Ultrasonography of the knee.

Sonography is a useful modality in the diagnosis of a wide variety of knee disorders, including lesions of the articular cartilage, tendons and ligaments, menisci, synovial space, and adjacent vessels and muscles. Sonography is noninvasive, rapidly performed, widely available, readily accepted by patients, uses no ionizing radiation, and is relatively inexpensive compared to other conventional imaging studies of the menisci, making it a potentially attractive screening examination for knee injuries. Although it is difficult to state the exact place of sonography in the diagnostic workup of kne…

Dislocation of the knee.

The complications of knee dislocations may be disastrous and must be anticipated. In most series, a 25 to 30 per cent incidence of arterial damage is reported. If the artery is not repaired, the incidence of amputation can be up to 72.5 per cent. The popliteal artery should be evaluated and vascular repair performed as needed. This must be completed within a 6 to 8-hour period after injury for optimal results. Peroneal nerve injuries are common, and permanent neurologic damage may result. The neurovascular injury should be well documented both at the time of injury and in the preinjury state. …

[The knee joint].

The knee is very important in the treatment of rheumatic diseases. Differentiating between the indications for different forms of therapy is dependent upon knowledge of the prospects for success. Synovectomy, preventive and palliative procedure, is the most important joint-saving measure, as confirmed by the long-term results presented here. Synoviorthese and arthroscopic operative procedures are alternatives and have expanded the spectrum. However, their long-term effectiveness has not yet been proved. Rheumatic, recurring synovitis has not yet been defined satisfactorily. Additional soft tis…

Arthroscopy of the knee.

Fiberoptics permit a minimally invasive direct inspection of the knee joint. Arthroscopy is indicated when no definitive clinical diagnosis can be made. An organized approach includes inspection of the suprapatellar pouch, the articular surfaces of the patella, the medial and lateral compartments of the knee and the intercondylar area. Common afflictions diagnosed by arthroscopy include chondromalacia, meniscal lesions, osteoarthritis, cruciate ligament injuries, chronic synovitis and loose bodies. Complications and morbidity are minimal.

[Arthrofibrosis of the knee].

Arthrofibrosis is a complex complication following surgery or trauma, leading to reduced mobility, pain, and decreased quality of life. The condition results from an excessive inflammatory response and the formation of scar tissue. Diagnosis is clinical, excluding mechanical and infectious causes. Treatment includes exercise, patient education, and, in select cases, surgery. This review finds that prognosis depends on early treatment and an individualised approach, while further research is needed for better diagnostics and management.

The locked knee.

The acute locked knee is an orthopaedic emergency requiring prompt diagnosis and treatment. It can be classified as acute or chronic. The term 'locked knee' refers to a knee that demonstrates fixed flexion or which has a 'block' to complete extension. Some degree of active or passive extension may be achievable, but not full extension. The most frequent causes of a locked knee are a meniscal tear, rupture of the anterior cruciate ligament or loose bodies. Magnetic resonance imaging is the gold standard in diagnostic imaging. Knee arthroscopy is considered the gold standard in management. This …

Arthrofibrosis of the knee.

Better understanding of surgical timing, improved surgical technique, and advanced rehabilitation protocols has led to decreased incidence of motion loss after anterior cruciate ligament injury and reconstruction. However, motion loss from high-energy, multiligament injuries continues to compromise functional outcome. Prevention, consisting of control of inflammation and early motion, remains the key element in avoiding motion loss. However, certain techniques, such as manipulation under anesthesia in conjunction with arthroscopic lysis of adhesions, are reliable treatment options. Open surgic

Actinomycosis of the knee.

We report a case of actinomycosis presenting as a knee swelling in a 34 year-old man. Knee actinomycosis poses a diagnosis challenge to clinicians as it is rare, often mimics knee tuberculosis and culture of the causative microbes is technically difficult. The classic microscopic appearance of this Gram-positive bacteria often forms the basis of diagnosis.

Moreover, the aspects of this disease entity that poses a diagnostic challenge to the treating physician are highlighted together with those that differentiate it from other overlappin …

Prosthetic knees.

The purpose of this article is to provide a framework for evaluating the features of a knee. A single knee unit has a combination of several control features. As is expected, new units are introduced with improvements to certain features. Prosthetic knee selection generally is based on the patient's ability. An inappropriate knee selection (or improper adjustment) may leave the patient apprehensive and fearful that the knee might buckle with each step. An appropriate selection of a properly adjusted knee unit closely matches the functional ability of the patient and yields a smooth, reliable g…

[Tendinitis of the knee].

Tendinitis of the knee occurs principally in those playing at competition level or for pleasure. Localisation of the tendinitis depends upon the type of sport; amongst those who jump the patellar tendinitis is predominant, with long distance runners tenosynovitis of the iliotibial tract are more often found, for activities involving lateral and brusque++ movements tendinitis of the biceps femoris and tenosynovitis of the "patted'oie" tend to occur. Whilst tendinitis is relatively easy to diagnose, its treatment is more difficult. Of the numerous treatments available, specific physiotherapy-mas…

Arthroscopy of the knee.

Arthroscopy of the knee was first performed early this century. However, it became generally accepted and grew to be a major contributor to orthopaedic surgery only during the last two decades. It has improved our understanding of numerous knee pathologies and allowed us to treat many of these disorders with considerably less morbidity. But there remain areas that warrant attention. Training in arthroscopic technique needs to be intensified and our knowledge of knee conditions should be improved if we are to avoid certain pitfalls and complications during arthroscopic surgery. Another issue th…

The Dislocated Knee.

Knee dislocation, although relatively rare, may be the result of high-or low-velocity injuries. Well established is the need for urgent diagnosis and treatment to avoid vascular complications and amputation. The initial evaluation should include objective assessment of arterial circulation by means of Doppler pressure measurements; the finding of any asymmetric pressure warrants an arteriogram. Late arterial occlusion may occur, which mandates careful serial reexamination in all patients, including those with initially symmetric pressure. Injury to the peroneal nerve is also common, and the re…

Dislocation of the knee.

The results of treatment for vascular and ligamentous injuries in 17 patients who suffered complete dislocation of the knee were reviewed. Eight patients had 23 associated injuries. All nine patients who had injuries of the popliteal artery had arterial reconstruction with saphenous vein bypass grafts, which were successful in eight (89%). Intraoperative arteriography after vascular repair showed a narrowed anastomosis requiring revision in two (22%) of the nine patients and distal thrombi requiring removal in five (55%) patients. Delay in arterial repair was responsible for the one failure. E…
