[Duodenal obstruction by biliary lithiasis (Bouveret's syndrome)].

The case of an 81-year-old woman admitted for abdominal pain, nausea and vomiting diagnosed with Bouveret syndrome is presented. This rare disease, consisting in obstruction of gastric emptying by gallstones located in the pylorus or the duodenal bulb predominates in elderly women and represents 2% of the cases of biliary ileum. Diagnosis is based on the clinical manifestations, the existence of aerobilia, visualization of lithiasis by radiography, echography or fiber gastroscopy and the demonstration of duodenal obstruction. The treatment of choice is surgery consisting in gastrotomy or enter…

[Lithiasis and ectopic pelvic kidney. Therapeutic aspects].

Kidney in ectopic position is dysplasic, and associated to other malformations. The advent of a lithiasis in these conditions rises questions about therapeutic options. We report on five observations of pelvic ectopic kidney with urinary lithiasis. Patients were aged from 16 to 42 years. Kidney was non functional in two cases, or with normal appearance sized 10 to 12 cm. We performed total nephrectomy in two cases, pyelolithotomy in the other cases. Surgical approach was subperitoneal via iliac route. A dismembered pyeloplasty was associated in one case. All patients did well. Radiologic contr…

[Treatment of ureteral lithiasis using rigid ureteroscopy].

The authors report 46 cases of rigid ureteroscopy for ureteral stone removal. Ureteroscopy procedures for extraction of lithiasis are the next-ones: stone forceps, stone basket, ultrasonic lithotripsy and electrohydraulic lithotripsy. Manipulation of calculi is done under direct vision. A success rate of ureteroscopy was of 89% (41 patients). All of the patients with stone removal failure underwent immediate operative intervention (open ureterolithotomy in 4 patients and ureteral reimplantation in 1). The best results were obtained for distal calculi (pelvic lithiasis): success rate of 91.4%. …

Accessory gallbladder lithiasis presenting as acute cholangitis.

Accessory gallbladder is an uncommon congenital anomaly that may present significant diagnostic and surgical challenges, particularly when associated with biliary complications. We report a case presenting features of common bile duct obstruction and cholangitis. Multimodality imaging, including computed tomography and magnetic resonance cholangiopancreatography, enabled preoperative identification of an accessory gallbladder. Magnetic resonance cholangiopancreatography was especially useful in differentiating this anomaly from other cystic biliary lesions and in accurately delineating the bil

Evolutionary conformation model of salivary gland lithiasis.

INTRODUCTION: Salivary stones, or sialoliths, are calcified concretions forming within salivary glands and their ducts through a two-stage process: an initial formation of a central core via precipitation of inorganic material mediated by organic substances, followed by layering of additional organic and inorganic material. Substrates for sialolith formation include mucoid agglomerates, organic vesicles, foreign bodies, and bacterial biofilms. Understanding the detailed structure of sialoliths may aid in developing specific preventive or therapeutic strategies. MATERIALS AND METHODS: This stu

Laparoscopic T-tube choledochotomy for biliary lithiasis.

T-tube choledochotomy has been an established practice in common bile duct exploration for many years. Although bile leaks, biliary peritoni … paroscopic and endoscopic techniques, which provide alternative therapeutic approaches to choledocholithiasis and allow the surgeon to avoid having to perform a choledochotomy with T-tube drainage.

Bladder lithiasis: from open surgery to lithotripsy.

Bladder calculi account for 5% of urinary calculi and usually occur because of bladder outlet obstruction, neurogenic voiding dysfunction, infection, or foreign bodies. Children remain at high risk for developing bladder lithiasis in endemic areas. Males with prostate disease or relevant surgery and women who undergo anti-incontinence surgery are at a higher risk for developing vesical lithiasis. Open surgery remains the main treatment of bladder calculus in children. In adults, the classical treatment for bladder calculi is endoscopic transurethral disintegration with mechanical cystolithotri…

[Vesico-urethral lithiasis secondary to foreign body].

We report a case of vesico-urethral litiasis secondary to foreign body (gauze) calcificated, left in prostatic cell during prostate open surgery of performed twelve years ago. In the related literature we observe that most of foreign urinals bodies are secondary to the introduction through the urethra of objects with autoerotic finality in normal patients or more often with psychiatric disorders, and only a small proportion are of iatrogenic origin, mainly in the last ten years.

Biliary lithiasis: combined endoscopic and surgical treatment.

A combined endoscopic and surgical procedure was performed in 69 patients suffering from gallbladder lithiasis with suspected associated lithiasis of the common bile duct (CBD). The procedure was spread over two days: Day 1 - Endoscopic retrograde cholangiopancreatography (ERCP) revealed choledocholithiasis in 50 cases and papillary sclerosis in 4 cases. In 15 patients, the common bile duct was found to be normal. Fifty-four endoscopic sphincterotomies were performed. Among the 50 cases of choledocholithiasis, endoscopic removal of the stones was judged complete in 44 cases and incomplete in t…

A Case Report of Inguinal Hernia Sac Lithiasis.

We report a case of a stone in inguinal hernia sac. A 41-year-old male patient presented to Hakim Hospital in Neyshabur city, Iran, with complaints of pain and swelling in the right abdominal region. After initial evaluations, a diagnosis of right inguinal hernia was made, and the patient was scheduled for hernioplasty. He had no history of previous surgeries, hospital admissions, underlying diseases, or kidney and gallbladder stones. The surgery was performed. The hernia sac was exposed. Inside the hernia sac, a stone measuring approximately 2 cm in diameter, with a hard consistency and yello

Chronic pancreatitis and pancreatic lithiasis. State of the art.

Chronic pancreatitis and pancreatic lithiasis, isolated or associated, are infrequent diseases in western countries. They are linked to alcohol abuse, cigarette smoking, repeated episodes of acute pancreatitis and hereditary genetic factors. They are characterised by persistent or recurrent epigastric pain, digestive insufficiency, steatorrhoea, weight loss and secondary diabetes. They are easily diagnosed with CT and MRI ultrasound, but are difficult to treat. Medical therapy is symptomatic for diabetes and digestive failure. Invasive treatment is indicated only for pain that cannot be treate…

Morphological and crystal chemical characteristic of panсreatic lithiasis.

OBJECTIVE: Introduction: Information on chemical and phase composition of pancreoliths is limited and discrepant. There are reports, that pa … n the ductal system of pancreas. Concretion sizes ranged from 0.5 to 1.5 cm in diameter. Pancreatic lithiasis' form depended on the location: in the duct of Wirsung single concretions were found (they were relatively large, oval stones with smooth, regular edges); multiple, small concretions with irregular edges, coral-like stones dominated in the ductal sy …

Application of mineralogical techniques in the study of human lithiasis.

The authors review the mineralogical methods and techniques of analyzing calculi, stony concretions in the body. They discuss the main types of kidney stones (prostate, testicular, salivary, and bile) and the different diagnostic methods in mineralogy. By applying the techniques of optical microscopy and electron microscopy, they describe the different characteristics of human stones, based on extensive experience as evidenced by their numerous studies.
