[Global surgery-Challenges in the treatment of children with cleft lip and palate].

BACKGROUND: Cleft lip and palate is the most frequent malformation in humans that requires surgical correction but is not primarily life-threatening. That is why in many economically not very well developed countries, special surgical care, such as for cleft lip and palate, is not guaranteed at all or is not sufficiently guaranteed, so that numerous aid organizations have been founded for over 50 years to provide help by organizing surgical aid missions. Even if this help seems primarily ethically harmless and very laudable, the lack of rules and instructions unfortunately regularly leads to t

Medical emergency motorcycle--is it useful in a Scandinavian Emergency Medical Service?

BACKGROUND: Medical emergency motorcycles (MEM) can be used in time-critical conditions like cardiac arrest and multi-traumatized patients in an attempt to reduce the response time. Other potential benefits with MEM are more efficient patient evaluation, reduction of unnecessary EMS car ambulance missions and reduced cost. The potential benefits have been evaluated in this study. The incidence of accidents when operating the vehicle was also of interest. METHODS: A prospective study was performed when MEM was introduced as a trial in an urban ambulance service in Norway. RESULTS: A total of

Reasons why smartphone-alerted first responders abort missions: Findings from a sequential mixed-methods study.

BACKGROUND: Smartphone-based first responder systems are implemented in 209 regions across Germany to reduce time to treatment in out-of-hospital cardiac arrests (OHCA). However, more than 20% of alerted responders abort a mission after accepting the alert. Understanding the underlying reasons is important for improving the prehospital chain of survival. METHODS: We conducted a mixed-methods study comprising of (1) semi-structured interviews with smartphone-alerted first responders in Munich to identify and categorise reasons why missions are aborted, and (2) a subsequent quantitative online

Healthcare mission to a humanitarian disaster: a descriptive study of multidisciplinary interventions during the 2024 floods in Rio Grande do Sul, Brazil.

OBJECTIVE: Floods are the most common natural disasters worldwide, and their health impacts are exacerbated by climate change and population vulnerability. Humanitarian missions are essential for providing rapid and sustained medical care in disaster settings. However, evidence regarding the clinical and procedural aspects of multidisciplinary care during such missions remains limited. We examined the clinical profile and interventions provided during a humanitarian mission conducted in response to severe flooding in southern Brazil. METHODS: This retrospective observational study was conduct

Bashkirian train-gas pipeline disaster: the American military response.

The United States Army Institute of Surgical Research was asked to provide burn care assistance in June 1989 following the explosion of leaking methane/propane gas in the Central Soviet Union, which destroyed two passenger trains and injured 800 passengers. A 17-member burn team flew from San Antonio, Texas, to Ufa, USSR and assisted in the management of 150 burn patients in a general medical-surgical hospital. Early problems included heavily colonized burn wounds, with a microbial flora that demonstrated broad antibiotic resistance. As wound complications were controlled, 28 operative procedu

Embedded military medical support in high-risk police operations: Analysis of 242 operations.

BACKGROUND: The National Gendarmerie Intervention Group (GIGN) is the French National Gendarmerie's tactical unit, operating in high-risk environments involving armed suspects, hostage situations, or counterterrorism. Since 1984, the GIGN has relied on an original medical support model integrating a military physician-nurse team within its tactical elements. The study aimed to describe this support within the specific context of a physician-led prehospital emergency medical system. METHODS: We conducted a retrospective, descriptive, observational study of all GIGN operations identified from a

Probabilistic Risk Assessment of Prophylactic Surgery Before Extended-Duration Spaceflight.

Introduction. Prophylactic surgery before spaceflight may eliminate the risk of appendicitis and cholecystitis in astronauts on deep space missions. However, even minimally invasive surgery increases the risk of small bowel obstruction (SBO). Probabilistic risk assessment (PRA) is a method that can be used to estimate the benefits and risks of prophylactic surgery. Methods. Risks of appendicitis and cholecystitis during a 2.5-year Mars mission are compared to the risk of SBO after laparoscopic removal of the appendix, gallbladder, or both. A PRA model using Monte Carlo methodology was used to

A Qualitative Investigation of Space Exploration Medical Evacuation Risks.

INTRODUCTION: Exploration beyond low Earth orbit requires innovative solutions to support the crew medically, especially as the opportunity for timely evacuation to Earth diminishes. This includes assessing the risks and benefits that a complicated medical evacuation (MEDEVAC) poses to the injured crewmember, the crew, and the mission. This qualitative study identifies common MEDEVAC risk assessment principles used in spaceflight and other extreme environments to better inform future risk assessment tools and exploration mission concepts.METHODS: Semistructured interviews were conducted with s

Medical School: Academic or Corporate Culture?

Integration between academic medical schools and health systems occurs through mergers with either private or public health system models. Such mergers can result in significant institutional cultural conflicts between academia and the corporate world, which can affect the educational and scholarship missions of the schools. One approach to evaluate institutional culture is by The Competing Values Framework, through which distinct cultural characteristics and preferences can be determined and compared. However, I posit that this framework may not fully capture the differences between the acade

Public health model for Dental Specialty MEDRETE.

OBJECTIVE: Dental readiness training exercises constitute one form of a specialty medical readiness and training exercise (MEDRETE). Traditional dental missions have been a part of general MEDRETEs and have focused care on extractions and the provision of oral hygiene instructions. This article describes the tenets of a prevention-based dental humanitarian mission to Honduras in 2002. MATERIALS AND METHODS: The U.S. Army Southern Command requested a Dental Specialty MEDRETE for fiscal year 2002. A site visit revealed the absence of water fluoridation, high levels of dental disease, and a desi

Lessons from failure: neurosurgical outreach in Managua, Nicaragua.

With respect to the tremendous deficit in surgical care plaguing developing nations, it is critical that medical outreach models be organized in such a fashion that sustainable advancements can be durably imparted beyond the duration of targeted missions. Using a didactic framework focused on empowering host neurosurgeons with an enhanced surgical skillset, a mission was launched in Managua, Nicaragua, after previous success in Kiev, Ukraine, and Lima, Peru. Unfortunately, the failure to critically assess the internal and external state of affairs of the region's medical center compromised the

Psychological health maintenance on Space Station Freedom.

The scheduling of crew rotations for up to 180 days on Space Station Freedom presents a special challenge for behavioral scientists who are tasked with providing psychological support for the crews, their families, and mission flight controllers. Preflight psychological support planning may minimize the negative impact of psychological and social issues on mission success, as well as assist NASA management in making real-time mission planning decisions in the event of a significant social event (for example, the death of a family member). During flight, the combined psychological, emotional, a

'Leave no one behind': cochlear implantation under regional anaesthesia in a humanitarian setting - a case series.

BACKGROUND: Cochlear implantation (CI) is the standard of care for severe-to-profound hearing loss. While traditionally performed under general anaesthesia (GA), GA poses significant logistical and safety challenges during humanitarian missions in low-resource settings, often excluding patients with complex medical comorbidities. Regional anaesthesia (RA) offers a practical alternative that avoids GA-related risks, reduces operative time, and allows treatment for otherwise ineligible patients, upholding the humanitarian principle of 'leaving no one behind'. Therefore, the objective of the stud

Leveraging Federated Satellite Systems for Unmanned Medical Evacuation on the Battlefield.

This paper evaluates the role of federated satellite systems (FSSs) in enhancing unmanned vehicle-supported military medical evacuation (MEDEVAC) missions. An FSS integrates multiple satellite systems, thus improving imaging and communication capabilities compared with standalone satellite systems. A simulation model is developed for a MEDEVAC mission where the FSS control of an unmanned aerial vehicle is distributed across different countries. The model is utilized in a simulation experiment in which the capabilities of the federated and standalone systems in MEDEVAC are compared. The perform

Protocol of a randomized controlled trial examining psychosocial enhancement and standard medication treatment for co-occurring opioid use and mental health disorders: A half fractional factorial randomized controlled trial.

BACKGROUND: The opioid epidemic disproportionately affects individuals with co-occurring opioid use and mental health disorders (COD), who often have poor treatment engagement. Multicomponent treatment models are popular solutions to increase treatment access and engagement for those with COD. Maintaining Independence and Sobriety through Systems Integration, Outreach and Networking (MISSION) is a hybrid multicomponent linkage and treatment approach that provides assertive community outreach combined with psychosocial treatment. This protocol paper describes a randomized controlled trial compa
