Static lung mechanics in patients of progressive systemic sclerosis without obvious pulmonary involvement.

Pulmonary fibrosis is a common complication in progressive systemic sclerosis (PSS). Physical examination and/or routine pulmonary function tests can, however, detect only relatively advanced disease. We assessed the utility of static lung pressure-volume analysis in detecting lung involvement in patients of PSS without obvious clinical, radiological or spirographic evidence of pulmonary disease. Static lung volumes were determined and expiratory lung pressure-volume measurements were obtained in 10 patients with PSS, using a computerized whole body plethysmograph. Static pressure-volume data

Assessment of the fetal lungs in utero.

Antenatal diagnosis of abnormal pulmonary development has improved significantly over recent years because of progress in imaging techniques. Two-dimensional ultrasound is the mainstay of investigation of pulmonary pathology during pregnancy, providing good prognostication in conditions such as congenital diaphragmatic hernia; however, it is less validated in other high-risk groups such as those with congenital pulmonary airway malformation or preterm premature rupture of membranes. Three-dimensional assessment of lung volume and size is now possible using ultrasound or magnetic resonance imag

Respiratory Function in Healthy Taiwanese Infants: Tidal Breathing Analysis, Passive Mechanics, and Tidal Forced Expiration.

BACKGROUND: Although infant lung function (ILF) testing is widely practiced in developed Western countries it is not typically performed in Eastern countries, and lung measurements are scarce for Asian infants. Therefore, this study aimed to establish normal reference values for Taiwanese infants. MATERIALS AND METHODS: Full-term infants without any chronic diseases and major anomalies were enrolled in the Prediction of Allergies in Taiwanese Children (PATCH) cohort study. Detailed medical data, such as body weight and length, birth history, and histories of previous illness and hospitalizati

Evaluation of Quantitative Computed Tomography Indices in Patients with Pneumonia and Acute Respiratory Failure in the Intensive Care Unit (ICU).

Background: In this study, we aimed to explore the relationship between quantitative indices derived from computed tomography (CT) attenuation histograms and disease prognosis in patients with pneumonia and acute respiratory failure. We also sought to assess the effectiveness of these parameters as clinical prognostic markers. Methods: CT images of patients with pneumonia and acute respiratory failure were analyzed using Vitrea® Advanced Visualization software. The analyzed quantitative CT (qCT) indices included mean lung Hounsfield unit (HU) and density-based volume measurements, specifically

The effect of pectus excavatum deformity on lung volume: fact or myth?

BACKGROUND AND PURPOSE: Most of the previous studies evaluating lung volume of pectus excavatum (PE) patients were based on spirometric measurements. We aimed to calculate lung volume of patients with PE and compare them with lung volume of patients without chest wall deformity using CT volumetry. METHODS: After institutional review board approval, preoperative chest CT of PE patients who underwent minimal invasive procedure between January 2012 and February 2018, were evaluated retrospectively. As a control group, age and sex matched patients who underwent chest CT scan in the same period we

Mathematical reappraisal of the congenital cystic adenomatoid malformation volume ratio.

OBJECTIVE: The CCAM (CPAM) volume ratio (CVR), an ultrasound index to predict hydrops or neonatal demise in fetuses affected with CPAM, is presumed to be independent of gestational age. A CVR of 1.6 has been proposed as predictive of hydrops. The purpose of our study was to assess the fundamental mathematical concept of the CVR. STUDY DESIGN: Using published formulas, the right fetal lung volume was used as a surrogate of a theoretical CPAM involving the entire right lung. The head circumference was estimated using the previously published formula by Chitty et al. The ratio of right lung volu

Compensatory Lung Growth after Pneumonectomy: Case Report.

Lung expansion after pneumonectomy is well documented in animal models like dogs or mice. We present a lung expansion case after left pneumonectomy for an advanced carcinoid tumor in a 16-year-old patient with hemoptysis and acute dyspnea. This tumor was localized in the left upper lobe (LUL) invading the left main bronchus. During the follow-up we noticed a real growth of the remaining lung.

Familial lymphoid interstitial pneumonia: a long-term follow-up.

We describe two brothers who developed chronic pulmonary disease in early childhood. Lung biopsies were diagnostic of lymphoid interstitial pneumonia (LIP). Familial LIP has not been previously reported, and the natural history is unknown. The elder brother experienced progressive respiratory disability and died 10 years after the onset of symptoms. The younger brother age 13, has been observed for 11 years and despite progression of pathological changes revealed in his second lung biopsy, he has had few symptoms and leads an active life. Current pulmonary function tests reveal decreased lung

Oscillometry for personalizing continuous distending pressure maneuvers: an observational study in extremely preterm infants.

RATIONALE: Lung recruitment and continuous distending pressure (CDP) titration are critical for assuring the efficacy of high-frequency ventilation (HFOV) in preterm infants. The limitation of oxygenation (peripheral oxygen saturation, SpO2) in optimizing CDP calls for evaluating other non-invasive bedside measurements. Respiratory reactance (Xrs) at 10 Hz measured by oscillometry reflects lung volume recruitment and tissue strain. In particular, lung volume recruitment and decreased tissue strain result in increased Xrs values. OBJECTIVES: In extremely preterm infants treated with HFOV as fi

[The Successful Reduction of Resected Lung Volume for Intralobar Pulmonary Sequestration with Infrared Thoracoscope].

A 59-year-old male patient was referred to our hospital for further examinations and treatment due to an abnormal shadow detected in his left lower lung lobe on computed tomography. The patient was diagnosed with intralobar pulmonary sequestration and scheduled for an operation. During the surgery, after resection of the aberrant artery, indocyanine green was intravenously injected, and the border between normal lung and sequestrated lung was clearly identified by an infrared thoracoscope. Subsequently, wedge resection was performed, and the patient was discharged on postoperative day 5. Spiro

Novel Surface Topographic Assessment of Lung Volume and Pulmonary Function Tests in Idiopathic Scoliosis: A Preliminary Study.

OBJECTIVE: Severe spinal deformity results in restrictive pulmonary disease from thoracic distortions and lung-volume limitations. Though spirometry and body plethysmography are widely accepted tests for pulmonary function tests (PFTs), they are time-consuming and require patient compliance. This study investigates whether surface topographic [surface topography (ST)] measurements of body volume difference (BVD) and torso volume difference between maximum inhale and exhale correlate to values determined on PFTs. METHODS: This study included patients with idiopathic scoliosis and thoracic/thor

[Mechanical ventilation in acute respiratory insufficiency in patients with chronic obstructive lung diseases].

Chronic obstructive lung disease (COLD) is a common disease and cause of death. At an advanced stage, acute respiratory failure will repeatedly arise and mechanical ventilation may be the only solution. The best result of treatment is a return to the habitual condition. In this article a survey is presented regarding the course and survival following mechanical ventilation of acute exacerbation of COLD. Survival rate after mechanical ventilation was 50-70% compared to a survival rate of 75-95% in patients not mechanically ventilated. Mechanical ventilation influenced factors were taken into co

Body carbon dioxide storage capacity in exercise.

Body CO2 storage capacity was measured in nine subjects at two levels of exercise, approximating 30 and 60% VO2max, by measuring the excess CO2 output associated with hyperventilation at constant end-tidal PCO2, and the change in mixed venous PCO2 (PVCO2) measured by rebreathing. CO2 output was measured during 20-s periods and monitored throughout the procedure; hyperventilation was continued until CO2 output had returned to control values. Washout of CO2 was more rapid than previously found at rest, 90% of the change following an increase in ventilation occurring within 4 min. CO2 storage cap

Pulmonary function after recovery from the adult respiratory distress syndrome.

Eight adult patients with acute respiratory distress syndrome were treated with positive end-expiratory pressure (PEEP) ventilation. The results of detailed examinations of pulmonary function in these eight patients obtained after different periods of time following discharge from the respiratory care unit were analyzed to determine the degree of disturbances in pulmonary function. For comparison, examinations of pulmonary function were also performed on two patients who were treated with zero end-expiratory pressure ventilation. This follow-up study showed remarkably few abnormalities. Whe pr

Validation of a nitrogen washout system to measure functional residual capacity in premature infants with hyaline membrane disease.

A multiple-breath nitrogen washout system designed to measure lung volume in mechanically ventilated infants was validated by assessing three performance criteria: 1) accuracy of lung volume measurements in the presence of an endotracheal tube leak was assessed by comparing the measurements of functional residual capacity (FRC) in a mechanical lung model with and without airway leak; 2) in vivo accuracy was assessed in rabbits by comparing FRC measurements obtained by this system with measurements obtained by helium dilution; and 3) in vivo precision was assessed by analyzing measurements of F

Respiratory effects of obesity on the ventilatory response to exercise in older adults: the impact of a shift in operational lung volumes.

We examined the exercise ventilatory response to heavy exercise (ventilation [V̇E] and carbon dioxide elimination [V̇CO2] relationship from ventilatory threshold to peak exercise) and operational lung volumes (end expiratory [EELV] and end inspiratory [EILV]) in older adults with and without obesity. Forty-nine older adults with (BMI: 33.9 ± 5.0 kg/m2, age: 70 ± 2 y) and without obesity (BMI: 24.6 ± 3.0 kg/m2, age:71 ± 3 y) were evaluated, and performed a maximal incremental cycling test. The V̇E/V̇CO2 slope was lower in those with (43.1 ± 8.0) vs. without obesity (51.4 ± 11.1). While EELV was

Correlation of fetal lung area with MRI derived pulmonary volume.

BACKGROUND: Neonatal chest-Xray (CXR)s are commonly performed as a first line investigation for the evaluation of respiratory complications. Although lung area derived from CXRs correlates well with functional assessments of the neonatal lung, it is not currently utilised in clinical practice, partly due to the lack of reference ranges for CXR-derived lung area in healthy neonates. Advanced MR techniques now enable direct evaluation of both fetal pulmonary volume and area. This study therefore aims to generate reference ranges for pulmonary volume and area in uncomplicated pregnancies, evaluat
