Dietary mimicry of melena following chocolate cookie ingestion in a patient with colostomy: a case report.

BACKGROUND: Melena is dark, tarry stools that typically indicate upper gastrointestinal bleeding. However, various dietary substances can alter stool color and potentially lead to diagnostic confusion. We present a rare case of stool discoloration caused by the ingestion of chocolate sandwich cookies. CASE PRESENTATION: A Black man in his 30s with a transverse colostomy presented with sudden-onset dark, tarry stool, raising concern for melena. Initial management included intravenous proton pump inhibitors and consideration of urgent endoscopy. However, the patient's laboratory results and ove…

Hepatosplenic Schistosomiasis Presenting as Melena in an Adolescent Filipino Male: A Case Report and Literature Review.

Schistosomiasis, a snail-borne disease caused by infection with a trematode parasite of the genus Schistosoma, is one of the most neglected tropical diseases in the world. One of its rare complications is hepatosplenic schistosomiasis which ultimately leads to fibrosis and presinusoidal portal hypertension. We report a case of a 13-year-old Filipino male from Quezon City with previous one year residence in the endemic island of Leyte, presenting with melena. Diagnostic work-up revealed hepatosplenomegaly and periportal fibrosis with multiple hepatic nodules on ultrasound, positive Schistosoma …

Adenocarcinoma in the Jejunum Presenting as Acute Abdomen and Melena in an Otherwise Healthy Patient: A Case Report.

A jejunal adenocarcinoma is a rare form of cancer that is difficult to diagnose due to its vague and non-specific symptoms, often leading to delayed treatment and poor outcomes. In this case, we report a 43-year-old male who presented with acute abdominal pain, weight loss, and melena. A CT scan revealed a mass in the small intestine, and subsequent exploratory laparotomy confirmed the presence of a tumor, which was surgically excised. Histopathological analysis confirmed jejunal adenocarcinoma with positive CDX2 staining, ruling out other adenocarcinoma subtypes. This case underscores the imp…

Jejunal Gastrointestinal Stromal Tumor Presenting as Life-Threatening Melena and Profound Anemia: A Case Report.

Gastrointestinal stromal tumors (GISTs) are rare mesenchymal neoplasms and the most common sarcomas of the gastrointestinal tract. Small bowel GISTs, particularly jejunal tumors, frequently present with occult or overt bleeding leading to anemia, but are uncommon causes of melena when routine endoscopy has not been performed. We report a 67-year-old man who presented with one week of intermittent melena and profound anemia (hemoglobin 5.3 g/dL). Computed tomography (CT) angiography revealed a 5 cm jejunal mass without active bleeding. The patient underwent exploratory laparotomy with segmental…

[A Cure Case of Rectal Arteriovenous Malformation with a Large Volume of Melena Caused by Surgery for Rectal Cancer].

This study presents the case of a 52-year-old male suffering from extensive melena. A detailed examination using lower gastrointestinal endoscopy revealed a polyp at the lower rectum; subsequently, endoscopic mucosal resection(EMR)was performed. No other lesion for the melena was observed. Histopathologic findings of the EMR confirmed adenocarcinoma in adenoma, indicating the need for additional surgery. Preoperative contrast-enhanced CT revealed an arteriovenous malformation( AVM)of about 5 cm in the rectal wall at the anal side of the lower rectal cancer. Extensive melena was inferred to be …

Jejunal arteriovenous malformation and multiple acquired extrahepatic portosystemic shunts in a juvenile dog, presenting with melena.

A juvenile dog referred with a 1-month history of persistent melena and severe anaemia, was diagnosed with a jejunal arteriovenous malformation, and multiple acquired extrahepatic portosystemic shunts. A midline coeliotomy was performed, the jejunal arteriovenous malformation was localised intraoperatively and was successfully removed via an enterectomy. Histopathology confirmed a true arteriovenous malformation. Despite the initial improvement, the patient developed seizure episodes secondary to hepatic encephalopathy 8 months after surgery. Fifteen months after surgery, the owner opted for e…

Refractory Iron-deficiency anemia and intermittent melena in phenotypic type 2 Glanzmann thrombasthenia: a diagnostic challenge.

Glanzmann thrombasthenia (GT) is a rare platelet disorder caused by quantitative or qualitative defects of glycoprotein (GP) IIb/IIIa, leading to impaired platelet aggregation[1]. Classic hereditary GT, when molecularly confirmed, typically manifests with lifelong mucocutaneous bleeding, whereas gastrointestinal bleeding is uncommon and usually presents as overt hemorrhage. Here we report a case of phenotypic type 2 GT characterized predominantly by iron-deficiency anemia and intermittent melena. GT was considered a potential contributing factor based on systematic diagnostic reasoning. This c…

An unusual cause for Melena: The role of endoscopic mucosal resection in a rare case of the duodenal Brunner's gland hamartoma.

Brunner's gland hyperplasia (also referred to as adenoma or hamartoma) is a rare, benign proliferative lesion of the duodenum. Although often asymptomatic, it can occasionally present with gastrointestinal (GI) bleeding, anaemia or obstructive symptoms. We report a case of a 65-year-old woman who presented with melena and symptomatic anaemia. Upper GI (UGI) endoscopy revealed multiple sessile polyps in the duodenum, with the largest measuring 2 cm. Given her history of a recent myocardial infarction and ongoing antiplatelet therapy, an endoscopic approach was chosen. Endoscopic mucosal resecti…

[A case of melena and perforation during chemotherapy for peripheral T-cell lymphoma, not otherwise specified of the small intestine].

A 71-year-old male presented with gastrointestinal bleeding. Double-balloon endoscopy revealed diffuse thickening of the wall and a shallow ulcer measuring approximately 10cm in the mid jejunum. Biopsy revealed diffuse proliferation of large lymphoid cells that were immunohistochemically positive for CD3, CD5, and bcl-2 but negative for CK, CD10, CD20, and bcl-6. The cyclin D1 labeling index was 3% and the MIB-1 labeling index was 61%. On the basis of these findings, he was diagnosed with peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS) of the small intestine. Following four cour

Symptoms in patients with peptic ulcer and hematemesis and/or melena related to the use of non-steroid anti-inflammatory drugs.

One hundred and seven consecutive patients with hematemesis and/or melena and a diagnosis of duodenal, gastric, or esophageal ulcers were interviewed immediately before or after endoscopy about the use of non-steroid anti-inflammatory drugs (NSAIDs) and symptoms before the hemorrhage. If the patients admitted no symptoms of abdominal pain or discomfort, nausea, vomiting, or heartburn, they were classified as having no ulcer symptoms before the hemorrhage. Patients who had not taken NSAIDs during the last 48 h before the hemorrhage were classified as not having taken NSAIDs. Significantly fewer…

Hidden ruptured internal carotid artery aneurysm causing occult epistaxis, melena, syncope, and hemodynamic collapse: a case report.

BACKGROUND: Life-threatening epistaxis as the initial manifestation of an internal carotid artery aneurysm is rare and diagnostically challenging, particularly in the absence of trauma or surgical history. Rupture into the sphenoid sinus may mimic gastrointestinal or neurological emergencies, delaying appropriate vascular intervention. CASE PRESENTATION: We report the case of a 71-year-old French Canadian woman with a history of hypertension and aspirin use who presented with recurrent syncope, melena, visual disturbances, and intermittent epistaxis. She was initially found to have profound a…
