A MRI based study in Indian population.

BACKGROUND: The relationship of Common peroneal nerve (CPN) to the posterolateral corner of the knee joint is important for surgeons who perform total knee arthroplasty to avoid injury to the nerve during surgery. This relationship varies among different races on account of anthropometry. This study aims to evaluate the anatomical location of this nerve in Indian patients using an MRI based reference system. METHODS: 213 knee magnetic resonance images (MRIs) were evaluated in axial plane 8 mm below the joint line for distance of the CPN from the closest posterolateral capsule. The angle of th

Comparison of therapeutic efficacy between arthroscopic capsular release and transarterial musculoskeletal embolization in patients with adhesive capsulitis of the shoulder.

BACKGROUND: Adhesive capsulitis is a debilitating shoulder disorder characterized by pain and restricted motion. While arthroscopic capsular release is a well-established surgical treatment, transarterial musculoskeletal embolization (TAME) has recently emerged as a minimally invasive alternative. The aim of this study is to compare the short-term clinical outcomes of arthroscopic capsular release and TAME in patients with adhesive capsulitis of the shoulder. MATERIAL AND METHODS: We retrospectively analyzed 40 patients with refractory adhesive capsulitis who underwent either arthroscopic cap

Feasibility of Laryngeal Joint Replacement: A Proof of Concept Study in Cadavers.

OBJECTIVE: Posterior laryngeal stenosis is a vexing problem despite efforts at early tracheostomy in those intubated on ventilatory support. Treatment in advanced cases relies on permanent tracheostomy or irreversible arytenoidectomy/cordectomy, which are compromises between voice, swallowing, and airway. While having intact laryngeal neuromuscular function, there is immobility of the vocal cord due to scarring and ankylosis of the cricoarytenoid joint. In this proof of concept study, we explore the feasibility of repairing cricoarytenoid joint function with joint replacement in the fresh cada

Advanced imaging and arthroscopic management of shoulder contracture after birth palsy.

Modern imaging techniques applied to the pediatric glenohumeral joint have advanced understanding of the anatomic changes that occur secondary to muscular imbalance after brachial plexus birth palsy. A better understanding of the progression and timing of glenohumeral dysplasia has also increased awareness and vigilance of this problem. Early detection of glenohumeral joint subluxation is now possible, allowing for prompt treatment with closed, arthroscopic, or open joint reduction with and without tendon transfers. Dynamic ultrasound imaging, Botox, and arthroscopic techniques have expanded t

Acquired Hallux Varus Deformity Correction: Pivotless Distraction Technique.

Acquired hallux varus deformity secondary to postburn contracture is a rare condition. It causes cosmetic disfigurement, pain, and inability to wear a shoe. Soft-tissue procedures and tendon transfers have been described for correction; however, these may require multiple operations and the outcome may be poor. We report a 6-year-old neglected case of hallux varus deformity secondary to burn contracture successfully managed by contracture release with pivotless distraction technique using a biplanar distractor and skin grafting with Z-plasty in the same procedure. The deformity was corrected t

Two-stage total hip arthroplasty following skeletal traction after extensive soft tissue release for severe limb-length discrepancy.

INTRODUCTION: To maximise limb-length discrepancy (LLD) resolution during total hip arthroplasty (THA) for untreated developmental hip dysplasia or septic hip sequelae, THA following limb lengthening was introduced using different methods. We aimed to evaluate 2-stage THA results following limb lengthening via skeletal traction after extensive soft tissue release. METHOD: In total, 24 hips with severe LLD in 10 men and 14 women (mean age, 49.6 ± 15.2 years) underwent 2-stage THA and were followed thereafter (mean 6.3 ± 3.7 years). The entire abductor muscle origin was subperiosteally released

Acquired valgus instability after knee replacement.

Four cases of acquired valgus instability occurred after knee resurfacing operations; all four were initially stable. Three cases originally had a varus deformity which required extensive releases for its correction. All of these patients fell, disrupting the medial joint capsule and medial collateral ligament. This complication in the two early cases could have been prevented by adequate bracing until full recovery of the quadriceps had occurred. In one case, attribution due to projection medially of the tibial component may have played a role. In the fourth case, the tibial component was ins

Regional distribution prevalence of heterotopic ossification in the elbow joint: a 3D study of patients after surgery for traumatic elbow injury.

BACKGROUND: Heterotopic ossification (HO) is a common complication after elbow fracture surgery and can lead to severe upper extremity disability. The radiographic localization of postoperative HO has been reported previously. However, there is no literature examining the distribution of postoperative HO at the three-dimensional (3D) level. This study aimed to investigate 1) the distribution characteristics of postoperative HO and 2) the possible risk factors affecting the severity of postoperative HO at a 3D level. METHODS: A retrospective review was conducted of patients who presented to ou

Posterolateral capsular release for correction of valgus deformity.

Achieving coronal balance during arthroplasty of a valgus knee presents a challenge for the surgeon. Various techniques for lateral release had been described with no consensus among surgeons. We retrospectively reviewed 42 consecutive patients (38 women, mean age 72.7 years, follow-up 42 ± 23 months) with a valgus deformity who were operated while using a posterolateral capsular release as the sole method of lateral release. All knees were successfully balanced, without the need for further release of other structures. Valgus angles improved from 17.5 ± 4.6 degrees (range 11 to 31) to 6.3 ± 2

The midlumbar myelomeningocele hip: mechanism of dislocation and treatment.

We define the "bowstring" force of the iliopsoas, the force applied to the femoral head as the tendon angles across the hip joint, which we believe is an important cause of dislocation of the hip in patients with a midlumbar myelomeningocele. An operative procedure consisting of iliopsoas recession and suture of its tendon to the anterolateral hip joint capsule has been developed and used in 10 dislocated and 9 subluxated hips. The procedure was performed on patients with an average age of 6.6 months; the average follow-up was 45 months. Arthrography demonstrated the subluxation pattern of Lev

Arthroscopic Management of Arthrofibrosis After Hemiarthroplasty for Proximal Humerus Fracture Dislocation.

Poor functional outcomes after hemiarthroplasty for proximal humerus fractures are common, yet revision surgery is relatively rare. Arthroscopic treatment for postoperative stiffness can be considered in the setting of functional limits to glenohumeral range of motion impacting activities of daily living after adequate conservative treatment with physical therapy and in the setting of healed, well-positioned tuberosities and humeral components. This Technical Note illustrates a stepwise approach to an arthroscopic lysis of adhesions and capsular release for the treatment of arthrofibrosis of t

Elbow arthroscopy.

Arthroscopy of the elbow was originally considered to be an unsafe procedure because of the small size of the elbow joint capsule and its proximity to several crucial neurovascular structures. Over the past decade, however, the procedure has become safer and more effective. These improvements can be attributed to a better understanding of elbow anatomy and of the disorders about the elbow as well as to advances in arthroscopic equipment and surgical technique. The most common indications for elbow arthroscopy include removal of loose bodies, synovectomy, débridement and/or excision of osteophy

Revision Total Hip Arthroplasty with Femoral Component Retention.

BACKGROUND: Revision total hip arthroplasty (THA) for isolated polyethylene exchange or acetabular revision with retention of the femoral component can present a challenge for adequate exposure. A systematic approach to a proper release can facilitate exposure and reduce the risk of iatrogenic complications. DESCRIPTION: The posterior approach is an extensile and versatile approach for revision THA. After incising the fascia and iliotibial band, the insertion of the gluteus maximus is fully released. After releasing any scar along the inferior gluteus medius, a retractor is placed to hold the
