BACKGROUND: This study examines whether states implementing the 2014 ACA Medicaid expansion experienced prior changes in physician Medicare Part D prescribing patterns compared with non-expansion states, evaluating potential anticipatory behavioral adaptations.
RESEARCH DESIGN AND METHODS: Utilizing provider-year panel data from ProPublica's Medicare Part D Prescriber dataset (2011-2016), we implement an event-study difference-in-differences framework across a geographically restricted sample of adjacent expansion/non-expansion border counties. Key physician-level outcomes include total drug …

The Centers for Medicare & Medicaid Services uses the Star quality program to evaluate Medicare Advantage (Medicare Part C) care quality. Since the 21st Century Cures Act, ESRD enrollment within Medicare has shifted from <30% in 2021 with Medicare Advantage to more than 50% in 2024. Given this influx and growing CKD prevalence, evaluating how Stars measures shape kidney care has significant population health implications. The American Society of Nephrology Quality Committee convened a working group to assess the 2026 Medicare Part C and D Stars quality measures. Measures were reviewed using a

BACKGROUND: High and rising prescription drug costs for asthma and chronic obstructive pulmonary disease (COPD) contribute to medication nonadherence and poor clinical outcomes. The recently enacted Inflation Reduction Act includes provisions that will cap out-of-pocket prescription drug spending at $2,000 per year and expand low-income subsidies. However, little is known about how these provisions will impact out-of-pocket drug spending for Medicare beneficiaries with asthma and COPD.
OBJECTIVE: To estimate the impact of the Inflation Reduction Act's out-of-pocket spending cap and expansion

To understand the roles of physician associates [PAs] and nurse practitioners [NPs] in rheumatology care delivery, we examined the pharmacoepidemiology of disease-modifying antirheumatic drug [DMARD] prescriptions among Medicare beneficiaries. The intent was to inform regulatory decisions, optimize prescribing, and better manage risk in older populations. An observational, retrospective cohort study of Medicare Part D claims from 2013 to 2023 compared rheumatologist prescribers with primary care providers (family practice [FP] and internal medicine [IM]) and with PAs and NPs. Trends in DMARD p…

Today, approximately 50% of patients eligible for Medicare have opted for Medicare Advantage as their primary coverage. Whereas Medicare Advantage is a reasonable option for healthy senior Americans, issues arise once they have serious or chronic medical problems, which are prevalent among older Americans. This review details the pros and cons of standard Medicare vs Medicare Advantage. The authors recommend considering standard Medicare as a better form of insurance coverage. In addition, patients should also enroll in Medicare Part D to get prescription drug coverage; buy a supplemental Medi…

BACKGROUND/OBJECTIVES: Psychotropic and opioid medications are routinely prescribed for symptom management in hospice, but national estimates of prescribing are lacking. Changes in Medicare hospice payment in 2014 provide the first opportunity to examine psychotropic and opioid prescribing among hospice beneficiaries, and the factors associated with use of specific medication classes.
DESIGN: Cross-sectional analysis of a 20% sample of traditional and managed Medicare with Part D enrolled in hospice, 2014-2016.
SETTING: Beneficiaries enrolled in the Medicare hospice benefit.

BACKGROUND: Veterans enrolled in both Veterans Affairs (VA) health care and Medicare Part D can choose to obtain medications through the VA, Medicare Part D, or both, with each option differing in cost, coverage, and coordination of care. Poorly informed choices can lead to veteran frustration when their expectations are not met, delays in medication access, and increased risks. This study aimed to develop a decision aid (DA) to help veterans with diabetes make informed choices about medication sourcing (ie, whether to fill medications through VA health care only, Part D only, or both).
