CASE HISTORY: Nine of 24 captive kiwi treated with oral levamisole at a dose between 25-43 mg/kg showed signs of respiratory distress. Six died within 4 h of treatment and the remaining three made a full recovery within 24 h.

We investigated inclusion complexation of levamisole (LEV+), a common adulterant of cocaine (COC+), with six molecular baskets 13--618- of increasing cavity depth and anionic charge at the portal. 1H NMR and mass spectrometry showed that 13- to 618- bind to LEV+ to form binary complexes in water, with the stability increasing across the series and highest for dendritic basket 618- (Kd = 10 µM). UNLABELLED: Leukocytoclastic vasculitis is a cutaneous, small-vessel vasculitis. In 50% of cases the aetiology is idiopathic but it can be linked to drugs, infections, autoimmune disorders and various types of cancer. Levamisole is used as an adulterant in cocaine and heroin and has been associated with the development of leukocytoclastic vasculitis. Both cell mediated immunity and humoral immunity was assessed in 16 patients of hydatidiform mole and 6 patients of choriocarcinoma. Fifty percent patients of choriocarcinoma and 11 patients of vesicular mole were given levamisole (LVM) trial and were followed for 2 months. Twenty subpolar lepromatous leprosy patients under multidrug therapy for a period of 1 to 3 years, who continued to be bacteriologically positive (BI 2 to 1 in Dharmendra's grade) were taken in the study. Levamisole has been successfully used therapeutically in twenty-eight cases of chronic cutaneous leishmaniasis. It is suggested that repeated sandfly bites not initiating a clinical lesion may immunize against later infection by the formation of blocking antibodies to Leishmania tropica or by denying the parasite a locus in which it can become established. Late-appearing lesions in natives of endemic areas should raise the suspicion of impaired immunity from systemic disease such as malignancy.

A small pilot study of levamisole in primary biliary cirrhosis failed to demonstrate any improvement in liver function and immunological abnormalities even in 'early' cases. This may be due to the fact that the defect in the immune system lies in its afferent arm. At present, levamisole, which is potentially toxic, cannot be recommended in conventional doses for the treatment of this disease.

Levamisole and D-penicillamine are not skin sensitizers in rats. Furthermore, they do not stimulate T-lymphocyte proliferation in the draining lymph node when injected in oil into the hindpaw or base of the tail. Their copper coordination compounds also do not sensitize rats though they are irritant when injected locally.

Of 60 patients with rheumatoid arthritis treated with levamisole 35% showed a persistent decrease in neutrophil counts from pre-treatment levels and 6 (10%) develped severe neutropenia of less than 1.0 X 10(9)/1. One of these neutropenic patients recoverd without stopping therapy and the other 5 patients recovered rapidly when the drug was withdrawn. In some patients neutropenia recurred on reinstitution of levamisole.
