The current study is a review of 20 patients treated by percutaneous stabilization for a floating knee. All patients were treated with a retrograde femoral intramedullary nail and a small diameter tibial intramedullary nail through a 4-cm medial parapatellar tendon incision. The average Injury Severity Score was 19. Two patients died in the early postoperative period and one patient was lost to followup. The average time to union for the 17 remaining patients with femoral shaft fractures was 14.7 weeks. One patient required dynamization. Four of the 17 patients with tibia fractures required an

Magnetic resonance (MR) imaging is the imaging modality of choice for evaluation of acute traumatic musculotendinous injuries of the knee. Three discrete categories of acute injuries to the musculotendinous unit can be defined: muscle contusion, myotendinous strain, and tendon avulsion. Among the quadriceps muscles, the rectus femoris is the most susceptible to injury at the myotendinous junction due to its superficial location, predominance of type II fibers, eccentric muscle action, and extension across two joints. Among the muscles of the pes anserinus, the sartorius is the most susceptible

AIM: We demonstrate the management and treatment of dislocation of the knee associated with vascular injury. The goal of the treatment is to avoid complications due to ischemia. The injured vessel can be repaired either by direct suture or by interposition of a saphenous vein graft. Capsule and ligaments should be reconstructed secondarily.
PATIENTS AND METHODS: The charts of ten patients treated in the Division of Traumatology of the University of Zurich between 1979 and 1996 have been retrospectively checked.
RESULTS: In eight of ten patients the injured vessel has been reconstructed with

Data on knee injuries sustained by the University of Iowa wrestling team over a 6 year period were compiled. There were a total of 136 wrestlers during this time. Fifty-one different wrestlers sustained 136 injuries (64 knees). Multiple parameters were examined for each injury, including date of injury; days lost; diagnosis; side injured; mechanism; occurrence during competition or practice; whether the wrestler was in control, "on the bottom," or both wrestlers standing; type of move being made at the time of injury; how far into a match or practice the injury happened; initial versus recurre…

Most knee injuries are the result of rotational, varus, valgus, hyperextension, or translational forces or a combination of these. The pattern of abnormalities seen on magnetic resonance imaging depends on the mechanism of trauma and the position of the joint at the time of injury. Knee injuries can be classified under two broad categories of direct and indirect injuries.

BACKGROUND: Several different etiologies cause knee pain in the pediatric and adolescent population, including anterior knee/patellofemoral pain, patellar instability, anterior cruciate ligament (ACL) tears, meniscal tears, osteochondritis dissecans (OCD) lesions, and discoid meniscus. The purpose of the current study was to determine the relative morbidity of different causes of knee pain in children and adolescents using the International Knee Documentation Committee (IKDC) score.
METHODS: We performed a retrospective review of prospectively collected data of a cohort of pediatric and adole

OBJECTIVE: To explore the clinical outcome of floating knee injury treated by open reduction and internal fixation.
METHODS: The course of treatment of floating knee in 78 cases by open reduction and internal fixation were reviewed. There were 59 males and 19 females, aged from 17 to 58 years old, with an average age of 37.5 years. Intramedullary nail, pressure plate hollow screw, multi-function single side external fixation holder and other internal fixature were used in the operation. Early exercises were followed postoperatively.
RESULTS: All patients were followed-up for 8 to 35 months (

Overuse injuries are frequent in the knee joint. The reason for this is that the knee joint is engaged in all sports activities. Furthermore, the joint area has numerous attachment points for muscles and tendons and numerous bursae. Another reason is that the specific joint between the patella and femur (patellofemoral joint) constitutes a part of the knee joint. Speaking in general terms, all overuse injuries in the knee joint can be divided in four groups according to the aspect: anterior aspect--patellofemoral pain syndrome, patellar tendinitis (jumper's knee), Osgood-Schlatter disease, Sin

Over the past decade, there has been a resurgence of interest in techniques of rehabilitation of the injured knee. Technologic advances have … ivity and the goals of both the patient and the surgeon taken into consideration. The management of knee injuries, whether from an overuse stress syndrome or following severe trauma with major ligament disruption, deal with the management of the inflammatory response. Since we do not know the exact time period for full mechanical competence following ligament h …

BACKGROUND: Multi-ligamentous knee injuries (MLKI) in pediatric patients generally occur due to high-energy mechanisms, exhibiting unique injury patterns when compared to adults. Further understanding of the injury patterns, functional outcomes, and post-surgical complications in pediatric patients sustaining MLKI is warranted.
HYPOTHESIS/PURPOSE: To systematically review the literature to evaluate causes and outcomes of MLKIs in pediatric patients.
STUDY DESIGN: Level IV Methods: Studies published in PubMed, Embase, and the Cochrane Library reporting on MLKIs in the pediatric population fro…

Similar to other fields, there is growing evidence of disparities in pediatric sports medicine. For children and adolescents with knee injuries, disparities persist along the lines of sex, gender, race, ethnicity, insurance, socioeconomic status, and other determinants. These factors influence the evaluation, treatment, and outcomes for a variety of knee pathologies, including anterior cruciate ligament ruptures, meniscus tears, osteochondritis dissecans, cartilage injuries, and tibial spine fractures. Inequities in care may have long-term functional and psychosocial implications. Additional r…

Despite early reports of high failure rates in knee ligament repair techniques resulting in favor of reconstruction, newer advances in surgical technology have shifted the attention back to repair with the addition of various tissue augmentation techniques. Ligament repair preserves proprioceptors in the native ligament and avoids autograft tendon harvest, minimizing the complications associated with donor site ruptures in reconstruction techniques. Tissue augmentation has been successfully used in knee ligamentous and tendon repair procedures, as well as in some upper extremity procedures. Th

Acute knee injury ranges among the most common joint injuries in professional and recreational athletes. Radiographs can detect joint effusion, fractures, deformities, and malalignment; however, MR imaging is most accurate for radiographically occult fractures, chondral injury, and soft tissue injuries. Using a structured checklist approach for systematic MR imaging evaluation and reporting, this article reviews the MR imaging appearances of the spectrum of traumatic knee injuries, including osteochondral injuries, cruciate ligament tears, meniscus tears and ramp lesions, anterolateral complex…

Traditionally, while managing ligament injuries around the knee, medial side injuries are frequently overlooked or considered 'benign' with very little influence on overall knee stability outcomes. However, much has changed in the recent past, and like the lateral side of the knee, it is gaining considerable attention. It is now well known that the Medial collateral ligament and Posteromedial corner are fundamentally two distinct structures that differ in anatomy and biomechanics. When it comes to decision making between conservative versus operative approach for medial side injuries, treating

The multiple ligament injured knee is a complex biomechanical environment. When primary stabilizers fail, secondary stabilizers have an increased role. In addition, loss of primary restraints puts undue stress on the remaining intact structures of the knee. Treatment of these injuries requires accurate diagnosis of all injured structures, and careful consideration of repairs and reconstructions that restore the synergistic stability of all ligaments in the knee.

Open knee joint injuries are potentially devastating injuries if not properly diagnosed and treated. Current diagnostic techniques, such as the saline load test (SLT), are based on outdated literature. Diagnosis of traumatic arthrotomies via the presence of intra-articular air on computed tomography (CT) scan has recently been shown to be 100% sensitive and specific to detect these injuries. Additionally, open knee joint injuries have a high rate of associated periarticular fractures (51%). The workhorse open surgical approach to the knee is the medial parapatellar approach; however, arthrosco
