Internal derangement and the accessory ligaments around the temporomandibular joint.

A model that describes the mechanics of jaw opening and the associated movements of the disc in a healthy human temporomandibular joint is used to explain the abnormal movements associated with internal derangement of the joint. A change in the constraints on jaw opening imposed by the accessory ligaments around the jaw can account for, among other things, clicks on opening and closing, disc displacement and replacement, and the conditions of open and closed lock.

Ligaments of the larynx and the adjacent pharynx and esophagus.

Two ligament systems of the larynx are demonstrated by dissection. The suspensory ligament of the esophagus is attached to the posterior aspect of the cricoid cartilage and is also a part of the fascial sheath which is common to the hyoid, thyroid, and cricoid. The ligaments at the inner margins of the vocal, ventricular, and aryepiglottic folds are distinctive in site and, inferentially, in function. The aryepiglottic ligaments join at the incisura between the arytenoid cartilages and are continued as the corniculopharyngeal ligament which splays into the flexible tissues in the anterior wall…

Magnetic resonance imaging of injury to the lateral ankle ligaments.

To establish the value of magnetic resonance imaging in determining which patients with ankle sprains will benefit from surgical treatment, 1 uninjured volunteer and 15 patients with acute, subacute, and chronic injuries of the lateral ankle ligaments were imaged at 1.0 tesla using a fast imaging with steady-state precision three-dimensional technique and 1.5-mm slice thickness. A dedicated knee coil was used to hold the foot in a neutral or plantar-flexed position. In cases of acute, low-grade injuries, fraying of the anterior talofibular ligaments with intact calcaneofibular ligaments was ob…

Primary orthotic treatment of ruptured ankle ligaments: a recommended procedure.

The results of a study after 1 and 2 years of a prospective randomised trial of operative versus conservative treatment of ankle ligament rupture, demonstrate that purely functional orthotic therapy is the method of choice. This relates both to patient need and economical considerations. The trial demonstrated that without an operation it was possible to achieve a high degree of mechanical stability, a reduction of work disability time down to 3 weeks and full sports capability within 3 months. Consequently, and as a result of the trial, the only remaining surgical indications would seem to be

Use of carbon fibers in the reconstruction of knee ligaments.

Carbon fibers were used to reconstruct the anterior cruciate ligament and other knee ligaments in a consecutive series of 26 (mostly acute) patients who had suffered serious occupation-related injuries. After an average follow-up time of 52.4 months (one patient lost, one patient refused follow-up examination), the anterior drawer at 30 degrees was less than 5 mm (side-to-side difference) in 14 patients, 5-10 mm in six patients, and greater than 10 mm in four patients. Of 24 evaluatable patients, 22 returned to work, including 20 patients who work underground. Chronic pain, recurrent effusion,…

Steroid hormone receptors in pelvic muscles and ligaments in women.

Using monoclonal antibody assay techniques estrogen receptors (ER) and progesterone receptors (PgR) were quantitated in female pelvic floor muscles, urogenital ligaments and uterus (myometrium). Receptors were detected in all these structures. In addition qualitative assessment showed distinct nuclear staining in the same structures. ER and PgR were not found in the musculus rectus abdominis with either method. The findings provide a rationale for estrogen treatment in women with urogenital disorders.

Biomechanics of the craniocervical region: the alar and transverse ligaments.

In the treatment of spine fractures and fracture-dislocations, stability of the spine is one of the major objectives. In the craniocervical joint, the alar and transverse ligaments provide much of the stability of the healthy spine. Because the anatomy appears well described, the contribution of each of these structures so far has received little attention. The alar ligament restrains rotation of the upper cervical spine, whereas the transverse ligament restricts flexion as well as anterior displacement of the atlas. A lesion in one or both structures can produce damage to the neural structure…

CT of the main ligaments of the cervico-occipital hinge.

The results of a CT-anatomical correlative study of the main ligaments of the cervico-occipital hinge are reported. CT criteria of normal ligamentous structure of this region are presented in axial, coronal and sagittal views with a special attention to the transverse ligament of the Atlas. Examples of pathological transverse ligaments are illustrated and emphasis is laid upon the better statement CT allows in such cases.

Historical perspectives on injuries of the ligaments of the ankle.

As is readily apparent, there is not a consensus regarding the treatment of lateral ligamentous ruptures of the ankle. Certainly, the elimination of functional instability is the goal of treatment. What actually causes this is still open to debate, but from Harrington's work, it appears that degenerative changes can result from mechanical instability. We are all aware that many athletes compete actively with "two-plane" instability of the ankle, but it remains to be seen if they will also suffer the late degenerative changes that Harrington describes. In the meantime, each physician must decid

Ruptures of the lateral ligaments of the ankle. Clinical diagnosis.

During the 11-month period 1 October 1977 to 31 August 1978 a total of 44 patients with acute supination trauma of the ankle were examined. The clinical findings were compared with the results of arthrography. This revealed that direct and indirect tenderness of the anterior talofibular ligament and calcaneofibular ligament respectively, combined with a greater than or equal to 4 cm swelling anteriorly and over the lateral malleolus, indicated a ligament injury with great likelihood. If some of the diagnostic signs are absent, most emphasis should be laid on the swelling over the lateral malle

The name cranial ovarian suspensory ligaments in mammalian anatomy should be used only to indicate the structures derived from the foetal cranial mesonephric and gonadal ligaments.

The term ovarian suspensory ligament appears ambiguous when human adult anatomy textbooks are compared with human embryology or with general mammalian anatomy textbooks. The term ovarian suspensory ligament in laboratory rodents and domestic animals indicates homologous structures during foetal (the cranial mesonephric and gonadal ligaments) and later life (the cranial mesonephric ligament derivatives). In human foetal anatomy textbooks ovarian suspensory ligament is generally applied to this same ligament. However, in human adult anatomy textbooks ovarian suspensory ligament is widely applied…

Physiological Load-Response in Synthetic Ligaments through Active Filament-Mixing Fabrication.

PURPOSE: Additive manufacturing has enhanced medical trainers and their functionality, but synthetic tissues that are both anatomically accurate and capable of replicating musculoskeletal load responses remain largely unexplored. The purpose of this study is to demonstrate fabrication of synthetic tissue using active filament-mixing fabrication to enable local composition control with thermoplastic compositions tuned to match the modulus of elasticity of the respective tissues and functionally graded transitions between ligament and bone. METHODS: Synthetic femoral anterior cruciate ligament

Characterization of middle ear ossicular ligaments using photon counting detector CT.

BACKGROUND AND PURPOSE: The middle ear suspensory ossicular ligaments have been previously described on traditional energy integrating detector CT scanners. With recent developments in photon counting detector CT, these ligaments can be better visualized due to greater spatial and contrast resolution. MATERIALS AND METHODS: 50 patients (100 temporalbones) without hearing loss or middle ear cavity pathology were retrospectively reviewed to characterize the middle ear suspensory ossicular ligaments including the anterior, superior, lateral mallear, and posterior incudal ligaments, and stapes an…

The different subtalar ligaments show significant differences in their mechanical properties.

BACKGROUND: Today, the relative contribution of each ligamentous structure in the stability of the subtalar joint is still unclear. The purp … ficant differences in their intrinsic mechanical properties. Both the CFL and CL are more compliant ligaments and seem to be involved in the development of subtalar instability. Based on the material properties, a gracilis tendon graft seems more appropriate than a synthetic ligament to reconstruct a CL or CFL. A partial rupture was the most commonly seen i …

A bioinspired robotic knee with controlled joint surfaces and adjustable ligaments.

The knee joint plays a key role in kinematic and kinetic performances of pedestrain locomotion. The key role of meniscus with matched ligaments in joint stability and movability has not been fully explored in current robotic knee designs. We fabricate a bioinspired robotic knee based on a kinematic model of an anatomical knee in order to reveal the relationship between the meniscus, ligaments and their stability and movability, respectively. The kinematic model was built from magnetic resonance imaging of the human knee with generated contact profiles and customized ligament fibers. Then, the …
