Treatment of metatarsalgia by sliding distal metatarsal osteotomy.

A consecutive series of 113 patients who had distal sliding metatarsal osteotomy performed between 1976 and 1983 at Winford Orthopaedic Hospital were reviewed. All patients originally had symptoms and signs of pressure metatarsalgia. A total of 124 feet in 94 patients were available for assessment. Review was performed using a symptomatic scoring system, clinical examination, AP and lateral standing radiographs, and walking foot pressure studies obtained from a Harris-Beath mat. The mean follow-up period was 3 years and 4 months (range nine to 102 months). Symptomatically, 58 feet (47%) were r…

[Metatarsalgia caused by compression of the internal plantar nerve].

The Author has presented a rare form of internal plantar nerve compression that was produced by a synovial cystic formation, which caused a plantar pain during the load that radiated to the first three toes. Besides he has considered the other possible causes of metatarsus pain for alterations of soft parts.

Central Metatarsalgia and Walking on Pebbles: Beyond Morton Neuroma.

OBJECTIVE: Central metatarsalgia relates to abnormalities of the second, third, and fourth metatarsals and their respective metatarsophalangeal joints. A variety of disorders present with central forefoot pain; they range from traumatic lesions (acute or chronic repetitive), inflammatory and infective disorders, nonneoplastic soft-tissue lesions, and benign tumors to malignant lesions. Patients often present with symptoms of localized pain in the forefoot that worsens on weight bearing (walking or running), which can be sharp or dull and often is perceived as a lump felt inside or underneath t…

[Morton metatarsalgia. Results of surgical treatment in 54 cases].

This syndrome, which involves nerve compression, is probably often overlooked and is, therefore, more frequent than supposed. It is characte … isorder with depressive features, e.g. increased nocturnal suffering which is atypical for Morton's metatarsalgia. It is concluded that the accuracy of diagnosis according to strict criteria is decisive for surgical outcome.

[Management of metatarsalgia using Helal's method for metatarsal osteotomy].

Authors report on the result of Helal's metatarsal osteotomies performed on 62 feet of 48 patients for metatarsalgia. In 85 per cent the result proved to be excellent, in 15 per cent there was no improvement. Analysing the causes of the postoperative complaints they call attention to the overload of the marginal arches and to the metatarsalgia developing on the non osteotomized neighbouring arch. These complaints appeared in 1/3 of their material and could be generally well influenced with conservative treatment.

Diagnostic and Therapeutic Challenge of Metatarsalgia in a Patient With Rheumatoid Arthritis.

A 63-year-old female patient, with a past history of rheumatoid arthritis, presented with insidious pain on the left foot second and third metatarsophalangeal joints, associated with swelling and morning stiffness (mean time: four hours). Physical examination evidenced a tender and soft nodularity in the third intermetatarsal space, along with sharp pain, consistent with Morton's neuroma. Foot ultrasound suggested Morton's neuroma, but not excluding the possibility of rheumatoid arthritis involvement. Foot magnetic resonance imaging suggested the possibility of extensive synovitis of the third

The impact of body mass index on metatarsalgia surgical outcomes.

INTRODUCTION: Several studies have suggested that an increased body mass index (BMI) is a negative factor for forefoot plantar pain but its influence in the surgical correction of metatarsalgia is unknown. The purpose of the present study is to evaluate the influence of the BMI on the surgical outcomes of metatarsalgia. It has been hypothesized that the higher the BMI, the worse the functional outcomes after metatarsalgia surgical treatment at one year follow-up. MATERIAL AND METHODS: A prospective cohort study that included all patients operated on for third rocker metatarsalgia was conducte…

Forefoot Complaints - the Morton's Metatarsalgia. The Role of MR Imaging.

An illustrated case with Morton's metatarsalgia is presented. MR imaging was helpful for determining the presence, location and magnitude of intermetatarsal neuroma. Moreover, it had a large influence on the differential diagnostic thinking because many disorders may produce forefoot complaints mimicking Morton's metatarsalgia.

Considerations in metatarsalgia and midfoot pain: an MR imaging perspective.

There are many conditions that have the potential to produce the symptom of pain in the metatarsal and midfoot regions. Pathologic processes that cause pain include post-traumatic disorders, circulatory disorders, arthritides, neuroarthropathy, and conditions that result in a biomechanical imbalance in the foot. Magnetic resonance (MR) imaging is a powerful imaging tool since it provides a noninvasive method for diagnosis of different foot disorders. In this article, we will discuss the MR manifestation of a number of pathologic entities that elicit foot pain as a primary symptom.

Results of excision of the interdigital nerve in the treatment of Morton's metatarsalgia.

Thirty-one patients (thirty-two feet) had excision of the interdigital nerve as treatment of their Morton's metatarsalgia. A longitudinal dorsal incision was used in all cases. Twenty-five out of 32 cases had a macroscopically visible neuroma, and only two had no evidence of a neuroma on histological examination. All thirty-two patients were available for follow-up at an average of 44.7 months (range 14 to 71 months) postoperatively. Eighty-one per cent of the patients had a good or excellent result, 12.5% had a fair result, with residual pain and some restriction of activities, and 6.5% had n…

Metatarsalgia caused by an increase in circulating platelets: a case report.

A patient with a year-long history of metatarsalgia was found to have an elevated platelet count due to essential thrombocythemia, a benign myeloproliferative disease. Reduction of the platelet count with chemotherapy eliminated the foot pain. The authors recommended that a platelet count be part of the work-up of patients with metatarsalgia when the etiology of the foot pain is unclear.

A problem-based approach in musculoskeletal ultrasonography: central metatarsalgia.

Ultrasonography (US) is a useful diagnostic method that can be easily applied to identify the cause of metatarsalgia. The superficial location of structures in the foot, dynamic capability of US, and the ability to perform direct real-time evaluations of the pain site are also strong advantages of US as a modality for examining the foot. Moreover, knowing the possible pain sources to investigate when a patient has a specific site of pain will enhance the diagnostic quality of US, and will help radiologists to perform US efficiently and effectively. The purpose of this article is to review the …

Metatarsal osteotomy for primary metatarsalgia: radiographic and pedobarographic study.

The effect of surgical procedures for primary metatarsalgia on the load distribution and bony architecture of the forefoot is poorly understood. A prospective study was performed on 45 feet with this diagnosis treated by dorsal wedge osteotomy and compared with 29 symptom-free contralateral feet. Each foot was evaluated preoperatively and postoperatively with quantitative radiographic analysis, a pedobarographic study, and by physical examination. Following osteotomy there was a 4.5-mm increase in average height from ground and a 7.0 psi decrease in pressure. The symptom-free control group dem…
