Effects of macrolide antibiotics on CYP3A expression in human and rat hepatocytes: interspecies differences in response to troleandomycin.

The effects of various macrolide antibiotics [triacetyloleandomycin (TAO), clarithromycin, azithromycin, roxithromycin, erythromycin base] and the new ketolide HMR3004 on CYP3A expression were evaluated in human and rat hepatocytes. Cells were treated for 3 days with nontoxic concentrations of the drugs, and CYP3A induction was assessed through midazolam hydroxylase activity and Western and Northern blot analyses. In rat hepatocytes, no induction of CYP3A1 expression was observed following exposure to macrolides, even to erythromycin base and TAO (well known in vivo CYP3A1 inducers), whereas d

Benefit-risk assessment of telithromycin in the treatment of community-acquired pneumonia.

The purpose of this review is to assess the benefits and risks associated with the use of the ketolide antibacterial telithromycin, currently licensed for the treatment of adults with mild to moderate community-acquired pneumonia (CAP). Telithromycin is active against both the major (Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis) and atypical/intracellular (Chlamydophila pneumoniae, Legionella pneumophila and Mycoplasma pneumoniae) CAP pathogens. It is associated with a low potential to select for resistance and has maintained its in vitro activity against isolates

[New antibiotics for the therapy of respiratory tract infections].

The need for new antibiotics is ongoing in view of manifold resistance developments. The resistance of Streptococcus pneumoniae against macrolides and penicillins in the USA already amounts to more than 30 %. The approval of new antibiotics, particularly those with a new mode of action, has been decreasing over the past years which is anticipated to result in a lack of resourceful therapy options in the future. Some new substances with good in vitro activity against S. pneumoniae have turned out to raise safety issues as shown for a number of quinolones. Likewise daptomycin, a lipopeptide with

Structural studies on ribosomes of differentially macrolide-resistant Staphylococcus aureus strains.

Antimicrobial resistance is a major global health challenge, diminishing the efficacy of many antibiotics, including macrolides. In Staphylococcus aureus, an opportunistic pathogen, macrolide resistance is primarily mediated by Erm-family methyltransferases, which mono- or dimethylate A2058 in the 23S ribosomal RNA, reducing drug binding. Although macrolide-ribosome interactions have been characterized in nonpathogenic species, their structural basis in clinically relevant pathogens remains limited. In this study, we investigate the impact of ermB-mediated resistance on drug binding by analyzi

New antibiotics: optimal use in current clinical practice.

The optimal choice of antibacterial therapy among the few available options for infections caused by pathogens with advanced antimicrobial drug resistance is fundamental to maximize clinical effectiveness and minimize the likelihood for further resistance development. We herein review the available data on the effectiveness of antibiotics introduced in clinical practice during the past 10 years for specific clinical indications. Quinupristin-dalfopristin, linezolid, daptomycin and tigecycline have increased the available therapeutic options against specific types of meticillin-resistant Staphy

[Recent trend for the development of new antibacterial agents].

Multidrug resistant organisms such as MRSA, PRSP, BLNAR, ESBL-producing K. pneumoniae and VRE have in recent years emerged as significant pathogens in hospitals. In this paper, I describe the recent trend for the development of new antibacterial agents that are active against these multidrug resistant organisms. A number of new cephem and carbapenem antibiotics were identified to have attractive potency against MRSA, PRSP and P. aeruginosa. These beta-lactam antibiotics had high affinity for PBP 2a of MRSA. AM-112 is a novel beta-lactamase inhibitor that has potent activity against class A, B,

Structural insights into the mechanism of overcoming Erm-mediated resistance by macrolides acting together with hygromycin-A.

The ever-growing rise of antibiotic resistance among bacterial pathogens is one of the top healthcare threats today. Although combination antibiotic therapies represent a potential approach to more efficiently combat infections caused by susceptible and drug-resistant bacteria, only a few known drug pairs exhibit synergy/cooperativity in killing bacteria. Here, we discover that well-known ribosomal antibiotics, hygromycin A (HygA) and macrolides, which target peptidyl transferase center and peptide exit tunnel, respectively, can act cooperatively against susceptible and drug-resistant bacteria

Design and synthesis of novel macrolones bridged with linkers from 11,12-positions of macrolides.

Resistance to telithromycin and off-target effects associated with the metabolic instability present serious and challenging problems for the development of novel macrolides. Herein, studies of hybrids of macrolides and quinolones (termed macrolones) bridged with linkers from 11,12-cyclic carbamate of macrolides revealed different structure-activity relationships from the previously reported macrolones bridged with linkers derived from 6-, 9- and 4''-positions of macrolides. The optimized macrolone 34 g with a longer and rigid sidechain than telithromycin had improved metabolic stability compa

In vitro bactericidal activity of ABT-773 and amoxicillin against erythromycin-susceptible and -resistant strains of Streptococcus pyogenes.

The bactericidal activity of ABT-773 was compared with amoxicillin against 10 clinical isolates of S. pyogenes (six erythromycin susceptible and four erythromycin resistant). The MIC ranges (mg/L) were 0.004-0.25 of ABT-773 and 0.015-0.12 of amoxicillin. At 24 h, ABT-773 concentrations of 2 x MIC and 8 x MIC were bactericidal against three and six organisms, respectively. In comparison, amoxicillin was bactericidal against all 10 organisms at both test concentrations.

Interpretative criteria and quality control parameters for telithromycin disc diffusion susceptibility tests.

In vitro studies were carried out with 4175 Gram-positive clinical isolates, comparing telithromycin broth microdilution MICs with disc diffusion zone diameters. Provisional interpretive criteria for testing staphylococci were > or =23 mm for susceptible and < or =19 mm for resistant. Because pneumococci and other streptococci require increased CO(2), the corresponding zone sizes are 3 mm smaller (> or =20 mm and < or =16 mm). Two collaborative studies were performed in order to propose the following quality control limits: 24-30 mm for Staphylococcus aureus ATCC 25923 and 27-33 mm for Strepto

The lifestyle of Corynebacterium urealyticum derived from its complete genome sequence established by pyrosequencing.

Corynebacterium urealyticum is a lipid-requiring, urealytic bacterium of the human skin flora that has been recognized as causative agent of urinary tract infections. We report the analysis of the complete genome sequence of C. urealyticum DSM7109, which was initially recovered from a patient with alkaline-encrusted cystitis. The genome sequence was determined by a combination of pyrosequencing and Sanger technology. The chromosome of C. urealyticum DSM7109 has a size of 2,369,219bp and contains 2024 predicted coding sequences, of which 78% were considered as orthologous with genes in the Cory

What's new in clinical pharmacology and therapeutics.

The US Food and Drug Administration (FDA) has approved several new drugs in the last few years. We have summarized a few of these that should be of interest to a primary care physician. These belong to either a new class of drugs or have a better drug profile in terms of ease of administration, prolonged duration of action, or fewer side effects. Daptomycin is a cyclic lipopeptide, active against methycillin resistant Staphylococcus aureus (MRSA). Telithromycin is a ketolide that can be used in place of macrolide antibiotics. Rifaximin is a semi-synthetic derivative of rifampin approved by the

[Risks of antibacterial agents in pregnancy].

Antimicrobial therapy in the pregnant woman has to consider the potential risks of antibacterial agents for the developing foetus and the mother. Extensive clinical experience shows that penicillins, cephalosporins and erythromycin (except erythromycin estolate) can be considered safe for the developing foetus and for the pregnant woman. Nitrofurantoin is a valid antibacterial option in pregnancy, except in the latter stages. Isoniazid and ethambutol are the safest drugs for the treatment of tuberculosis in pregnancy, but attention must be paid to the potential toxicity of isoniazid for the mo

Molecular Mechanisms of Drug Resistance in Staphylococcus aureus.

This paper discusses the mechanisms of S. aureus drug resistance including: (1) introduction. (2) resistance to beta-lactam antibiotics, with particular emphasis on the mec genes found in the Staphylococcaceae family, the structure and occurrence of SCCmec cassettes, as well as differences in the presence of some virulence genes and its expression in major epidemiological types and clones of HA-MRSA, CA-MRSA, and LA-MRSA strains. Other mechanisms of resistance to beta-lactam antibiotics will also be discussed, such as mutations in the gdpP gene, BORSA or MODSA phenotypes, as well as resistance

Acquired resistance to macrolides in Pseudomonas aeruginosa from cystic fibrosis patients.

Cystic fibrosis (CF) patients receive chronic treatment with macrolides for their antivirulence and anti-inflammatory properties. We, however, previously showed that Pseudomonas aeruginosa, considered as naturally resistant to macrolides, becomes susceptible when tested in a eukaryotic medium rather than a conventional broth. We therefore looked for specific macrolide resistance determinants in 333 CF isolates from four European CF centres in comparison with 48 isolates from patients suffering from hospital-acquired pneumonia (HAP).Minimum inhibitory concentrations (MICs) of macrolides and keto

Antimicrobial resistance in Haemophilus influenzae.

Haemophilus influenzae is a major community-acquired pathogen causing significant morbidity and mortality worldwide. Meningitis and bacteremia due to type b strains occur in areas where the protein-conjugated type b vaccine is not in use, whereas nontypeable strains are major causes of otitis media, sinusitis, acute exacerbations of chronic bronchitis, and pneumonia. Antibiotic resistance in this organism is more diverse and widespread than is commonly appreciated. Intrinsic efflux resistance mechanisms limit the activity of the macrolides, azalides, and ketolides. beta-Lactamase production is

Antibiotic susceptibility of Tropheryma whipplei in MRC5 cells.

Whipple's disease is considered a rare chronic disease with a broad spectrum of clinical manifestations. Several antibiotics have been used for the treatment of this disease, and the current reference treatment was determined empirically on the basis of only a few clinical observations. Patients should be treated for months, and many relapse after antibiotic withdrawal. We report here the first extensive study on the susceptibilities of three reference strains of Tropheryma whipplei to antibiotic in cell culture by using a real-time PCR assay as previously described. We found that doxycycline,

Characteristic expression of three genes, msr(A), mph(C) and erm(Y), that confer resistance to macrolide antibiotics on Staphylococcus aureus.

We have reported that the gene mph(C) (formally referred to as 'mphBM') is located on plasmid pMS97 342 bp downstream of the msr(A) gene. msr(A) specifies resistance to macrolides by ABC-transporter-mediated efflux, and mph(C) has 49% identity to the amino acid sequence of MPH(2')II, which encodes a phosphotransferase that inactivates some macrolide antibiotics. A strain of Staphylococcus aureus NCTC8325 containing plasmid pMS97 inactivated unlabeled and (14)C-labeled erythromycin when tested by bioautographic and radioautographic techniques.
