Two biproducts of the revolution in diagnostic imaging techniques are unintended discoveries, and uncertainty for the patient and the clinician. To address the uncertainty associated with adrenal incidentalomas, clinicians need to understand the definition, differential diagnosis, and options for assessment with respect to functional status and malignancy potential. This article presents an algorithmic approach that addresses these issues.

BACKGROUND AND PURPOSE: There is scant information available on the incidence of transient ischemic attack (TIA) in a defined population. This study defines incidence rates of first TIA and subtypes of TIA during 1985-1989 and compares the incidence to that obtained from a 1960-1972 cohort study.
METHODS: Medical records of all residents of Rochester with potential diagnosis of TIA during 1985-1989 were screened to determine whether the case met the criteria for TIA. The cumulative incidence of mental retardation in a birth cohort of children born from 1976 through 1980 in Rochester, Minnesota (n = 5,919) was estimated. Rochester is the site of the Rochester Epidemiology Project, which captures virtually all medical care delivered locally. OBJECTIVE: To investigate trends in evaluation and management of patients with cCMV at a major pediatric center following the institution of universal cCMV screening.
STUDY DESIGN: Retrospective cohort study.
SETTING: Tertiary academic hospital.
METHODS: Patients <18 years with a diagnosis of cCMV between January 2021 and February 2024 were examined. Three cohorts were analyzed: 2021 and earlier (before screening guidelines), 2022 (screening recommended but not mandated), and 2023 and later (universal screening). In July 2023, the Minnesota Department of Health (MDH) was notified of possible occupational exposures to anthrax during an outbreak in animals. In consultation with the Centers for Disease Control and Prevention, MDH epidemiologists created a questionnaire that assessed exposure risks and helped determine individual illness monitoring and antibiotic post-exposure prophylaxis needs. This investigation and the resources developed for it could be useful in future scenarios where there are occupational exposures to naturally occurring anthrax.

PURPOSE: To determine the population-based incidence and disease associations of episcleritis and scleritis in a midwestern U.S.
DESIGN: Population-based retrospective cohort study.
METHODS: All residents of Olmsted County, Minnesota from 2006 to 2015 were identified by using the Rochester Epidemiology Project. A total of 223 episcleritis cases and 77 scleritis cases were identified. The transmission dynamics of Anaplasma phagocytophilum (Ap) and Borrelia burgdorferi (Bb) among Ixodes scapularis (Is) and mammalian hosts was investigated at Camp Ripley, an area representative of central Minnesota. Prevalence of white-footed mouse infection with Ap and Bb were 20% and 42%, respectively, with a coinfection level of 14%. Peak levels of infection with both agents occurred in May. The objectives of the present study were to investigate the pathogenesis of a recent isolate of avian metapneumovirus (aMPV) in turkeys and to evaluate the quantitative distribution of the virus in various tissues during the course of infection.
