Childhood immunizations: position on the enhanced inactivated poliovirus vaccine and live attenuated oral poliovirus vaccine dilemma.

Recent review of the polio vaccines (live attenuated oral poliovirus vaccine [OPV] and enhanced inactivated poliovirus vaccine [eIPV]) for children has generated much debate between infectious disease experts and public health officials. Poliomyelitis was a common medical condition in the 1940s and 1950s, and the success of OPV in eradicating poliomyelitis from the United States and even the Western hemisphere cannot be disputed. However, the adverse condition of vaccine-associated paralytic poliomyelitis (VAPP) has been reported in eight to nine cases per year as a result of exclusively using

Variations in provision and uptake of antenatal care.

Antenatal care providers vary from one country to another. In Europe, most of the care is provided by obstetricians in some countries, while the role of midwives is important in other countries. Many women are attended by general practitioners in Canada and, to a lesser extent, in the US. Involvement of traditional birth attendants in antenatal care in more than 5% of the pregnancies has been reported in Guatemala, Honduras and Mexico only. Several indicators may be used to measure the utilization of antenatal care: the percentage of women receiving care, timing of the first visit, proportion

From Barriers to Solutions: Establishing an Academic Pediatric Surgical Hub in an Underserved Region of Tunisia.

In low- and middle-income countries (LMIC), pediatric surgical care remains concentrated in urban tertiary centers, resulting in delayed access for rural populations. To address this inequity, we established an academic pediatric surgery department within a secondary-level regional hospital in Zaghouan, Tunisia, serving a broad underserved catchment area. Operating without dedicated pediatric infrastructure or pediatric anesthesiology services, we implemented a "service without walls" model by embedding pediatric surgery within an existing adult surgical department. Adult wards and perioperati

Establishment of an Alternate Care Site (ACS) in Imperial County During COVID-19.

Imperial County is in southern California, one of the state's two counties at the international United States-Mexico border. The county is one of the most resource-limited in the state, with only two hospitals serving its 180,000 citizens, and no tertiary care centers. A significant portion of the population cared for at the local hospitals commutes from Mexicali, a large city of 1.2 million persons, just south of Imperial County's ports of entry. Since May 2020, following an outbreak in Mexicali, Imperial County has seen a significant increase in the number of COVID-19 patients, quickly outpa

Tubal factor infertility in Benin City, Nigeria - sociodemographics of patients and aetiopathogenic factors.

In Africa, infertility constitutes a major gynaecological complaint and causes enormous socio-psychological stress to the patients. This study examined retrospective data at the University of Benin Teaching Hospital, Benin City, Nigeria, over a 5-year period to determine the factors associated with tubal infertility. Tubal infertility was confirmed in 13.5% of the 1181 new cases of infertility over the study period. The mean age of the patients was 33.2+/-9.5 years. Over 65% were nulliparous and all socioeconomic classes were affected. Major associated factors included infections such as post-

The Change Club intervention: 2-year impacts from a cluster-randomized trial in rural communities in New York and Texas.

INTRODUCTION: Obesity and related chronic diseases in the U.S., particularly within rural communities and/or medically underserved areas, are a major societal and cost burden. Built environments with health-promoting attributes, which can be influenced through civic engagement, are a potential pathway to help address this substantial problem. METHODS: We conducted a community-randomized delayed intervention trial in medically underserved rural communities (n = 12) in Texas and New York to evaluate the impacts of a civic engagement curriculum designed to create healthier physical activity and …

Impact of Limited Access to Surgical Care in Medically Underserved Communities.

Summary/BackgroundMedically underserved communities and ethnic minorities constitute a significant portion of the vulnerable population within the United States. Recent changes in the health care structure, rising inflation with a decline in median household income, and the SARS-CoV-2 pandemic have disproportionately impacted communities of low socioeconomic status. Healthcare providers and federal organizations must be aware of how these factors influence access to surgical care to tailor treatment, interventions, and policies better to meet the needs of these populations.MethodWe systematica

Evaluation of factors predicting transition from prediabetes to diabetes among patients residing in underserved communities in the United States - A machine learning approach.

INTRODUCTION: Over one-third of the population in the United States (US) has prediabetes. Unfortunately, underserved population in the United States face a higher burden of prediabetes compared to urban areas, increasing the risk of stroke and heart disease. There is a gap in the literature in understanding early predictors of diabetes among patients with prediabetes living in underserved communities in the United States. Hence, this study's objective is to identify factors influencing the transition from prediabetes to diabetes in rural or underserved communities using a machine learning appr

The Importance of Including Underserved Populations in Research.

This paper provides an overview of the ethical considerations surrounding the inclusion of underserved populations in later-phase clinical trials. Underserved populations, defined here as those with restricted access to or limited benefits from healthcare, often face systemic, logistical, and social barriers that limit their participation in research. This results in a lack of representation that undermines fairness in research and also hampers the development of effective inclusive healthcare practices. This paper argues that including underserved populations in research is crucial for promot

Improving genetics equity: identifying women eligible for genetic care services using mammography clinics in underserved areas as screening hubs.

PURPOSE: Fewer than 20% of underserved individuals undergo guideline-concordant hereditary breast and ovarian cancer (HBOC) genetic testing (GT). Our study aimed to determine the proportion of women eligible for HBOC GT using a cancer genetics risk assessment (CGRA) tool at breast cancer (BC) screening clinics in underserved communities and to describe the program's impact. METHODS: Participants were women who presented for BC screening at The Rose clinics, serving low-income underserved communities in southeast Texas, and completed the CGRA. High-risk individuals received bilingual education

Physiotherapists in under-resourced South African communities reflect on practice.

Increasing access to health services and the introduction of primary health care are the cornerstones of transforming healthcare initiatives in South Africa. Sustainability of healthcare provision in rural and under-resourced areas is secured by legislation of a year-long community service (CS) contract with graduating healthcare practitioners. This study explored how the first cohort of physiotherapists experienced their year-long CS and how they felt their undergraduate training prepared them for practice. A survey was conducted at the end of 2003 amongst the first cohort of CS physiotherapi

Avoidable maternal deaths: three ways to help now.

The current paper examines the realities of women delivering in resource-poor settings, and recommends cost-effective, scalable strategies for making these deliveries safer. Ninety-five percent of maternal deaths occur in poor settings, and the largest proportion of these deaths are women who deliver at home, far away from health care facilities, and without financial access to skilled providers. This situation will improve only when policymakers and programme planners refocus their attention on service delivery and financing interventions, with the potential to reach the largest portion of wo

[The concept of neglected disease].

"Neglected disease", "neglected population" and more generally "public health negligence" are emerging concepts being put forward by numerous humanitarian groups. Sleeping sickness provides a typical example to illustrate these concepts. After causing a major epidemic in the 1930s, sleeping sickness had been practically eradicated by the end of decolonization. Because of more urgent priorities, independent national governments relinquished control activities thus allowing the disease to return insidiously. By the beginning of the 1990s the situation was comparable to that prevailing in 1930 wi

Breast cancer screening in underserved women in the Bronx.

This article reports the results of mammography screening among socioeconomically disadvantaged women in Bronx, NY using a federally funded low-cost or no-cost cancer screening service. The New York State Department of Health provided funds for the uninsured through the Bronx Breast Health Partnership. All women < or = 40 years underwent screening mammography using both a mobile van unit and hospital-based mammographic x-ray unit, both American College of Radiology (ACR) accredited. Return visits were coordinated by a follow-up clinic at Montefiore Medical Center using a patient navigator who
