Development and Validation of Machine Learning Models for Predicting Early Cognitive Decline Using Home Sensor-Derived Behavioral Data: Sensors in-Home for Elder Wellbeing (SINEW) Cohort Study.

BACKGROUND: As the global population continues to age, the prevalence of geriatric conditions, including dementia and frailty, is also increasing. Early identification of individuals at an elevated risk of these conditions, such as those presenting with mild cognitive impairment (MCI) or prefrailty, can provide a critical window for prompt intervention aimed at preventing or reversing disease progression. To promote such early identification, there is a burgeoning interest in the use of digital sensor technology and predictive modeling. OBJECTIVE: This study aimed to use a continuous, home-ba

The effect of age, weight, and lifestyle factors on calcaneal quantitative ultrasound: the ESOPO study.

Quantitative ultrasound (QUS) techniques have been shown to be as good as bone mineral density (BMD) assessed by dual-energy X-ray absorptiometry (DXA), in predicting fracture risk: QUS technique could increase substantially the accessibility to a reliable bone osteoporosis risk evaluation, but little is know regarding the relationship of QUS to risk factors that have been found to predict DXA-BMD values and this is even more true in men. We studied 6,811 postmenopausal women 40 to 80 years of age and 4,981 men 60-80 years of age representative of the general population of all regions of Italy

Machine learning allows robust classification of visceral fat in women with obesity using common laboratory metrics.

The excessive accumulation and malfunctioning of visceral adipose tissue (VAT) is a major determinant of increased risk of obesity-related comorbidities. Thus, risk stratification of people living with obesity according to their amount of VAT is of clinical interest. Currently, the most common VAT measurement methods include mathematical formulae based on anthropometric dimensions, often biased by human measurement errors, bio-impedance, and image techniques such as X-ray absorptiometry (DXA) analysis, which requires specialized equipment. However, previous studies showed the possibility of cl

Protocol for the internet-based lifestyle intervention to eradicate obese frailty in prostate cancer survivors (iLIVE) randomized controlled trial: a type I hybrid effectiveness implementation trial.

BACKGROUND: Obese frailty is a growing national problem as trends towards aging and obesity converge. This problem is especially acute in men with prostate cancer treated with androgen deprivation therapy (ADT). ADT is the backbone of therapy for advanced prostate cancer, a disease which affects mostly older men. But, ADT accelerates muscle loss and gains in adiposity which threaten physical functioning and quality of life. Diet and resistance training may mitigate obese frailty but have rarely been combined in an accessible delivery format. METHODS: The Internet-Based Lifestyle Intervention t

Are Females at Greater Risk for Revision Surgery After Hip Resurfacing Arthroplasty With the Articular Surface Replacement Prosthesis?

BACKGROUND: Female patients undergoing hip resurfacing arthroplasties may be at greater risk of revision surgery than males, but it is unclear whether this is related to sex or other factors. We focused our analysis on data from a prospective multicenter cohort study monitoring the ASR(TM) hip resurfacing arthroplasty prosthesis on the potential association of sex on patient-reported outcome measures (PROMs), metal ion levels, revision surgery, and presence of adverse local tissue reaction. As thousands of patients with the ASR(TM) prosthesis are still undergoing followup it is critical to opt

Early Lessons From a Worldwide, Multicenter, Followup Study of the Recalled Articular Surface Replacement Hip System.

BACKGROUND: Adverse local tissue reactions (ALTRs) around hip arthroplasties are an important reason for failure of metal-on-metal (MoM) hip implants. Little is known about capsular dehiscence patterns as ALTRs decompress from the hip into the surrounding tissue planes; these patterns may also influence the onset and severity of patient symptoms. QUESTIONS/PURPOSES: Through a multicenter study approach, we asked: (1) Is ALTR location related to the surgical approach used for arthroplasty in patients who underwent hip arthroplasty (resurfacing or THA) with a single, recalled hip arthroplasty s

A novel convolution-transformer model for differentiating hypertrophic cardiomyopathy from Phenocopies using cardiac MRI.

BACKGROUND AND OBJECTIVES: Hypertrophic Cardiomyopathy (HCM) is the most prevalent inherited cardiomyopathy. Its left ventricular hypertrophy (LVH) phenotype exhibits substantial radiological overlap with HCM phenocopies such as Hypertensive Heart Disease (HHD) and Cardiac Amyloidosis (CA), posing significant challenges to clinical differential diagnosis. Differentiating HCM from its phenocopies and normal hearts on cardiac MRI is an important but difficult task. This study leveraged a cardiovascular magnetic resonance foundation model to develop a deep learning approach that enables accurate

Adaptive servo-ventilation for sleep-disordered breathing in patients with heart failure with reduced ejection fraction (ADVENT-HF): a multicentre, multinational, parallel-group, open-label, phase 3 randomised controlled trial.

BACKGROUND: In patients with heart failure and reduced ejection fraction, sleep-disordered breathing, comprising obstructive sleep apnoea (OSA) and central sleep apnoea (CSA), is associated with increased morbidity, mortality, and sleep disruption. We hypothesised that treating sleep-disordered breathing with a peak-flow triggered adaptive servo-ventilation (ASV) device would improve cardiovascular outcomes in patients with heart failure and reduced ejection fraction. METHODS: We conducted a multicentre, multinational, parallel-group, open-label, phase 3 randomised controlled trial of peak-fl

Can smartwatches predict migraines? Using machine learning (ML) with wearable-derived nocturnal autonomic nervous system (ANS) and sleep metrics for headache prediction.

OBJECTIVE: To investigate whether nocturnal autonomic nervous system (ANS) activity and sleep metrics, as measured by a wearable device, can predict the occurrence of next-day migraine in patients with episodic and chronic migraine. BACKGROUND: The unpredictable nature of migraine episodes contributes to disease burden and limits effective application of tailored preventive strategies. Small-molecule calcitonin-gene-related peptide (CGRP) receptor antagonists, available in limited monthly quantities, are now used for both acute and preventative treatment of migraine. Consequently, improving t

Improving Access to Patient Education: an Audit of Extant Educational Materials.

Health information exchange between provider and patient, along with patient participation in their care (self-management), can lead to improved health outcomes. A step towards achieving better outcomes is the systematic provision of education materials to patients and caregivers throughout the cancer trajectory. An audit of patient education (PE) materials was conducted at a cancer center to identify content gaps and determine areas for future development. The PE audit was conducted in all outpatient clinics (13) and clinic-specific PE materials were identified, reviewed, and categorized by c
