Localized lepromatous leprosy in household contact of multibacillary disease.

BACKGROUND: Lepromatous leprosy (LL) is a generalized disease, usually with numerous papules, nodules, or plaques containing abundant Mycobacterium leprae and affecting wide areas of the skin. AIM: To report on an LL patient presenting with two lesions on the face and to discuss its epidemiologic significance in the current context of leprosy elimination. METHODS: A 52-year-old housewife presented to our department with erythematous lesions over her face for 2 months. Her husband and son were treated for multibacillary leprosy 15 years previously. An infiltrated erythematous nodule was locat

Reversal reaction and Mitsuda conversion in polar lepromatous leprosy: a case report.

A 22-year-old male polar lepromatous leprosy patient who became Mitsuda positive after 36 months of multidrug therapy (MDT) is reported. Lepromatous leprosy (LL) is a state of specific immunosuppression and is invariably irreversible. The finding of Mitsuda positivity in histopathologically proven polar lepromatous leprosy is extremely uncommon, and conversion of lepromin status following MDT has not so far been reported. This case reports confirms the observations made by Waters et al. regarding lepromin conversion in lepromatous patients.

Primary involvement of penile skin in lepromatous leprosy.

Skin lesions in lepromatous leprosy (LL) are usually multiple and widespread. Though the lesion may occur anywhere on the skin, male genitalia is rarely involved. In all cases reported so far about penile lesions of LL, there were lesions on the other parts of the body also. In some of the cases scrotum was also involved. We report here a patient who presented himself with a single macular lesion of leprosy on the shaft of his penis diagnosed as a case of lepromatous leprosy on slit-skin smear and histopathological examinations.

[A review on Hansen's disease].

Hanseńs disease or leprosy is considered a pre-eradicated condition in Spain, with a prevalence rate of 0.1 cases per 10,000 inhabitants (below 1 case per 10,000 inhabitants, which was the World Health Organizatiońs health goal worldwide for year 2000). The purpose of this review was to study this disease with a particular focus on its clinical aspects (diagnosis, forms, complications, etc.) and drug therapy. Main sources used for this review included Micromedex Healthcare System, PudMed, Cochrane Library, and World Health Organization recommendations. Leprosy is characterized by the existence

Lepromatous leprosy in an HIV-positive patient in Spain.

We report a 35-year-old HIV-positive male affected by lepromatous leprosy which, to our knowledge, is the first case of its kind in Spain. The relationship between the two conditions is discussed.

A case of acute dapsone poisoning: toxicological data and review of the literature.

A case of nonfatal, acute poisoning following the ingestion of an undetermined amount of dapsone (DDS) in a 49-year-old woman is presented. The clinical features were dyspnea and deep cyanosis. Methemoglobinemia was 39.0% on admission. DDS was identified and quantitated in blood samples taken on the third day (sample A) and fifth day (sample B) in the hospital using a high-performance liquid chromatographic technique with diode-array detection. DDS concentrations were 26.99 and 8.40 micrograms/mL in samples A and B, respectively. Results are discussed in the light of an extensive review of the

Chemotherapy of lepromatous leprosy: recent developments and prospects for the future.

Leprosy is a major debilitating infectious disease, primarily of the developing world. In this paper the current status and future prospects of antimicrobial therapy of the severe anergic lepromatous form of the disease are reviewed. Until the last few years only dapsone, rifampicin, clofazimine and ethionamide have had practical application in its therapy, and only rifampicin was bactericidal. Recently, antibiotics from three different classes have been found to be bactericidal in lepromatous patients: a tetracycline (minocycline), a macrolide (clarithromycin), and several fluoroquinolones (i

Chemotherapy of leprosy--current status and future prospects.

The introduction of multi-drug therapy (MDT) by the World Health Organization in 1982 has proved to be the most important advance in the management and control of leprosy since the first use of the sulphone drugs 40 years earlier. For the first time, the number of registered leprosy cases has shown a decline from a peak of 5.37 million in 1985 to 3.1 million in February 1992. The 2 standard MDT regimens have proved simple to apply in most parts of the world, are relatively cheap, generally acceptable, and have shown remarkably few toxic side-effects. Nevertheless, difficulties have arisen in d

Unusual arthritis: be on the alert for leprosy.

A 50-year-old Indonesian man presented with arthritis of the left ankle, wrist and hand joints and a diffusely swollen left hand and foot. A few months later granulomatous skin lesions developed and renewed physical examination revealed a paresis of the intrinsic muscles of the left hand and the left M. extensor hallucis longes and thickening of several peripheral nerves. The skin lesions appeared to be anesthetic. A diagnosis of borderline tuberculoid leprosy was made and treatment resulted in cure with permanent peripheral nerve damage. This case emphasizes the importance of early recognitio

Evaluation of levamisole, an immunopotentiator, in the treatment of lepromatous leprosy.

Twenty subpolar lepromatous leprosy patients under multidrug therapy for a period of 1 to 3 years, who continued to be bacteriologically positive (BI 2 to 1 in Dharmendra's grade) were taken in the study. Ten cases (trial group) were given levamisole as an immuno-stimulator for 1 year along with chemotherapy. Another 10 cases (control group) continued to take chemotherapy alone. There was no conversion of Mitsuda reactivity in any of the cases from both the groups. There was no improvement of leucocyte migration inhibition in either group. In both trial and control groups, statistically signif

Dual arthropathy in previously treated multibacillary leprosy: Inflammatory polyarthritis with neuropathic ankle destruction in an endemic setting.

A 51-year-old woman previously treated for multibacillary leprosy presented with polyarthritis of wrists, ankles and small joints of the hands, resembling rheumatoid arthritis. She had completed multidrug leprosy therapy in 2014 and had a previous history of erythema nodosum leprosum treated with corticosteroids. Slit-skin smear demonstrated acid-fast bacilli and skin biopsy confirmed granulomatous inflammation consistent with leprosy. She was treated with anti-inflammatory therapy and rehabilitation measures, resulting in gradual clinical improvement.

Newly or switching effect of a selective EP2 agonist on intraocular pressure in Japanese patients with open-angle glaucoma.

PURPOSE: To evaluate omidenepag isopropyl (OMDI) for its efficacy in intraocular pressure control (IOP) and adverse reactions following administrations in Japanese patients with open-angle glaucoma (OAG) over a 3-month period. STUDY DESIGN: Retrospective observational study. SUBJECTS AND METHODS: Group 1 included untreated OAG patients, Group 2 included OAG patients treated with prostaglandin F (FP) receptor agonists (monotherapy) and Group 3 included OAG patients treated with multidrug therapy, including FP receptor agonists. OMDI was newly administered in Group 1, and FP receptor agonists

Current and emerging medications for the treatment of leishmaniasis.

INTRODUCTION: Leishmaniasis is a vector-borne neglected tropical disease which manifests as visceral leishmaniasis (VL), cutaneous leishmaniasis (CL), and mucocutaneous leishmaniasis (MCL). The current drugs are toxic, duration of treatment is long, there is regional variation in efficacy, and emergence of resistance is common. AREAS COVERED: This manuscript is based on literature derived from PubMed and reviews the current and emerging medications for the treatment of leishmaniasis. A single dose of liposomal amphotericin B (L-AmB) and multidrug therapy are the best options for VL in the Ind
