[Reduced sense of taste as a complication of the laryngeal mask use].

We report a unilateral lingual nerve paralysis following the use of a laryngeal mask airway in a 73-year-old man with hypertrophy of the prostate undergoing a TUR-P. The operation was performed in general anaesthesia using propofol, fentanyl, alfentanil and ventilation with 100% oxygen. The operation was uncomplicated and had a duration of two hours and twenty minutes. A week later the patient perceived a reduced sense of taste. Six months later the situation was unaltered.

Positive pressure ventilation with the laryngeal mask airway in children.

We studied the safety of positive pressure ventilation (PPV) when using the size 2 laryngeal mask airway (LMA) in 46 ASA physical status I or II children (aged 38 +/- 21 mo) undergoing elective surgery. The LMA cuff was inflated in incremental steps to achieve a cuff leak pressure > or = 15 cm H2O. Abdominal circumference was measured before and after PPV in study patients, as well as in a control group managed with tracheal intubation. Cuff leak pressure was 17 +/- 4 cm H2O (range 12-34 cm H2O). Forty-five patients successfully underwent PPV. Gas leak around the LMA cuff prevented PPV in one

The laryngeal mask airway. Safety, efficacy, and current use.

In 1983, the laryngeal mask airway (LMA) was introduced as a new airway device. It can be inserted without the aid of a laryngoscope or neuromuscular blockade. Compared with the face mask, the LMA allows for a more "hands-free approach" to airway management providing the anesthesiologist the freedom of not holding a mask. In addition, the LMA is more effective than the face mask, with or without a Guedel airway, during emergency resuscitation in situations where endotracheal intubation is difficult or not possible. The LMA seems to provide better oxygenation than the face mask; but unlike the

The role of the laryngeal mask airway in pre-hospital care.

Since its introduction into clinical practice in 1988, the laryngeal mask airway (LMA) has fundamentally changed the airway management of patients undergoing routine anaesthesia. Currently in the UK, the LMA is used in > 50% of surgical procedures where an endotracheal tube (ETT) would formerly have been used. It seems timely to review the role of this device in resuscitation and its potential role in the pre-hospital arena.

Flexible bronchoscopy via the laryngeal mask: a new technique.

Malignant tracheal tumours often cause airway obstruction and this may be aggravated by vocal cord paralysis due to invasion of the recurrent laryngeal nerve. Conventional endoscopic techniques performed under general anaesthesia do not give a simultaneous view of vocal cord function and the distal airways. The technique of bronchoscopy via the laryngeal mask allowed full assessment of the cause of stridor in a patient with a malignant tracheal tumour that was causing airways obstruction and vocal cord paralysis.

The use of a laryngeal mask airway in spontaneously breathing patients.

The Laryngeal Mask Airway (LMA) is a new type of oropharyngeal airway that provides an alternative to endotracheal intubation and standard mask anaesthesia in certain cases. Once the patient is adequately anaesthetised, it can be inserted blindly, without recourse to laryngoscopy or muscle relaxants. Anaesthetists of all grades, given minimum instruction, were able to provide a clinically satisfactory airway in 49 out of 50 spontaneously breathing, anaesthetised patients. The advantages over standard mask anaesthesia are: better airway control, minimal leakage of anaesthetic gases, secure airw

[Comparison of the Supreme Laryngeal Mask Airway(SLMA), single use, with the reusable Proseal Laryngeal Mask Airway(PLMA) in anesthetized adult Japanese patients].

BACKGROUND: Supreme laryngeal mask airway (SLMA) is a single use, new supraglottic airway device with anatomically designed curved airway conduit. It has second channel for gastric tube insertion. We compared the performance of the SLMA with that of the Proseal LMA (PLMA), the older reusable LMA, during anaesthesia in spontaneously breathing adult patients. METHODS: One hundred adult patients were studied in a prospective randomized controlled study. Insertion time, first attempt success rates, leak pressure, ease of gastric tube placement, and the incidence of blood staining on removal, as w

Assessing the Efficacy of Ventilation of Anesthetized Neonatal Calves Using a Laryngeal Mask Airway or Mask Resuscitator.

Calves that have undergone a dystocia are often hypoxic and acidemic, which can result in reduced vigor and subsequent mortality. Methods of field resuscitation of apneic newborn calves are often ineffective and therefore underutilized. This proof-of-concept study aimed to determine the efficacy of the laryngeal mask airway (LMA) as well as the current industry standard method of ventilation, the McCulloch Calf Aspirator/ Resuscitator (MMR) for positive pressure ventilation of neonatal calves. Five LMA models of various sizes were first tested in cadaver heads to assess anatomical fit. Three L

[Successful intubation in a difficult case using an Ambu Laryngeal Mask Angle Type-i, and the ability to pass a gastric tube behind the laryngeal mask].

A 71-year-old male was scheduled for a laparoscopic cholecystectomy. The plan was to intubate him using direct laryngoscopy. After induction of anesthesia and analgesia direct laryngoscopy was performed but it was difficult to get a good view of epiglottis, and at 2nd attempt there was laryngeal edema, which made the procedure more difficult. The patient was then ventilated with a facemask and it was decided to intubate him with an Ambu Laryngeal Mask Angle Type-i (Aura-i) and a bronchofiberscope. The procedure worked, and the patient was successfully intubated. We did not want to remove the A

A randomized crossover comparison of the size 2 1/2 laryngeal mask airway ProSeal versus laryngeal mask airway-Classic in pediatric patients.

The laryngeal mask airway (LMA)-ProSeal (P-LMA) forms a more effective seal than the LMA-Classic (C-LMA) and facilitates gastric tube (g-tube) placement in adults. The first pediatric sizes of P-LMA recently became available. In 30 anesthetized, nonparalyzed children, aged 7.7 +/- 2 yr and weighing 27 (20-35) kg, we inserted the size (1/2) P-LMA and C-LMA in random order. Ease of insertion, quality of initial airway, fiberoptic position, airway leak pressure, and maximum tidal volume were determined. G-tube placement was assessed for the P-LMA. Ease of insertion was similar for both devices. T

Cricoid pressure impedes placement of the laryngeal mask airway and subsequent tracheal intubation through the mask.

We have studied 40 patients to see if cricoid pressure affects the success rate of ventilation through, and the position of, the laryngeal mask and subsequent fiberscope-aided tracheal intubation. Adequate ventilation of the lungs was produced through the laryngeal mask in 19 of 20 patients in the control group, but in only 10 of 20 patients in the cricoid pressure group (P < 0.002). The laryngeal mask was positioned correctly in 16 patients in the control group, but in only two patients in the cricoid pressure group (P < 0.001). Tracheal intubation was accomplished in 19 patients in the contr

Laryngeal tube S II, laryngeal tube S disposable, Fastrach laryngeal mask and Fastrach laryngeal mask disposable during elective surgery: a randomized controlled comparison between reusable and disposable supraglottic airway devices.

BACKGROUND AND OBJECTIVE: Analogue disposable devices of the laryngeal tube S II (LTS II) and the Fastrach laryngeal intubation mask (ILMA, Fastrach), namely the laryngeal tube disposable (LTS-D) and the Fastrach laryngeal intubation mask disposable (Fastrach-D), have recently been introduced. The purpose of this study was to compare each reusable device with the corresponding disposable one, during routine surgery. METHODS: After informed consent, 120 American Society of Anesthesiologists I-III patients scheduled for routine minor surgery were randomly allocated to the LTS II (n = 30), the L

Pharyngo-laryngeal discomfort after breast surgery: comparison between orotracheal intubation and laryngeal mask.

Sore throat and dysphonia are a significant source of postoperative discomfort for patients scheduled for breast surgery who have been intubated for general anaesthesia. The aim of this study was to compare the incidence of postoperative pharyngo-laryngeal pain and dysphonia in the use of the laryngeal mask airway (LMA) or tracheal intubation in these patients. In a prospective, double-blind, randomised clinical trial we studied 53 women undergoing elective breast surgery to test the hypothesis that the use of the LMA could reduce the incidence of pharyngo-laryngeal morbidity compared with tra

Laryngeal reflex before and after placement of airway interventions: endotracheal tube and laryngeal mask airway.

BACKGROUND: Previous reports indicate that detrimental laryngeal function persists over several hours after tracheal extubation even in patients who have regained full consciousness from anesthesia. The authors hypothesize that even after minor surgery, the presence of an endotracheal tube (ETT) impairs the receptors at the vocal cord and diminishes the defensive laryngeal function. The hypothesis was tested by comparing types of experimentally induced laryngeal airway reflexes before and after surgery in anesthetized patients with use of either an ETT or a Laryngeal Mask Airway. METHODS: Twe

[A case report of severe laryngeal edema which occured before removal of a laryngeal mask airway].

A 41-year-old woman was scheduled for a partial hip arthroplasty. She was suffering from malignant rheumatoid arthritis for a long time and we recognized her difficult airway. After epidural catheterization, anesthesia was induced with fentanyl, thiopental, and 5% sevoflurane with oxygen. A laryngeal mask airway (LMA ; size 3) was inserted smoothly on third trial after suxamethonium chloride administration. Anesthesia was maintained with fentanyl, vecuronium bromide, sevoflurane with oxygen and epidural anesthesia. No complications were observed during the operation. After reversal of muscle r

Laryngeal resistance before and after minor surgery: endotracheal tube versus Laryngeal Mask Airway.

BACKGROUND: The placement of an endotracheal tube (ETT) may promote laryngeal swelling, which is an important cause of upper airway obstruction after extubation. The authors hypothesized that laryngeal swelling after ETT placement increases laryngeal resistance and tested that hypothesis by comparing postoperative laryngeal patency between patients with ETT placement and those with a Laryngeal Mask Airway trade mark (LMA). METHODS: Fourteen adult patients who underwent elective minor surgeries were randomly allocated to two groups whose airway would be managed through ETTs (the ETT group) or
