Self-help for bulimia nervosa: a preliminary report.

Eighteen patients with DSM-III-R bulimia nervosa were treated by providing them with supervision in the use of a self-help manual based on the cognitive behavioral treatment for the disorder. The patients were assessed before beginning the self-help program, and again 4 to 6 months later, using standardized measures of psychopathology. The findings were encouraging. At follow-up one half of the patients had ceased bulimic episodes and self-induced vomiting, and most of the remainder had made significant improvements. These preliminary findings, together with related published work, suggest tha

Secondary affective disorder in women with somatization disorder.

Of 60 women with somatization disorder (SD), 54 (90%) met DSM-III criteria for a major affective episode. This group with secondary affective disorder (SAD) was compared with 29 women who had primary affective disorder (PAD). SAD patients reported significantly more symptoms, more psychiatric admissions, more and longer episodes of depression; more had attempted suicide, and more had made multiple attempts. SAD patients had often been hospitalized and were likely to have been treated with lithium and a variety of antidepressants. Studying relatively homogeneous primary psychiatric diagnoses fa

Diagnostic considerations in panic disorders.

Approaches to the classification and treatment of anxiety have been lacking because of the unclear borders between pathologic and normal anxiety and the heterogeneity of patients called "anxious." The differentiation of panic from other anxiety states is a recent development in psychiatric nosology. The diagnosis of panic disorder and agoraphobia and their demarcation from other psychiatric and medical disorders are reviewed. Proposals to revise DSM-III criteria to provide concordance with current etiologic theories are discussed, and an overall treatment strategy that takes into account the m

The concept of 'bouffée délirante' with special reference to the Scandinavian concept of reactive psychosis.

The nosological concepts of 'bouffée délirante', first described by Magnan in the 1880s, and of 'psychogenic psychosis', first described by Wimmer in 1916, are still in use in their countries of origin, France and the Scandinavian countries. Both refer to psychotic episodes of good prognosis, not connected with schizophrenia, appearing in a special fragile personality. An examination of the diagnostic criteria of the two categories shows, however, that important differences exist. Contrary to what has been claimed, Wimmer's original description, although showing an encyclopedic knowledge of th

Evaluation and management of common anorectal conditions.

The prevalence of benign anorectal conditions in the primary care setting is high, although evidence of effective therapy is often lacking. In addition to recognizing common benign anorectal disorders, physicians must maintain a high index of suspicion for inflammatory and malignant disorders. Patients with red flags such as increased age, family history, persistent anorectal bleeding despite treatment, weight loss, or iron deficiency anemia should undergo colonoscopy. Pruritus ani, or perianal itching, is managed by treating the underlying cause, ensuring proper hygiene, and providing symptom

The efficacy and safety of acids as topical antimicrobial agents: a review.

OBJECTIVE: Infected wounds, refractory to conventional therapy, are a major burden on the healthcare system. Available data show that many commonly used antiseptic agents may be toxic to the cells involved in the healing process and may affect the normal tissue repair. The topical use of different acids to control wound infections effectively and promote healing is well known. The present review aims to summarise the safety and efficacy of various acids as topical agents for treating wound infections. METHOD: A literature search was performed in PubMed and manually from other sources (cross r

[Acquired clitoral phimosis causing a periclitoral pseudocyst].

Clitoral phimosis or preputial fusion may occur as a result of atrophic vaginitis among other conditions. A 72-year-old woman presented with atrophic vaginitis, preputial fusion, and a painful periclitorial pseudocyst. We suggest that a minimal surgery approach in a manual retraction of the synarchies without making an incision is a gentle and effective surgical management of preputial fusion. Suturing the ends from each other combined with continuous topical prophylaxis will minimise the risk of recurrence of pseudo-cyst and prevent worsening scar tissue formation.

Access to treatment protocols and manuals for evidence-based psychological interventions for severe mental disorders: a survey of randomised trials included in network meta-analyses.

BACKGROUND: Evidence-based psychological interventions for mental disorders are described in treatment protocols and manuals, which detail treatment components and conditions of application. Systematic evaluations of the accessibility of treatment protocols and manuals across multiple mental disorders are absent. OBJECTIVE: We assessed whether treatment protocols or manuals for psychological interventions for severe mental disorders are accessible and publicly available. STUDY SELECTION AND ANALYSIS: We surveyed randomised controlled trials (RCTs) from six large network meta-analyses of psyc

Public Discourse, User Reactions, and Conspiracy Theories on the X Platform About HIV Vaccines: Data Mining and Content Analysis.

BACKGROUND: The initiation of clinical trials for messenger RNA (mRNA) HIV vaccines in early 2022 revived public discussion on HIV vaccines after 3 decades of unsuccessful research. These trials followed the success of mRNA technology in COVID-19 vaccines but unfolded amid intense vaccine debates during the COVID-19 pandemic. It is crucial to gain insights into public discourse and reactions about potential new vaccines, and social media platforms such as X (formerly known as Twitter) provide important channels. OBJECTIVE: Drawing from infodemiology and infoveillance research, this study inve

Lidocaine Toxicity During Attempted Epistaxis Cautery.

BACKGROUND: Epistaxis is a common problem that occurs in up to 60% of the general population, and is a common emergency department (ED) complaint. The use of lidocaine for analgesia is common when cauterization is required for bleeds that are refractory to manual compression. Although the use of lidocaine is generally thought of as a benign intervention, it is not completely without risk. CASE REPORT: We present the case of a 19-year-old man who presented to the ED with persistent anterior epistaxis. He developed severe lidocaine toxicity resulting from topical anesthesia applied prior to int

National clinical guidelines for non-surgical treatment of patients with recent onset neck pain or cervical radiculopathy.

PURPOSE: To summarise recommendations about 21 selected non-surgical interventions for recent onset (<12 weeks) non-specific neck pain (NP) and cervical radiculopathy (CR) based on two guidelines from the Danish Health Authority. METHODS: Two multidisciplinary working groups formulated recommendations based on the GRADE approach. RESULTS: Twelve recommendations were based on evidence and nine on consensus. Management should include information about prognosis, warning signs, and advise to remain active. For treatment, guidelines suggest different types of supervised exercise and manual thera

Injury prevention in physiotherapists--a scientific review.

This review targeted all research previously conducted on the topic of musculoskeletal disorders and injury among physiotherapists, with a particular focus on studies that had examined individual, physical and psychosocial risk factors and provided suggestions or recommendations to prevent such injuries. Scientific literature published in English languages was searched using electronic way. A total of 17 appropriate studies were located and examined, most of which had focused on the prevalence of musculoskeletal disorders/pan or any other injury. From the review, it was shown that major risk f

Oral myiasis in a maxillofacial trauma patient.

Myiasis is a rare disease primarily caused by the invasion of tissue by larvae of certain dipteran flies. Oral myiasis is still more "rare" and "unique" owing to the fact that oral cavity rarely provides the necessary habitat conducive for a larval lifecycle. Common predisposing factors are poor oral hygiene, halitosis, trauma, senility, learning disabilities, physically and mentally challenged conditions. Oral myiasis can lead to rapid tissue destruction and disfigurement and requires immediate treatment. Treatment consists of manual removal of maggots from the oral cavity after application o

Manual lymphatic drainage for lymphedema in patients after breast cancer surgery: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Studies have shown that manual lymphatic drainage (MLD) has a beneficial effect on lymphedema related to breast cancer surgery. However, whether MLD reduces the risk of lymphedema is still debated. The purpose of this systematic review and meta-analysis was to summarize the current evidence to assess the effectiveness of MLD in preventing and treating lymphedema in patients after breast cancer surgery. METHODS: From inception to May 2019, PubMed, EMBASE, and Cochrane Library databases were systematically searched without language restriction. We included randomized controlled tria

Differentiating the subtypes of social anxiety disorder.

Since the inclusion of subtypes of social anxiety disorder (SAD) in the DSM-III-R, the most studied have been generalized versus specific subtypes. Previous research indicated that the generalized subtype was associated with greater severity, comorbidity and functional impairment compared to the specific subtype, but more recent evidence supports a dimensional conceptualization of SAD. Earlier studies also possessed limitations, such as heterogeneity in definitions of generalized SAD. Based on the more recent findings and the limitations of the earlier studies, the DSM-5 eliminated the general

The relevance of queries and coding procedures to the writing of cause-of-death statements.

A cause-of-death statement may result in a query by the vital records registrar to clarify cause of death information or to educate the certifier of death in recommended death certification procedures. The level of querying depends on local need and resources, and is usually conducted at one of six query priority levels. When applying International Classification of Disease (ICD) coding to cause-of-death information, nosologists use a General Rule and 12 additional rules, as needed, to identify an underlying cause of death for statistical purposes. Incorrectly or poorly written cause-of-death

Toward an assessment of the construct validity of four measures of narcissism.

In recent years there has been growing conceptual interest in narcissism, coupled with the rapid development of several paper and pencil measures. Clearly, construct validation should be performed prior to the adoption of any measure. This study reports on the construct validation of four scales that purport to measure narcissism. A conceptual definition of narcissism is provided, and a number of empirical analyses are performed, including examination of internal consistency reliabilities, intercorrelations among the four narcissism scales, and correlations with related and unrelated measures.

The development of the concept of reactive psychoses.

Although reactive (psychogenic) psychoses have no doubt occurred throughout the history of mankind, it is only during the last century that the corresponding concept has been defined, beginning with Sommer's definition of the term 'psychogenic' and Jaspers' distinctions between 'reaction' and 'process', and between 'neurosis' and 'psychosis', respectively. A change in Kurt Schneider's nosological concepts induced the majority of German-speaking psychiatrists to avoid the concept of psychogenic psychoses. The very varying use of the concept of reactive psychoses in different countries is descri

Pediatricians' reported practices in the assessment and treatment of attention deficit disorders.

This study examined, in a national survey, the assessment and treatment practices of pediatricians who care for children with attention deficit disorders (ADD). A questionnaire was sent to randomly selected members of the American Academy of Pediatrics (AAP), stratified by state. Of 417 (52%) responses returned, there were 290 (38%) completed questionnaires. It was found that most pediatricians did not utilize specific DSM-III criteria for ADD; however, they do rely on symptoms of distractibility, overactivity, and impulsivity, which are the behaviors central to the DSM-III diagnosis. In addit

Alcoholism followed by schizophrenia.

On the basis of findings of earlier workers, and our own unsystematic observations, we put forward the hypothesis that alcohol abuse was an important etiological factor in some schizophrenia-like psychoses. We tested the hypothesis by comparing schizophrenic patients who had abused alcohol with schizophrenic patients who had not. The findings generally support the hypothesis, and indicate that a chronic psychosis distinguishable by a hallucinatory onset and, at its height, by visual hallucinations, may be one of the complications of alcohol abuse.
