308-nm excimer laser for the treatment of alopecia areata in children.

Alopecia areata (AA) is a common skin disease which is characterized by nonscarring localized or diffused hair loss. In this study we assessed the efficacy of 308-nm Excimer laser in the treatment of alopecia areata in children. A total of 9 children with 30 recalcitrant patches alopecia areata and two children with alopecia areata totalis were enrolled in this study which included seven male and four female patients, aged between 4 and 14 years and the durations of their disease were between 7 and 25 months. All of these patients had more than one lesion of alopecia areata and at least one of

[Excimer laser angioplasty for subclavian artery stenosis--case report].

Endovascular angioplasty for subclavian artery stenosis is effective and less invasive than other methods; however, it is difficult to advance guidewires or balloon catheters through severely stenotic lesions. We report a case of subclavian artery stenosis treated with excimer laser angioplasty. A 58-year-old man with hypertension and hyperlipidemia was admitted to a hospital with different blood pressure in each arm. A CT scan revealed left subclavian artery stenosis, and the patient was transferred to our hospital. An angiogram revealed a severely stenotic lesion in his left subclavian arter

Corneal forward shift after excimer laser keratorefractive surgery.

The excimer laser keratorefractive surgery inevitably compromises structural integrity of the cornea by the surgical tissue subtraction and loss of integrity of Bowman's membrane. Forward shift of the cornea is commonly seen after both photorefractive keratectomy (PRK) and laser in situ keratomileusis (LASIK). Antero-posterior movement of the cornea is evaluated by measuring the posterior corneal elevation with the scanning-slit corneal topography (Orbscan). Eyes with thinner cornea, higher intraocular pressure, and higher myopia requiring greater laser ablation were more predisposed to forwar

Treatment of vitiligo with the 308-nm excimer laser: a pilot study.

BACKGROUND: Present vitiligo therapies require many months of treatment and often result in disappointing outcomes. Common therapeutic options include phototherapy with psoralens plus ultraviolet A (UVA) radiation and broadband or narrowband UVB radiation phototherapy. Some of these modalities require regular phototherapy sessions several times a week for up to a year to achieve a therapeutic response. Targeted phototherapy with single-wavelength laser light is a treatment alternative that may prove to be a time-efficient and effective therapeutic option for the management of vitiligo. METHOD

Intraocular pressure after excimer laser myopic refractive surgery.

The aim of this study was to determine whether intraocular pressure (IOP), as measured by Goldmann applanation or non-contact tonometry, shows systematic changes in patients who have undergone photorefractive keratectomy (PRK) or laser in situ keratomileusis (LASIK). IOP was measured by central Goldmann and non-contact tonometry in 54 patients pre and post-PRK, and in 43 patients pre- and post-LASIK. An interval of 12 months was allowed after surgery. Patients were selected to have one of four specific initial values of refractive error (-2.5, -5.0, -7.5 and -10.0 D). Fellow unoperated eyes we

Mycobacterium chelonae keratitis after excimer laser photorefractive keratectomy.

This is the first report of a severe case of Mycobacterium chelonae keratitis; it occurred in a 26-year-old man after he had undergone excimer laser photorefractive keratectomy for the correction of severe myopia, once the epithelium was already healed. The diagnosis was made by culture results and acid-fast staining of corneal scrapings. Topical ciprofloxacin sodium, 0.3 mg/mL, plus amikacin sodium, 10 mg/mL, and oral clarithromycin sodium led to remission of the ulceration after 3 months of therapy. Subsequent topical corticosteroid therapy led to complete visual recovery during 1 year of fo

Ultrastructural and immunohistochemical findings after linear excimer laser keratectomy.

BACKGROUND: One human pathology specimen has been studied previously following the use of the excimer laser for the correction of astigmatism. We report histopathologic findings following linear corneal excisions with the excimer laser. METHODS: A 193 nm excimer laser was used to create symmetrical, transverse excisions in a human eye to correct astigmatism. Three months later, a full-thickness corneal transplant was performed due to unsatisfactory refractive results. The excised corneal button was examined with light microscopy, transmission electron microscopy and immunohistochemistry. RES

Effect of excimer laser keratectomy on the mechanical performance of the human cornea.

Enucleated human eye globes were set-up for measurements of pressure (2-100 mmHg) induced tangential epithelial and endothelial side corneal strain, and for measurements of central corneal thickness and central radius of curvature. In one experimental group (N = 6), a shallow ablation (15-20 microns deep) removing Bowman's layer was performed in the central 7 mm diameter zone of the cornea. The fellow eye served as control (N = 6). In another experimental group (N = 6) a 70% deep keratectomy was performed in the central 7 mm diameter zone. The pressure induced central corneal strain was 5-10%

Experimental globe rupture after excimer laser photorefractive keratectomy.

OBJECTIVE: To assess corneal weakening after photorefractive keratectomy. METHODS: Photorefractive keratectomy was performed in 16 human eye-bank eyes by means of a 193-nm excimer laser (range, 6 to 54 diopters [D]). Porcine eyes underwent either photorefractive keratectomy procedures (range, 30 to 93 D) with a 5-mm beam diameter or deep 5-mm plano disc ablations. Intraocular pressure was increased gradually with the use of nitrogen gas until the globe ruptured. RESULTS: Human eyes with less than 42 D of correction ruptured at the limbus or behind the insertions of the extraocular muscles. T

Disturbances in night vision after excimer laser photorefractive keratectomy.

Eighty-four patients with up to -6.00 dioptres of myopia underwent photorefractive keratectomy (PRK), using 5.00 mm ablation zones. Three months post-operatively 38 (45%) complained of disturbances in night vision, compared with 21 (25%) pre-operatively. In the majority, these disturbances were regarded as negligible. However, 9 (11%) reported significant problems, defined as an inability to drive safely at night with the treated eye. At 12 months, 32 patients (38%) complained of impaired night vision, 4 (5%) of whom had significant problems. A series of measurements were performed to investig

Use of the excimer laser in high-flow bypass surgery of the brain.

The technique of laser-assisted anastomosis for high-flow bypass surgery using the excimer laser is described in 15 rabbits and in one patient. The left common carotid artery of the rabbits was excised and, with two anastomoses, connected to the right common carotid artery. An end-to-side anastomosis technique was used that obviated the temporary occlusion of the recipient artery. The end of the donor artery was connected for its full circumference with the exterior of the recipient artery and, with the aid of an excimer laser catheter (introduced via an artificial side branch of the donor art

Corneal epithelial changes after excimer laser photorefractive keratectomy.

We used the specular microscope to investigate corneal epithelial changes after excimer laser photorefractive keratectomy. Thirteen patients underwent photorefractive keratectomy on one eye to correct myopia. We observed the central corneal epithelium of each eye at postoperative periods from one to 15 months after photorefractive keratectomy. The fellow eye served as a control. In 11 eyes (85%) the central cornea showed normal epithelial patterns. Two eyes had an abnormal epithelial pattern that included elongated epithelial cells. There were no difference in the mean cell area or coefficient

Acute ultrastructural changes of cornea after excimer laser ablation.

Corneal ultrastructural changes induced by an argon fluoride excimer laser using different parameters were investigated. Twenty-eight rabbit corneas were ablated at light doses per pulse and repetition rates ranging from 25-800 mJ/cm2 and 1-100 Hz, respectively, at four different total light doses (25-150 J/cm2). Transmission electron microscopy showed that corneal ablations done at subthreshold light doses per pulse with repetition rates higher than 30 Hz and with an exposure more than 100 sec caused significant surface coagulation and an increase in pseudomembrane thickness. These changes we

[Superficial ablation of the cornea using the excimer laser (193 nm)].

The final result of corneal surgery such as resection of the pterygium or lamellar keratoplasty is a rough corneal surface. Scanning electron microscopy shows that it is possible to obtain a smoother surface by using the 193 nm excimer laser, as it shows significantly better results than with conventional methods. This procedure was tested on eight patients and documented photographically.
