Community-based approaches to address childhood undernutrition and obesity in developing countries.

Community-based approaches have been the mainstay of interventions to address the problem of child malnutrition in developing societies. Many programs have been in operation in several countries for decades and originated largely as social welfare, food security and poverty eradication programs. Increasingly conceptual frameworks to guide this activity have been developed as our understanding of the complex nature of the determinants of undernutrition improves. Alongside this evolution, is the accumulation of evidence on the types of interventions in the community that are effective, practical

Reducing infant mortality rates using the perinatal periods of risk model.

Despite decreases in the last 50 years, infant mortality rates in the United States remain higher than in other industrialized countries. Using overall infant mortality rates to determine the effectiveness of interventions does not help communities focus on particular underlying factors contributing to static, and sometimes increasing, community rates. This study was designed to determine and rank contributing factors to fetal-infant mortality in a specific community using the Perinatal Periods of Risk (PPOR) model. The PPOR model was used to map fetal-infant mortality for 1995 to 1998 in the

Severe and acute maternal morbidity: Lithuanian experience and review.

OBJECTIVE: To measure the incidence of severe acute maternal morbidity (SAMM) and to find the risk factors that could be useful in trying to predict the development of SAMM. METHODS: The data were collected prospectively in 7 maternity units from different Lithuanian regions over a 1-year period. Every woman with a clinical diagnosis of severe preeclampsia, eclampsia, Hemolysis, Elevated Liver enzymes, and Low Platelet count (HELLP) syndrome, severe hemorrhage, uterine rupture, or severe sepsis during pregnancy or delivery or until the 42nd day post partum was included after verification of d

Animal welfare implications of neonatal mortality and morbidity in farm animals.

Much has been learnt during the last 50 years about the causes of neonatal mortality and morbidity and about practical means for minimising them in newborn lambs, kids, bovine calves, deer calves, foals and piglets. The major causes of problems in these newborns are outlined briefly and include hypothermia due to excessive heat loss or to hypoxia-induced, starvation-induced or other forms of inhibited heat production. They also include maternal undernutrition, mismothering, infection and injury. The published literature reveals that the scientific investigations which clarified these causes an

The global impact of pre-eclampsia and eclampsia.

Over half a million women die each year from pregnancy related causes, 99% in low and middle income countries. In many low income countries, complications of pregnancy and childbirth are the leading cause of death amongst women of reproductive years. The Millennium Development Goals have placed maternal health at the core of the struggle against poverty and inequality, as a matter of human rights. Ten percent of women have high blood pressure during pregnancy, and preeclampsia complicates 2% to 8% of pregnancies. Preeclampsia can lead to problems in the liver, kidneys, brain and the clotting s

[Pregnancy, essential hypertension and chronic renal disease].

Women with chronic hypertension or renal disease are at a particular high risk of developing pre-eclampsia or eclampsia. Pre-eclampsia is associated with an increased risk of fetomaternal complications. In women with uncomplicated mild and moderate hypertension, pregnancy is usually normal. Treatment of high blood pressure aims at reducing maternal cardio- and cerebrovascular catastrophies, and the benefit of the treatment must be weighed against possible harmful effects on the foetus. In some cases, antihypertensive treatment can be discontinued, or medication changed. Preconceptional counsel

A study of an emerging hospital service: family planning in teaching hospitals.

This report presents the methodologic problems of a 1964 study of family planning assistance given in one midwestern metropolitan area in 20 hospitals that had obstetrics residencies; assesses the ability of administrators, obstetrics chiefs, and other staff members to estimate numbers and characteristics of patients served, in the absence of systematic records of family planning services; and discusses the nature, origin, and operation of policies on family planning assistance. The widespread lack of specific policies, other than negative policies in Catholic hospitals, resulted in great vari

Quality Improvement Opportunities Identified Through Case Review of Pregnancy-Related Deaths From Cardiovascular Disease.

OBJECTIVE: To analyze quality improvement opportunities (QIOs) identified through review of cases of maternal death from cardiovascular disease (CVD) by the California Pregnancy-Associated Mortality Review committee. DESIGN: Qualitative descriptive design using thematic analysis. SAMPLE: A total of 269 QIOs identified from 87 pregnancy-related deaths from CVD in California from 2002 to 2007. METHODS: We coded and thematically organized the 269 QIOs using three of the four domains commonly applied in quality improvement initiatives for maternal health care: Readiness, Recognition, and Respon

Maternal health and HIV.

The HIV/AIDS epidemic is one of the major factors affecting women's health, with 20 million women living with HIV and more than two million pregnancies in HIV-positive women each year. Most HIV infections in women are in resource-constrained settings where the risk of maternal morbidity and mortality is also unacceptably high, and where most of the 529,000 deaths from complications of pregnancy, childbirth and abortion occur annually. There is increasing evidence that HIV/AIDS-related maternal deaths are escalating considerably, and AIDS has overtaken direct obstetric causes as the leading cau

Reporting requirement type matters: associations between distinct reporting requirements and birth outcomes in the U.S.

BACKGROUND: In the context of the maternal overdose crisis and concerns that child welfare reporting is a barrier to care and treatment, current policy discussions in the U.S. focus on alternatives to child welfare reporting requirements. Thus, we examine associations between different state-level reporting requirements and birth outcomes and prenatal care utilization. METHODS: We use U.S. state-level policy data from the Alcohol Policy Information System and outcome data from vital statistics birth certificate records from 2000-2019 to examine relationships between three reporting requiremen

Review: Nutrition-based perinatal strategies towards antimicrobial-free systems in piglets.

The use of antimicrobials in pig production keeps increasing with the global demand for pork, causing a potential increase in the circulating pool of antimicrobial-resistant genes and other environmental issues. However, developing antimicrobial-free systems is challenging due to a negative impact on welfare and productivity losses. Current alternatives based on feed additives have shown limited success. Piglet gastrointestinal tract health is critical at early stages and depends on the rapid establishment of a balanced microbiota, influenced by sow-derived faecal transfers. The fasting associ

Specialised antenatal clinics for women with a multiple pregnancy for improving maternal and infant outcomes.

BACKGROUND: Regular antenatal care for women with a multiple pregnancy is accepted practice, and while most women have an increase in the number of antenatal visits, there is no consensus as to what constitutes optimal care. 'Specialised' antenatal clinics have been advocated as a way of improving outcomes for women and their infants. OBJECTIVES: To assess, using the best available evidence, the benefits and harms of 'specialised' antenatal clinics compared with 'standard' antenatal care for women with a multiple pregnancy. SEARCH METHODS: We searched the Cochrane Pregnancy and Childbirth Gr

[Cesarean section versus vaginal delivery--state-of-the-art paper].

AIM OF THE STUDY: The aim of the study was to perform a comparative analysis of complications after vaginal deliveries and cesarean sections and to introduce emotional status of the mother as an indication for a cesarean section. MATERIAL AND METHODS: 406 patients after vaginal deliveries and cesarean sections, that took place in the first quarter of 2009, were enrolled into the study. RESULTS: In the studied group of 406 patients, 200 women (49.2%) had cesarean section and 206 women (50.8%) delivered vaginally. In both groups there were no differences in the clinical state of newborns, whic
